- Hemodynamic instability: systolic blood pressure < 90 mmHg or the need for continuous vasoactive support to maintain SBP ≥ 90 mmHg.
- Shock: hemodynamic instability accompanied by signs of hypoperfusion, including altered mental status, oliguria < 0.5 mL/kg/h, or serum lactate > 2 mmol/L.
- Neurological deficit: new focal neurological impairment at presentation or transient loss of consciousness attributable to suspected cerebral malperfusion.
- Respiratory failure requiring intubation: requirement for invasive mechanical ventilation prior to surgical induction due to hypoxemia (SpO2 < 90% despite supplemental O2), airway compromise, or reduced consciousness.
