Work-Related Low Back Pain and Psychological Distress Among Physiotherapists in Saudi Arabia: A Cross-Sectional Study

Background: Musculoskeletal disorders significantly affect healthcare professionals, particularly physiotherapists, due to the physical demands of their work. The link between physical ailments and psychological distress is especially prominent in clinical settings. Objectives: To assess the prevalence of work-related low back pain (LBP), stress, anxiety, and depression among physiotherapists in Saudi Arabia, and to identify associated local risk factors. Methods: A cross-sectional study using convenience sampling included 710 licensed physiotherapists across Saudi Arabia. Participants completed an online survey containing demographic data and the validated measures, including the Visual Analog Scale (VAS) for pain, the Oswestry Disability Index (ODI), and the Depression, Anxiety, and Stress Scale-21 (DASS-21) for psychological distress. Data were analysed using descriptive statistics, chi-square tests, correlation, and regression analyses. Results: Of 710 responses, 697 were valid; 378 physiotherapists reported work-related LBP. The mean pain intensity was 4.6 (SD = 1.6), with 54.2% experiencing moderate to severe disability. Mental health results showed 49.7% had depressive symptoms and 33.9% experienced some level of anxiety. Significant correlations were observed between disability and psychological distress (anxiety: r = 0.382; depression: r = 0.375; stress: r = 0.406; all p < 0.001). Regression analyses indicated psychological distress significantly predicted disability, with R2 values ranging from 0.125 to 0.248, being higher among inpatient physiotherapists. Conclusions: This study reveals a high prevalence of LBP and psychological distress among Saudi physiotherapists, with stress being the strongest predictor of LBP severity. Integrated ergonomic and mental health interventions, including workplace wellness programs and psychological support, are recommended to reduce risks and promote a healthier, more sustainable physiotherapy workforce. Background: Accelerated demographic aging in Hungary and across Europe presents significant public health and socioeconomic challenges, particularly in preserving cognitive function and preventing neurodegenerative diseases. Modifiable lifestyle factors—especially dietary habits—play a critical role in brain aging and cognitive decline. Objective: This narrative review explores the mechanisms by which Western dietary patterns contribute to cognitive impairment and neurovascular aging, with specific attention to their relevance in the Hungarian context. It also outlines the rationale and design of the Semmelweis Study and its workplace-based health promotion program targeting lifestyle-related risk factors. Methods: A review of peer-reviewed literature was conducted focusing on Western diet, cognitive decline, cerebrovascular health, and dietary interventions. Emphasis was placed on mechanistic pathways involving systemic inflammation, oxidative stress, endothelial dysfunction, and decreased neurotrophic support. Key findings: Western dietary patterns—characterized by high intakes of saturated fats, refined sugars, ultra-processed foods, and linoleic acid—are associated with elevated levels of 4-hydroxynonenal (4-HNE), a lipid peroxidation product linked to neuronal injury and accelerated cognitive aging. In contrast, adherence to Mediterranean dietary patterns—particularly those rich in polyphenols from extra virgin olive oil and moderate red wine consumption—supports neurovascular integrity and promotes brain-derived neurotrophic factor (BDNF) and nerve growth factor (NGF) activity. The concept of “cognitive frailty” is introduced as a modifiable, intermediate state between healthy aging and dementia. Application: The Semmelweis Study is a prospective cohort study involving employees of Semmelweis University aged ≥25 years, collecting longitudinal data on dietary, psychosocial, and metabolic determinants of aging. The Semmelweis–EUniWell Workplace Health Promotion Model translates these findings into practical interventions targeting diet, physical activity, and cardiovascular risk factors in the workplace setting. Conclusions: Improving our understanding of the diet–brain health relationship through population-specific longitudinal research is crucial for developing culturally tailored preventive strategies. The Semmelweis Study offers a scalable, evidence-based model for reducing cognitive decline and supporting healthy aging across diverse populations. Full article

