Background/Objectives: The effect of diet on maternal and infant genetic levels has been reported in the literature. Diet-associated DNA damage, such as the presence of micronuclei (MN), may be related to an increased risk of developing chronic diseases such as cancer. There is particular concern regarding this damage during pregnancy, as it may affect the newborn (NB). Thus, this review aims to summarize the primary evidence of the impact of diet on the frequency of MN in the mother–infant population. Methods: Five databases (PubMed, Embase, Web of Science, Scopus, and ScienceDirect) were used to search for observational studies. Google Scholar and manual searching were required to perform the “gray literature” search. Results: The search strategy retrieved 1418 records. Of these, 13 were read in full and 5 were included in the review. Most studies were of the cohort type (n = 4) and were carried out in the European region. A total of 875 pregnant women and 238 newborns were evaluated. Despite insufficient evidence to confirm that diet changes the frequency of MN, the included studies found possible effects from the consumption of fried red meat and processed meats and the adequate consumption of vegetables and polyunsaturated fats. Conclusions: Future research is needed in order to understand the effects of diet on genetic stability and to obtain evidence to help plan public policies on food and nutrition or reinforce protective dietary patterns for this and future generations. Background: Physical exercise during pregnancy is strongly recommended due to its well-established benefits for both mother and child. However, its impact on the pelvic floor remains insufficiently studied. This study aimed to evaluate pelvic floor adaptations to a structured prenatal exercise program using transperineal ultrasound, and to assess associations with the duration of the second stage of labor and mode of delivery. Methods: This is a planned secondary analysis of a randomized controlled clinical trial (RCT) (NCT04563065) including women with singleton pregnancies at 12–14 weeks of gestation. Participants were randomized to either an exercise group, which followed a supervised physical exercise program three times per week, or a control group, which received standard antenatal care. Transperineal ultrasound was used at the second trimester of pregnancy and six months postpartum to measure urogenital hiatus dimensions at rest, during maximal pelvic floor contraction, and during the Valsalva maneuver, to calculate hiatal contractility and distensibility and to evaluate levator ani muscle insertion. Regression analyses were performed to assess the relationship between urogenital hiatus measurements and both duration of the second stage of labor and mode of delivery. Results: A total of 78 participants were included in the final analysis: 41 in the control group and 37 in the exercise group. The anteroposterior diameter of the urogenital hiatus at rest was significantly smaller in the exercise group compared to controls (4.60 mm [SD 0.62] vs. 4.91 mm [SD 0.76]; p = 0.049). No other statistically significant differences were observed in static measurements. However, contractility was significantly reduced in the exercise group for both the latero-lateral diameter (8.54% vs. 4.04%; p = 0.012) and hiatus area (20.15% vs. 12.55%; p = 0.020). Distensibility was similar between groups. There were no significant differences in the duration of the second stage of labor or mode of delivery. Six months after delivery, there was an absolute risk reduction of 32.5% of levator ani muscle avulsion in the exercise group compared to the control group (53.3% and 20.8%, respectively; p = 0.009). Conclusions: A supervised exercise program during pregnancy appears to modify pelvic floor morphology and function, reducing the incidence of levator ani muscle avulsion without affecting the type or duration of delivery. These findings support the safety and potential protective role of prenatal exercise in maintaining pelvic floor integrity. Full article

