- Tumours with focal or moderate-to-severe multifocal atypia, without cell tumour necrosis and a mitotic count less than or equal to 10 mitotic figures per field.
- Tumours with moderate-to-severe diffuse atypia and a mitotic count of less than 10 mitotic figures per field.
- Tumours with cell necrosis, mild or absent atypia, and a mitotic count less than 10 mitotic figures per field.
- Tumours without cell necrosis, mild or absent atypia, and a mitotic count greater than or equal to 15 mitotic figures per field.