Background. Healthcare workers’ health can be influenced by physical, psychological, social, emotional, and work-related stress. MountainTherapy Activities (MTAs) are an integrated therapeutic approach that uses nature to enhance their well-being through group activities like hiking. This cross-sectional study examines well-being levels among Italian Departments of Mental Health workers who do or do not participate in MTAs. It hypothesizes that MTAs may reduce burnout, boost psychological resilience, and increase job satisfaction. Methods. The study involved 167 healthcare workers from 11 Italian Local Health Authorities, divided into MTA (who take part in MTA; n = 83) and non-MTA (who have never participated in MTA; n = 84) groups. They completed five validated questionnaires on psychological distress, burnout, resilience, job engagement, and psychological safety. Data were compared between groups, considering MTA frequency and well-being differences during MTAs versus workplace activities. Results. MTA participants scored higher in psychological well-being (t(117.282) = −1.721, p = 0.044) and general dysphoria (t(116.955) = −1.721, p = 0.042). Additionally, during MTAs, they showed greater job engagement (vigor: t(66) = −8.322, p < 0.001; devotion: t(66) = −4.500, p < 0.001; emotional involvement: t(66) = −8.322, p = 0.002) and psychological safety (general: t(66) = −5.819, p < 0.001; self-expression: t(66) = −5.609, p < 0.001) compared to other activities. Conclusions. MTAs can be considered a valid intervention for the promotion of the mental health of healthcare workers. Full article

Background/Objectives: Although eating disorders (EDs) affect a large portion of the population and have a significant impact on health and productivity, they are understudied in the workplace. We assessed the frequency of EDs and studied the relationship between EDs and occupational and individual factors. Methods: All workers undergoing health surveillance were invited to fill in the Eating Disorder Examination Questionnaire, short form (EDE-QS) and, before their routine medical examination that included metabolic tests, measure their level of health literacy, stress, quality of sleep, anxiety, depression, and happiness. Out of a total of 2085 workers, 1912 (91.7%) participated. Results: Suspected EDs affected 4.9% (CI95% 3.9; 5.9) of workers, with no notable difference in gender (5.3% CI95% 4.1; 6.7 in female workers vs. 4.2%, CI95% 2.9; 5.9 in male). Cases were significantly associated with trauma and emotional factors (anxiety, depression, unhappiness), but also with work-related stress and poor sleep quality, and negatively associated with health literacy. Using a hierarchical logistic regression model, suspected cases of EDs were significantly predicted in Model II by life trauma (OR 2.21 CI95% 1.40; 3.48, p < 0.001) and health literacy (OR 0.94 CI95% 0.90; 0.98, p < 0.001), in Model III also by work-related stress (OR 2.57 CI95% 1.68; 3.94, p < 0.001), and in Model IV by depression (OR 1.19 CI95% 1.02; 1.38, p < 0.05) and happiness (OR 0.88 CI95% 0.78; 0.99, p < 0.05). An association was also found between EDs and overweight, obesity, increased abdominal circumference, hypercholesterolemia, hypertriglyceridemia, hyperglycemia, arterial hypertension, atherogenic index of plasma, and metabolic syndrome. Conclusions: The workplace is an ideal setting for the prevention of EDs and their consequences. Occupational health intervention should promote health literacy, improve sleep quality, and reduce work-related stress. Full article

The aim of this work is to explore and understand the lived experience of nurses who chose to schedule visits with an Emotional Support Animal (ESA), i.e., a dog, during their working shift. Background/Objectives: Nursing practice is rigorous, weighted with intense responsibility that creates an environment conducive to stress and anxiety for the nurses, who have reported a higher level of work stress than other healthcare professionals. Knowing and addressing the factors impacting mental health/nurses’ well-being is crucial to providing care to patients. Stressful work environments result in burnout, compassion fatigue, depression, anxiety, suicide, and resignation. Understanding nurses’ perspectives on work related stress encourages hospitals to structure practices supporting nurses’ mental health and ability to provide quality care. Methods: A qualitative hermeneutic phenomenological approach was employed. Participants scheduled six weekly 10-min visits with Rex, a certified Service Dog for anxiety depression and PTSD, and registered ESA. A sample of 11 RNs participated in Rex visits at the workplace; field notes were taken during observations of visits. Semi-structured 30-min interviews conducted via Zoom, audio-recorded, and transcribed with NVivo were conducted. Results: Data were examined with Interpretative Phenomenological Analysis. Four main themes emerged: Preparing for the unknown, Doing the work, Refueling and resetting-Visits with Rex, What about nurses? Conclusions: This study highlights the need for nurses and hospital systems to incorporate self-care/self-reflection, including time/opportunities for nurses’ stress management during their practice. Knowledge of nurses receiving ESA interventions sheds light on how to protect/preserve the well-being of nurses practicing in this demanding profession. ESAs for nurses in the workplace offers an option to utilize dog therapy to re-energize and continue their shift renewed and refreshed. Full article