Background: Chronic pain affects one in five youth globally and is frequently accompanied by mental health challenges that extend into adulthood. Caregivers play a vital role in supporting youth with chronic pain, yet their own mental and physical health needs are often overlooked. While caregiver well-being is linked to child outcomes, few interventions directly address caregivers’ health, especially among those facing systemic barriers. This study explored the lived experiences of caregivers to better understand their unmet needs and inform the co-design of a supportive digital health solution. Methods: We conducted a qualitative exploratory study involving 32 caregivers of youth with chronic pain across Canada and Australia. Semi-structured interviews were co-facilitated by caregiver partners. Thematic analysis was applied to interview data. Results: Two overarching themes were identified: (1) bearing the weight and sacrifice of caregiving and (2) deep interrelatedness and blurred boundaries. Caregivers reported profound emotional, physical, and financial burdens; strained relationships; and social isolation. Many struggled with self-neglect, prioritizing their child’s needs over their own. Fathers’ evolving caregiving roles challenged traditional gender norms, though mothers continued to bear a disproportionate load. Despite challenges, caregivers demonstrated resilience and recognized their well-being as interconnected with their child’s health. Conclusions: Findings underscore the need for systemic investment in caregiver well-being. Digital health solutions, including virtual peer networks, mental health resources, and tailored education, offer scalable, accessible pathways for support. These insights will inform the development of Power over Pain for Primary Caregivers, a digital solution and knowledge hub aimed at improving caregiver well-being and family outcomes, aligning with global efforts to enhance family-centred pediatric pain care. Objective: This study aimed to investigate the relationship between social media use (SMU) in children diagnosed with major depression (MD) or attention deficit hyperactivity disorder (ADHD) and various psychosocial factors, including familial functioning, parental SMU, and parent-reported internalizing and externalizing symptoms. A healthy control group was included for comparison. Methods: The study included 121 children and adolescents aged 10–18 years (36 with MD, 41 with ADHD, and 44 healthy controls). The Social Media Addiction Scale—Short Form (SMDS) was administered to all participants, while mothers completed the McMaster Family Assessment Scale (FAS), the Social Media Addiction Scale—Adult Form (SMAS-AF), and the Child Behavior Checklist (CBCL). Psychiatric diagnoses were made using the K-SADS-PL DSM-5-T. Correlation and linear regression analyses were used to assess associations among variables. Results: SMU scores were significantly higher in the ADHD group compared to both the depression and control groups. Parental SMU was also higher in the ADHD group. In the depression group, child SMU was significantly associated with internalizing symptoms and impaired family communication. In the ADHD group, child SMU was predicted by poor family problem-solving and communication. Regression analyses showed that internalizing symptoms and family communication predicted SMU in the depression group (R2 = 0.335), while family problem-solving and communication predicted SMU in the ADHD group (R2 = 0.709). Conclusion: The findings suggest that social media use in children with depression and ADHD is associated with different psychosocial factors. While internalizing symptoms and family communication are more prominent in depressed children, family functioning—particularly problem-solving and communication—plays a larger role in children with ADHD. These results emphasize the need for targeted family-based interventions to mitigate problematic SMU in clinical populations. Full article

Background: Adolescents with a chronological age of less than 15 years or a gynecological age of less than 2 years may have a higher probability of complications because they are more likely to conceive within 1 to 2 years of menarche and, therefore, are still growing and maturing. This could impair their ability to adapt to the physiological demands of pregnancy. Objective: To evaluate the relationship between chronological age and gynecological age with low birth weight and small for gestational age among adolescent mothers in Mexico City. Methods: A retrospective cohort design of adolescent mother–child dyads was conducted. The study followed 1242 adolescents under 19 years of age and their children, collecting data on physical, socioeconomic, and clinical characteristics, including hemoglobin levels. Low birth weight was assessed using the Intergrowth-21st project standards and categorized as above or below 2500 g. The mothers were grouped by chronological age (<15 years and ≥15 years) and gynecological age (<3 years and ≥3 years). Adjusted odds ratios were calculated using binary logistic regression models. The outcome variables were low birth weight and small for gestational age. The independent variables included gynecological age, chronological age, age at menarche, hemoglobin concentration, and gestational weight gain, among others. All independent variables were converted to dummy variables for analysis. Calculations were adjusted for the following variables: marital