In order to comprehend the impact of globalization on local food systems, it is crucial to consider sociohistorical, socioeconomic, and sociocultural trajectories, accompanied by long-term and cross-sectional monitoring. To achieve this objective, it is necessary to develop research protocols that enable the comparative evaluation of diets from the perspective of dietetics and nutrition, as well as local representations of food. Within the framework of an interdisciplinary and international OHM (Human-Environment Observatories) research network, a multidisciplinary team of researchers specializing in ethnoecology, health, nutrition, ecotoxicology, anthropology, and sociology was assembled. The network’s role is to conduct long-term studies on human-influenced ecosystems that are susceptible to socio-ecosystemic crises, such as those related to food and health. The consortium comprised researchers working within five OHMs, namely Estarreja (Portugal), Téssékéré (Senegal), Littoral-Caraïbes (Guadeloupe, France), Oyapock (French Guiana, France), and Nunavik (Québec, Canada), which focus on five distinct socio-ecosystems. Results: A cross-sectional data collection protocol was developed, consisting of a two-part questionnaire. Part 1 involves a structured 24 h dietary recall (24HR) that deviates from standard 24 h questionnaires by excluding portion sizes, instead focusing on food acquisition strategies and the degree of food item transformation. Part 2 encompasses a semi-structured interview guide that explores the concept of "eating well," barriers and facilitators to achieving it, changes in diet and dietary habits, and the connection between diet and health. This questionnaire captures, in a single session, the food items that individuals consumed the previous day, including their origin and level of transformation, as well as the associated perceptions regarding those food items and the overall diet. This approach enables the collection of data that facilitate the assessment of factors influencing diet from both the researchers’ point of view (i.e., etic perspective) and that of local populations (i.e., emic perspective). The questionnaire thus adopts a holistic approach, enabling us to analyze the links that populations establish between the socio-ecosystemic crises they have undergone (or are currently undergoing), their health and the evolution of their food systems. Globalization is transforming food systems around the world. With few geographical areas spared from nutritional, dietary and epidemiological transitions, chronic diseases have reached pandemic proportions. A question therefore arises as to the sustainability of local food systems. The overall purpose of this article is to put in perspective how local food systems respond to globalization through the assessment of five different case studies stemming from an international research network of Human-Environment Observatories (OHM), namely Nunavik (Québec, Canada), Oyapock (French Guiana, France), Estarreja (Portugal), Téssékéré (Senegal) and Littoral-Caraïbes (Guadeloupe, France). Each region retains aspects of its traditional food system, albeit under different patterns of influence modelled by various factors. These include history, cultural practices, remoteness and accessibility to and integration of globalized ultra-processed foods that induce differential health impacts. Furthermore, increases in the threat of environmental contamination can undermine the benefits of locally sourced foods for the profit of ultra-processed foods. These case studies demonstrate that: (i) the influence of globalization on food systems can be properly understood by integrating sociohistorical trajectories, socioeconomic and sociocultural context, ongoing local environmental issues and health determinants; and (ii) long-term and transverse monitoring is essential to understand the sustainability of local food systems vis-à-vis globalization. Priorities, Barriers, and Facilitators towards International Guidelines for the Delivery of Supportive Clinical Care during an Ebola Outbreak: A Cross-Sectional Survey

During the Ebola outbreak, mortality reduction was attributed to multiple improvements in supportive care delivered in Ebola treatment units (ETUs). We aimed to identify high-priority supportive care measures, as well as perceived barriers and facilitators to their implementation, for patients with Ebola Virus Disease (EVD). We conducted a cross-sectional survey of key stakeholders involved in the response to the 2014–2016 West African EVD outbreak. Out of 57 email invitations, 44 responses were received, and 29 respondents completed the survey. The respondents listed insufficient numbers of health workers (23/29, 79%), improper tools for the documentation of clinical data (n = 22/28, 79%), insufficient material resources (n = 22/29, 76%), and unadapted personal protective equipment (n = 20/28, 71%) as the main barriers to the provision of supportive care in ETUs. Facilitators to the provision of supportive care included team camaraderie (n in agreement = 25/28, 89%), ability to speak the local language (22/28, 79%), and having treatment protocols in place (22/28, 79%). This survey highlights a consensus across various stakeholders involved in the response to the 2014–2016 EVD outbreak on a limited number of high-priority supportive care interventions for clinical practice guidelines. Identified barriers and facilitators further inform the application of guidelines. Full article