Workplace contextual emotions are structured ways of emotionally thinking about specific cues in the context that employees share within their organization. These dynamics reflect how employees emotionally interpret and respond to organizational environments. Contextual emotions may shape working relationships into different types of toxic emotional dynamics (e.g., claiming, controlling, distrusting, provoking) or, conversely, positive emotional dynamics (i.e., exchanging), thus setting the emotional tone that affects employees’ actions and their level of comfort/discomfort. The present study uses latent profile analysis (LPA) to identify subpopulations of employees who may experience differing levels of both positive and negative emotional dynamics (i.e., different configurations of emotional patterns of workplace behavior). Moreover, it examines whether the emergent profiles predict work-related (i.e., job satisfaction, burnout) and health-related outcomes (i.e., sleep disturbances, physical and mental health). Using data from 801 Italian employees, we identified four latent profiles: “functional dynamics” (low toxic emotions and high exchange), “dialectical dynamics” (co-existence of medium toxic emotions and medium exchange), “mild dysfunctional dynamics” (moderately high toxic emotions and low exchange), and “highly dysfunctional dynamics” (extremely high toxic emotions and extremely low exchange). Moreover, employees in the dialectical, mild dysfunctional, and highly dysfunctional groups reported progressively higher levels of poor health outcomes and progressively lower levels of satisfaction, whereas the functional group was at low risk of stress and was the most satisfied group. The theoretical and practical implications of the LPA-classified emotional patterns of workplace behavior are discussed in light of the relevance of identifying vulnerable subpopulations of employees diversely exposed to toxic configurations of emotional/relational ambience. Work-related well-being is an important research topic, as it is linked to various aspects of individuals’ lives, including job performance. To measure it effectively, unobtrusive sensors are desirable to minimize the burden on employees. Because there is a lack of consensus on the definitions of well-being in the psychological literature in terms of its dimensions, our work begins by proposing a conceptualization of well-being based on the refined definition of health provided by the World Health Organization. We focus on reviewing the existing literature on the unobtrusive measurement of well-being. In our literature review, we focus on affect, engagement, fatigue, stress, sleep deprivation, physical comfort, and social interactions. Our initial search resulted in a total of 644 studies, from which we then reviewed 35, revealing a variety of behavioral markers such as facial expressions, posture, eye movements, and speech. The most commonly used sensory devices were red, green, and blue (RGB) cameras, followed by microphones and smartphones. The methods capture a variety of behavioral markers, the most common being body movement, facial expressions, and posture. Our work serves as an investigation into various unobtrusive measuring methods applicable to the workplace context, aiming to foster a more employee-centric approach to the measurement of well-being and to emphasize its affective component. Effectiveness of Physical Activity-Led Workplace Health Promotion Interventions: A Systematic Review

Background: With increasing work pace and stress, sedentary office habits and insufficient physical activity (PA) pose significant threats to employee health and organizational productivity. Physical activity-led workplace health interventions (PAWHIs) have gained attention due to their multifaceted benefits for employees’ physical and mental health. This systematic review aims to evaluate the effectiveness of PAWHIs and examine the success rates of PA combined with various supplementary intervention approaches in improving employee health. Methods: This study was conducted according to the PRISMA 2020 statement guidelines. A systematic search was performed across four databases (PubMed, Web of Science, EBSCO, and Scopus) for randomized controlled trials (RCTs) published between 2013 and 2023. The Cochrane risk-of-bias tool was used to assess the risk of bias in the included studies. Results: After screening, 40 studies meeting the criteria were finally identified and subjected to quality assessment. The primary intervention measures of PAWHIs focused on education, physical activity, and dietary interventions. Fifty percent of the studies adopted multimodal combined intervention schemes involving two or more types of interventions. The most common intervention durations were 12 weeks (9 studies) and 24 weeks (24 studies). An analysis of the various intervention effects of PAWHIs revealed that the most successfully improved outcomes were increased physical activity levels (26/32), reduced psychological stress (4/5), and improved dietary habits (12/19), with over 60% of the related studies reporting positive effects. Additionally, improvements were also commonly observed in body composition (16/29) and clinical health outcomes (15/27). Conclusions: PAWHIs have positive effects on improving employee health status and promoting healthy behaviors, particularly in increasing PA levels and reducing psychological stress. However, interventions need to be contextualized and further optimized to achieve more comprehensive and sustainable health outcomes. Full article

Economic Evaluation of Proactive PTSI Mitigation Programs for Public Safety Personnel and Frontline Healthcare Professionals: A Systematic Review and Meta-Analysis