status, maternal education, occupation, educational lag, family structure, socioeconomic level, pre-pregnancy body mass index, and initiation of prenatal care. Results: The average age of the participants was 15.7 ± 1 years. The frequency of small for gestational age and low birth weight was 20% and 15.3%, respectively. Factors associated with small for gestational age included gynecological age < 3 years [aOR = 2.462, CI 95%; 1.081–5.605 (p = 0.032)], hemoglobin < 11.5 g/dL [aOR = 2.164, CI 95%; 1.08–605 (p = 0.019)], insufficient gestational weight gain [aOR = 1.858, CI 95%; 1.059–3.260 (p = 0.031)], preterm birth [aOR = 1.689, CI 95%; 1.133–2.519 p = 0.01], and living more than 50 km from the care center [aOR = 2.256, CI 95%; 1.263–4.031 (p = 0.006)]. An early age of menarche [aOR = 0.367, CI 95%; 0.182–0.744 (p = 0.005)] showed a protective effect against small for gestational age. Factors associated with low birth weight included gynecological age < 3 years [aOR = 3.799, CI 95%; 1.458–9.725 (p = 0.006)], maternal age < 15 years [aOR = 5.740, CI 95%; 1.343–26.369 (p = 0.019)], preterm birth [aOR = 54.401, CI 95%; 33.887–87.335, p = 0.001], living more than 50 km from the care center [aOR = 1.930, CI 95%; 1.053–3.536 (p = 0.033)], and early age of menarche [aOR = 0.382, CI 95%; 0.173–0.841 (p = 0.017), which demonstrated a protective effect, respectively. Conclusions: The study concludes that biological immaturity, particularly early gynecological age, significantly contributes to adverse birth outcomes during adolescent pregnancies. Interestingly, early menarche appeared to have a protective effect, whereas chronological age was not a significant predictor of small for gestational age. Chronological age has an even greater impact: women younger than 15 years are 5.7 times more likely to have low birth weight infants. However, chronological age did not increase the likelihood of having an SGA newborn. Full article

Background/Objectives: Screen-related parenting practices (e.g., restriction, coviewing) influence the way children use screen devices. Although children start using screen devices (e.g., televisions [TV], tablets) in the first few years of life, rigorously developed measures of screen-related parenting practices for parents of toddlers do not exist. The objective of this study was to develop culturally and contextually relevant survey measures of screen-related parenting practices for use in Mexican American families with toddlers. Methods: Measures were developed using an exploratory sequential mixed methods (qualitative → quantitative) approach. Mexican American mothers of toddlers (15–26 months of age) participated in semi-structured interviews. Using the interview findings, domains of parenting practices across screen device types were identified, and survey items were developed. Items were administered by phone to 384 Mexican American mothers. Analyses included evaluation of the factor structure and psychometric properties of nine domains of parenting practices and evaluations of correlations between each scale and demographic characteristics. Results: Factor analyses supported a one-factor solution for each parenting practice as follows: Restrict TV Time (8 items), Coview TV (10 items), Behavioral Regulation with TV (12 items), Restrict Mobile Device Time (8 items); Coview Mobile Devices (10 items); Behavioral Regulation with Mobile Devices (16 items), Restrict Screen Content (8 items), Allow Screen Use Around Sleep (6 items), and Allow Screen Use While Eating (6 items). The reliabilities were acceptable (Cronbach’s alphas > 0.80). Hispanic acculturation, maternal age, and child age were correlated with many of the scales of parenting practices. Conclusions: The measures developed in this study offer a way to evaluate the use and impact of screen-related parenting practices in Mexican American families with toddlers. The use of these measures will enable investigators to identify relationships among parenting practices, screen use, and child well-being, which could inform the design of early childhood interventions promoting healthy screen use in this population. Background/Objectives: Emotional interactions between mothers and children are essential for later developmental outcomes such as children’s health, social competence, and language skills. However, an observational assessment of such interactions cannot reveal how mothers perceive their relationships with their children. Therefore, in this study, we translated and validated the Japanese version of the Emotional Availability Self-Report (EA-SR-J) and examined its factor structure, reliability, and validity. Methods: The participants were 739 Japanese mothers with children aged from 1 month to 5 years and 11 months. Results: Exploratory