Mediating Effect of Sleep Quality on the Association Between Psychological Stress and Acne Vulgaris in Chinese College Students: A Cross-Sectional Study
Abstract
1. Introduction
- Higher level of psychological stress is independently associated with increased risk of acne vulgaris in college students, after adjusting for demographic and lifestyle covariates.
- Sleep quality mediates the association between psychological stress and acne, such that stress impairs sleep quality, which in turn increases acne risk.
- Screen time mediates the association between psychological stress and acne, such that stress increases daily screen time, which in turn increases acne risk.

2.2. Data Collection and Measurement Tools
2.3. Study Variables
2.3.1. Outcome Variable: Acne Prevalence
2.3.2. Exposure Variable: Psychological Stress Assessment
2.3.3. Mediating Variables
- (1)
- Sleep Quality: The Pittsburgh Sleep Quality Index (PSQI) was used to assess participants’ sleep quality over the preceding month [21]. The scale comprises 19 self-rated items grouped into 7 dimensions: subjective sleep quality, sleep latency, sleep duration, habitual sleep efficiency, sleep disturbances, use of sleep medication, and daytime dysfunction. Each dimension is scored from 0 to 3, with a total global score ranging from 0 to 21 (calculated as the sum of the 7 dimension scores). Higher total scores indicate poorer sleep quality. In the present study, the Cronbach’s α coefficient of the PSQI was 0.91, indicating good internal consistency reliability in the study cohort.
- (2)
- Screen Time: Assessed as the average daily duration of electronic device use (in hours).