Public safety personnel and frontline healthcare professionals are at increased risk of exposure to potentially psychologically traumatic events (PPTEs) and developing post-traumatic stress injuries (PTSIs, e.g., depression, anxiety) by the nature of their work. PTSI is also connected to increased absenteeism, suicidality, and performance decrements, which compromise occupational and public health and safety in trauma-exposed workers. There is limited evidence on the cost effectiveness of proactive “prevention” programs aimed at reducing the risk of PTSIs. The purpose of this meta-analysis is to measure the economic effectiveness of proactive PTSI mitigation programs among occupational groups exposed to frequent occupational PPTEs, focusing on the outcomes related to PTSI symptoms, absenteeism, and psychological wellness. Findings from 15 included studies demonstrate that proactive interventions can yield substantial economic and health benefits, with Return On Investment (ROI) values ranging widely from −20% to 3560%. Shorter interventions (≤6 months) often produced higher returns, while longer interventions (>12 months) showed more moderate or negative returns. Notably, the level at which an intervention is targeted significantly affects outcomes—programs aimed at managers, such as the 4 h RESPECT training course, demonstrated a high ROI and broad organizational impact by enhancing leadership support for employee mental health. Sensitivity analyses highlighted significant variability based on the organizational context, program design, and participant characteristics. The majority of proactive interventions successfully reduced psychological distress and enhanced workplace outcomes, although thoughtful consideration of program design and implementation context is essential. The decline in mental health among workers in Japan has become a serious concern. Autism spectrum disorder (ASD) traits are increasingly recognized as a potential risk factor for mental health issues, yet few individuals receive a formal ASD diagnosis. This study aims to examine whether undiagnosed but working individuals with strong ASD characteristics differ in factors related to presenteeism (low productivity) compared to their counterparts with fewer ASD traits. In particular, we investigated the impact of social camouflaging—the behavioral adaptation used to mask ASD traits—especially on workers with strong ASD characteristics. An online survey was conducted among white-collar workers aged 20 to 60. The survey included items collecting demographic information (gender, age, marital status, highest level of education completed, employment status, job title, length of employment, and working hours) and included the Japanese versions of four validated instruments to measure aspects of autism traits, social camouflaging, work functioning, and occupational stress. Of the 543 respondents, 139 (25.6%) exhibited strong ASD characteristics. These individuals reported lower productivity than their counterparts. While social camouflaging was observed across all ASD trait levels, it was specifically linked to poorer mental health and reduced productivity among workers with strong ASD characteristics. To support workplace well-being and productivity, it is crucial to increase awareness and understanding of ASD traits in professional settings and create an environment that accommodates diverse work styles. Participants completed standardized questionnaires, including the Maslach Burnout Inventory (MBI-HSS), Athens Insomnia Scale (AIS), depression, anxiety, and stress scale (DASS-21), and EQ-5D health-related quality of life questionnaire. Results: Insomnia was reported in 65.0% of female and 35.0% of male participants, with a mean age of 38.57 ± 11.57 years. Insomnia symptoms were more common among those reporting alcohol consumption (p = 0.007), smoking (p = 0.006), and sedative use (p = 0.038). A higher workload (p = 0.017), previous COVID-19 infection (p = 0.001), and quarantine (p = 0.008) were linked to insomnia. Healthcare professionals with insomnia reported lower quality of life across all EQ-5D dimensions (p < 0.001) and higher levels of stress, anxiety, and depression (p < 0.001). Burnout was significantly associated with emotional exhaustion (p < 0.001), while depersonalization and personal achievement showed no significant differences. Conclusions: This study highlights a significant relationship between burnout, mental health issues, and insomnia during the COVID-19 pandemic. Addressing these factors through targeted interventions and workplace policies is essential for improving healthcare professionals’ well-being and ensuring the healthcare system’s sustainability. Impact of Psychosocial Factors on Mental Health and Turnover Intention Among Health Workers at Different Occupational Statuses: An Exploratory Cross-Sectional Study in China