factor analysis revealed 19 items and four factors (Affect Quality, Hostility, Mutual Attunement, and Child Involvement with Parent) for the EA-SR-J. However, Intrusiveness was removed, and some items were assigned to different factors compared with the original scale. Cronbach’s alpha of each subscale ranged from 0.81 to 0.88, indicating sufficient internal consistency. The convergent validity of the scale was confirmed with expected correlations with variables related to attachment and child-rearing style. Discriminant validity was confirmed by group differences in variables related to children’s autism spectrum disorder symptoms and mothers’ depression symptoms. Conclusions: Although the EA-SR-J contains fewer items than the original scale, with the Intrusiveness item removed, it remains a reliable and valid tool. Further studies using the Emotional Availability Scale based on observation are needed in the future. Full article

Discipline is a significant predictor of parent–child attachment and relationship quality across the lifespan. Yet, much of the research on Black families’ disciplinary strategies uses a deficit and myopic lens that focuses on punitive punishment styles (e.g., spanking or taking away privileges). In the current exploratory qualitative study, we used an intergenerational narrative lens and thematic analysis to explore semi-structured interview data from 31 Black mothers (25–60 years, Mage = 46) in the United States around their mothers’ disciplinary practices during their childhood. We explored the connections that Black adult daughters made between their childhood disciplinary practices and their current disciplinary practices with their children, as well as their current relationships with their mothers. Adult daughters varied widely in their retrospective accounts of their mothers’ disciplinary strategies, which we categorized into three themes: (a) punitive, (b) logical, and (c) natural. We also identified three themes around how their mothers’ practices informed their current disciplinary practices with their own children: namely, (d) continuity, (e) mix, and (f) shift. Finally, we identified three themes around the current nature of their mother–daughter relationships: (g) strained, (h) progressing, and (i) healthy. The results highlighted the personal and cultural factors that informed Black women’s disciplinary strategies across two generations of mothers and revealed that when adult daughters shifted away from what they experienced during childhood—it was often towards less punitive strategies. Our exploratory findings also pointed to patterns regarding the extent to which Black adult daughters felt connected, validated, and supported by their mothers. The findings lend insight into Black mother–daughter relationship dynamics, particularly around the importance of communication patterns and emotional connection in the culture of discipline within families. Association of Prenatal and Intrapartum Antibiotic Use with Risk of Childhood Atopic Dermatitis: A Systematic Review and Meta-Analysis

Background/Objectives: Atopic dermatitis (AD) is a chronic inflammatory skin condition with rising global prevalence. Increasing maternal antibiotic use during pregnancy has raised concerns about its potential link to childhood allergic diseases, including AD. However, existing meta-analyses have yielded inconsistent results. A systematic review and meta-analysis were conducted to investigate the association between prenatal antibiotic exposure, including intrapartum antibiotic prophylaxis (IAP), and the risk of AD developing in offspring. Methods: A systematic search protocol (PROSPERO ID: CRD42024577804) was conducted up to 29 August 2024, across the PubMed, Embase, and Cochrane databases. Cohort and case–control studies reporting associations between maternal antibiotic exposure during pregnancy or intrapartum and the risk of AD in offspring were included. Data were analyzed using RevMan Web and Comprehensive Meta-Analysis software. Results: Twenty studies involving 3,256,929 mother–child pairs were reviewed. The meta-analysis data demonstrated that prenatal antibiotic exposure was associated with AD in the main analysis (odds ratio [OR]: 1.12, 95% CI 1.03–1.21), but not in a separate analysis with a pooled hazard ratio (HR) (HR: 1.12, 95% CI 0.96–1.31). Trim-and-fill correction for significant publication bias (Egger’s test p = 0.003) in the main analysis resulted in a non-significant effect size (OR: 1.09, 95% CI 0.99–1.20). Subgroup analysis and meta-regression suggested that publication years and sample sizes contributed significant heterogeneity (p < 0.05). Regarding IAP and the risk of AD, no association was found (OR: 1.62, 95% CI 0.87–3.00). Conclusions: Current evidence in the existing literature does not support a positive relationship between antibiotic exposure, either during pregnancy or in the intrapartum period, and the risk of development of AD in offspring. However, substantial heterogeneity and the very low certainty of evidence limit the strength of our findings. Further studies that address confounders more thoroughly are needed to confirm these results. Full article

