Behavioral Assessment of Equine Relaxation Following Manual Therapy: A Pilot Study

The aim of this pilot study was to evaluate the relaxation, stress reduction and behavioral changes observed after manual therapy applied to horses exposed to racing and physical training stimulus. This descriptive approach is aimed at veterinary clinicians to evaluate the therapy process more effectively with behavioral feedback. For this purpose, the study was conducted in two different equestrian clubs in Adana (Adana Mediterranean and Suvari Equestrian Clubs) between 2023 and 2024. A total of 32 racehorses (16 Thoroughbred, 16 Arabian; 16 female, 16 male) of different ages, genders and breeds were included in the study. Five minutes of manual therapy was applied for each of 7 different muscle groups. After the massage, behavioral observations were made for 10 min by moving 2 m away from the animals, and no separate baseline assessment was performed prior to the intervention. The application was carried out by a veterinarian with 15 years of experience. Importantly, no separate baseline assessment or control group was performed, and only behavioral responses were evaluated, which represents a major limitation of this pilot study. Among the observed behaviors in all horses, blinking, muscle twitching, respiratory changes, lip relaxation, licking and chewing were recorded for all horses. Relaxation signs such as head dropping (78.1%), yawning (34.4%), and ears falling to the side (62.5%) were frequently observed. Behaviors such as the appearance of the third eyelid (3.1%), grunting (12.5%) and sneezing (15.6%) were observed at a low percentage. Individual variables such as gender and breed did not have a statistically significant effect on the percentage of behavior (Chi-square test, p > 0.05). In conclusion, these preliminary findings suggest that manual therapy applications might be effective in reducing stress by triggering relaxation behaviors in riding horses, as these behaviors have been previously reported in the literature as reliable indicators of relaxation. Evaluation of behavioral responses after massage could be an important tool in determining physiotherapeutic effects. The fact that the application is performed by experienced people is an important factor that increases the success of the therapy and shows that manual therapy provides relaxation regardless of individual differences. Future controlled studies integrating physiological stress biomarkers are warranted to confirm these observations. Full article

Background and Objectives: Infrared light is used to image the Meibomian glands through their thermal profile. This study aimed to investigate the effects of a combination of heating and an eyelid massage on Meibomian gland visibility and tear film parameters. Materials and Methods: Twenty-four participants (26 ± 6.9 years) were enrolled in this prospective study, which involved imaging the Meibomian glands of both the lower and upper eyelid and assessing the non-invasive breakup time (NIBUT), tear meniscus height (TMH), and blink rate (using the CA-800, Topcon) at baseline after five minutes of eyelid warming followed by a five-minute eyelid massage. The second session, which was randomised in sequence, repeated the same measurements but without the inclusion of any eyelid warming or massage as the control condition. Results: While there was no change in lower lid Meibomian gland appearance as a result of eyelid heating, eyelid massage, or multiple lid eversion (median 2.0, range 0.0 to 4.0; p = 0.782), there was a change in upper lid appearance 5 min after heating and lid massage (p = 0.025), but again, multiple lid eversion had no effect (p > 0.05). The NIBUT decreased on second lid eversion (p = 0.049), although this was not evident on the third lid eversion (p = 0.090). The effect on NIBUT was also apparent with heating (p = 0.034 immediately after) but was sustained with 5 min of eyelid massage (p = 0.031). The TMH increased with heating (p < 0.001), and this effect was sustained with 5 min of eyelid massage (p = 0.011), but there was no lid eversion effect (p > 0.05). The blink rate was unaffected by heating, eyelid massage, or multiple eversions of the eyelids (median 24 blinks/min, range 8 to 59 blinks/min; p = 0.61). Conclusions: Eyelid warming can increase the visibility of the Meibomian glands, although this effect was only observed with upper lid imaging and the effect dissipated after 5 min of eyelid massage. Warming and massage also disrupt the tear film, as does multiple lid eversion, emphasising the need to use the least invasive tear film assessment techniques first. Full article

Purpose: The aim of this study was to use swept-source anterior segment optical coherence tomography (OCT) to explore imaging the meibomian gland openings and to identify their in vivo characteristics in patients with obstructive meibomian gland dysfunction (MGD) and healthy participants. Methods: We enrolled 49 patients with MGD and 54 health controls in this case-control study. Each participant underwent slit-lamp examination, meibography, and OCT scanning. Sixteen patients with MGD underwent a repeat OCT examination after eyelid massage. The outcome measures included determinations of meibomian gland openings (orifices and terminal ducts) from OCT images and comparisons of the meibomian openings between patients with MGD and normal controls before and after meibomian gland massage. Results: Using the same OCT scanning model, the number of visible orifices of the meibomian glands was similar between eyes with MGD and normal eyes (9.2 ± 2.3 vs. 9.7 ± 2.4). The mean diameter of the terminal ducts in patients with MGD was larger (120.22 ± 27.92 µm vs. 100.96 ± 20.30 µm) than in the normal controls, and had a larger coefficient of variation. Significant differences were observed in the mean diameter of the terminal ducts of patients with MGD before and after meibum gland massage (133.73 ± 27.81 μm vs. 102.26 ± 24.30 μm, p < 0.001). Conclusions: Patients with MGD have more diversified orifices and larger meibomian gland terminal duct diameters than normal subjects. In addition, meibomian gland terminal duct diameters seem to decrease in patients with MGD after meibum gland massage. Full article