Workplace psychosocial hazards pose significant risks to the well-being of health workers (HWs). This study aimed to explore the levels of psychosocial factors experienced by Chinese and international workers and examine associations between psychosocial factors, health-related outcomes (mental problems and general health), and turnover intention across various occupational stages. A cross-sectional study was conducted using stratified cluster sampling methods at four hospitals in southern China. Psychosocial factors, mental health, general health, and turnover intention were assessed using the Chinese version of the Copenhagen Psychosocial Questionnaire III. Multivariate linear and logistic regression analyses were conducted. A total of 1054 HWs participated in this study (response rate: 80.21%). Compared to international samples, Chinese HWs showed significant differences in nine psychosocial dimensions, particularly interpersonal relations, leadership, and the work–individual interface. Senior hospital managers reported significantly higher stress (51.09 ± 23.88, p < 0.001), sleep troubles (53.26 ± 24.92, p = 0.003), and poorer general health (57.61 ± 37.26, p = 0.035) than other occupational stages. Work–life conflict (β = 0.172), emotional demands (β = 0.132), and role clarity (β = −0.132) were the strongest predictors of mental health issues. Hiding emotions demands (β = −0.141) and leadership quality (β = 0.130) most strongly predicted general health. The turnover intention rate was 11.01%, with job satisfaction (OR = 0.964) being the strongest negative predictor. Reducing psychosocial stressors, particularly in work–life balance, role clarity, emotional demand, and leadership quality, could improve HWs’ well-being and reduce turnover intention. Full article

Background/Objectives: There is a significant gap in accessibility to mental healthcare in Canada. This study aims to examine the population of Canadian crisis hotline responders and investigate the variables that contributed to burnout and professional quality of life during COVID-19. Crisis hotline responders are hypothesized to be affected by burnout and poor professional quality of life, due to the inherent nature of the job and the widespread negative mental health effects of COVID-19, which are expected to continue even after the pandemic. Methods: An online, cross-sectional, mixed-methods survey assessed sociodemographic information, shift-related variables, burnout and related factors, and current support methods utilized by crisis hotline responders across Canada. The open-ended questions helped to more personally reflect participants’ experiences. Data were analyzed using chi-square tests, an analysis of variance, and a regression analysis. Results: The survey was completed by 136 participants (78.7% female) with an average age of 39.68. Participants reported relatively high levels of burnout/stress on both the Copenhagen Burnout Inventory and professional quality of life survey. Younger age, less work experience, and working overnight shifts emerged as possible predictors of worse mental wellbeing. Conclusions: Findings suggest that Canadian crisis hotline responders require greater support to manage workplace burnout/stress. Nevertheless, conducting comprehensive studies during times when there are no public health emergencies are warranted to understand the full scope of burnout in this population. We offer five recommendations to support the mental wellbeing of responders and improve access to this important public health resource. Full article

Background: Work engagement is defined as a positive and fulfilling work-related state of mind, characterized by vigor, dedication, and absorption. High levels of engagement are associated with improved organizational functioning and a stronger sense of belonging among employees. Objective: This study, conducted in collaboration with a large metalworking company in central Italy, aimed to explore the relationship between work engagement and organizational cynicism among Maintenance Team Leaders. Specifically, the goals were to assess the levels of engagement and emotional involvement and to examine how these dimensions vary according to socio-demographic factors such as gender, age, educational background, and seniority. The ultimate aim was to support occupational physicians and workplace safety officers in identifying the best practices for preventing psychosocial risks, work-related stress, and burnout. Methods: A total of 99 Maintenance Leaders participated in the study. The Italian version of the Utrecht Work Engagement Scale (UWES) was used to assess the three core dimensions of engagement: vigor, dedication, and absorption. Additionally, the Cynicism scale development by Naus, Van Iterson, and Roe was administered. Results: While the sample size limits generalizability, the findings offer preliminary insight into engagement levels within this population. The results emphasize the need to expand the sample and to conduct comparative analyses across different teams within the company better understand engagement patterns and inform targeted interventions. Conclusions: Although work engagement has been extensively studied in the healthcare sector, especially during the COVID-19 pandemic, limited research has addressed its role in the industrial context, and even less within the metalworking sector. This study contributes to filling that gap by providing an initial profile of engagement among maintenance leaders and by highlighting the interplay between engagement, cynicism, and individual characteristics in a high-demand industrial environment. The urgency of managing the COVID-19 health crisis in workplaces led to tensions, work overload, and confusion about preventive measures. This study presents a secondary analysis of qualitative data on paradoxes and double binds (PDBs) experienced by precarious essential workers in Canada who interacted with the public and their supervisors. Based on 13 interviews from a larger qualitative dataset, we examine how workers navigated public health recommendations and organisational demands during the pandemic. Findings reveal multiple organisational and managerial PDBs—both COVID-19-related and pre-existing—that contributed to psychological distress and compromised well-being. We argue that PDBs represent a significant occupational health hazard for precarious workers. Addressing these structural contradictions through proactive management strategies could help mitigate workplace tensions, reduce stress, and enhance resilience in both crisis situations and regular organisational contexts. Our study contributes to occupational health and safety (OHS) by underscoring the risks posed by PDBs and advocating for strategies to support vulnerable workers in navigating conflicting demands.