Methods: A total of 122 Israeli parents (55 fathers (Mage = 43.8; SD = 4.01) and 67 mothers (Mage = 41.6; SD = 4.59)) of 75 children with attention deficit hyperactivity disorder (ADHD) (Mage = 8.4; SD = 1.56) completed self-report measures: the Experience of Close Relationships scale (ECR), the Mindfulness Attention Awareness Scale (MAAS), the Parental Acceptance–Rejection Questionnaire (PARQ), and Achenbach Child Behavior Checklist (CBCL). Data were analyzed using descriptive statistics, Pearson correlations, and structural equation modeling (SEM). Results: The key finding was that a latent ‘parental rejection/non-warmth’ factor mediated the relationship between (a) parents’ anxious attachment and child behavior problems, and (b) parental mindfulness and child problems. Parental rejection emerged as the strongest predictor of child difficulties. Conclusions: Parental training for parents of children with ADHD should prioritize reducing rejection while also addressing mindfulness and anxious attachment style to promote child well-being. Background/Objectives: School bullying is a significant issue that negatively impacts children’s well-being, emphasizing the need to identify family-related factors contributing to bullying victimization. This study explored the potential link between parental marital quality and school bullying victimization by employing a moderated mediation model. Methods: Parent–child attachment, measured separately as father–child and mother–child attachment, was tested as a mediator, with child gender included as a moderator. Data were collected from both children and their mothers, comprising 358 mother–child pairs recruited from three primary schools in suburban Beijing, China. Results: Results revealed that greater parental marital quality was associated with a lower risk of bullying victimization, with father–child attachment mediating this relationship. Furthermore, child gender moderated the mediating effect of father–child attachment, such that the indirect pathway from parental marital quality to bullying victimization through father–child attachment was statistically significant for girls but not for boys. Conclusions: These findings highlight the importance of father–child attachment in preventing bullying victimization and suggest that gender-sensitive implications may be necessary. Full article

Background/Objectives: Early childhood is a critical window of development, which is influenced by early life exposures including breastfeeding. Observational and preclinical studies have linked human milk oligosaccharides (HMOs) with neurocognitive development. However, less attention has focused on behavioral outcomes including temperament and eating behaviors. Herein, we investigated the associations between HMO consumption and child temperament and child eating behaviors through four years of age. Methods: Participants were drawn from the STRONG Kids 2 cohort. Human milk was collected at 6 weeks postpartum, and HMO relative abundances were determined by HPLC-MS. Using validated questionnaires, child temperament was assessed at 3, 18, and 48 months of age, and child eating behaviors were measured at 12, 18, 24, 36, and 48 months of age. After adjusting for covariates, multiple linear regressions were carried out to assess the relationship between HMOs and the outcome measures. Results: The HMO profile of mothers showed two distinct clusters explained by maternal secretor status. Significant associations between HMO and surgency were only found in the full cohort and secretors, while more associations between HMO and negative affectivity were observed in non-secretors. A significant number of associations was observed between HMOs and child eating behaviors in full cohort, secretors, and non-secretors. HMO diversity, total fucosylation, and total sialylation were positively associated with food responsiveness, while neutral HMOs presented negative associations. However, these associations with food responsiveness were not observed in non-secretors. Conclusions: HMO profiles were associated with behavioral outcomes in the children, with variations by child age and maternal secretor status, highlighting the potential role of HMOs within the broader context of maternal and postnatal influences. Influence on Consultation Behavior of Pregnant and Postpartum Women in Japan: Insights from a Nation-Wide Survey

