- Threshold variability: low- and high-avidity cutoffs vary across commercial assays;
- Timing effect: test timing can critically influence negative predictive value; intermediate-to-high values after 21 weeks of pregnancy do not exclude primary infection, whereas high IgG avidity in the first trimester correctly identifies past infection;
- Prolonged low avidity: unusually persistent low-avidity IgG (lasting more than 18 weeks) may lead to misdiagnosis of primary CMV infection, especially when CMV IgM is also detected.