Background: In Japan, new challenges are emerging, such as declining birth rates, an increase in age at childbirth, postpartum depression, and child abuse. Methods: This study examines the factors influencing maternal consultation behavior at public institutions using some of the data from the Japan COVID-19 and Society Internet Survey (JACSIS). A total of 6227 women (1380 pregnant and 4847 postpartum) were analyzed through logistic regression. Results: The results showed different factors influencing consultations between pregnant women and postpartum women. Among pregnant women, different trends were observed between primigravidae and multigravidae, indicating that primigravidae aged ≥40 years and multigravidae with multiple childbirths tend to seek consultations more frequently. In postpartum women, psychological distress (K6), adverse childhood experiences (ACEs), and social support networks significantly influenced consultation behavior. Postpartum women with extensive support networks (≥3 confidants) exhibited higher consultation rates. In contrast, those with elevated Edinburgh Postnatal Depression Scale (EPDS) and Mother-to-Infant Bonding Scale (MIBS-J) scores were less likely to seek help, potentially due to mental health stigma. Conclusions: This study showed that pregnant and postpartum women facing physical, mental, family, or relationship issues are seeking consultations and receiving thorough support. On the other hand, since some pregnant and postpartum women refrained from seeking consultation, it is necessary to further consider support measures that make it easier for all pregnant and postpartum women to seek advice in the future. Research suggests that maternal stress is related to aspects of child development. Positive and stimulating interactions, such as shared reading, may act as protective factors, mitigating the negative effects of maternal stress on children’s development and behavior. This cohort study investigated the predictive relationship between maternal stress and children’s milestones and behavioral problems, with maternal interactive style during shared reading as a moderator. A total of 91 mother–child dyads participated. During the shared reading session, conducted in a private room at a research center, each mother and child interacted freely while reading a book, without specific instructions. The sessions were video-recorded and later analyzed by trained researchers using an established method. Children’s data were assessed using questionnaires completed by the mother on the same day. Descriptive, bivariate, and multivariate analyses were performed to build regression models with moderation analysis. Results revealed that maternal stress significantly predicted children’s internalizing and externalizing symptoms. Notably, more affectionate interactions during shared reading can moderate the effect of stress on children’s externalizing difficulties. These findings suggest that fostering positive and engaging interactions between mothers and children, such as shared reading, may have a beneficial impact on children’s behavioral development, even in the presence of maternal stress. Maternal childrearing practices play a prominent role in a child’s developmental outcomes. Difficult child temperament, specifically, negative emotionality, impacts parenting practices. The present study contributes to the existing literature by investigating the mediating role of parenting practices on associations between children’s temperament and academic and behavioural outcomes in a low-income and ethnically diverse sample. The present study consists of a sample of 163 families. The average age of the children was 32.40 months (SD = 2.61 months). The average age of the mothers was 34.35 years (SD = 5.32 years). Structural equation modelling examined the relationship between children’s temperament, parenting practices, and child outcomes. A two-step procedure was conducted to test this model: confirmatory factor analysis followed by latent path analysis. The results show that children’s temperament was significantly and positively associated with mothers’ hostile parenting and children’s conduct problems. Hostile parenting was positively associated with children’s conduct problems. While overprotective parenting was negatively associated with children’s receptive vocabulary scores, maternal responsivity was positively associated with better receptive vocabulary in children. Finally, hostile parenting was found to play a significant and positive mediating role in children’s conduct behaviour. Maternal practices are associated with outcomes in children with negative emotionality, underscoring the need for tailored interventions in diverse, low-income families. Full article
