Urinary tract infections (UTIs) are the second most frequent type of infection observed in clinical practice. Gram-negative Enterobacteriaceae are common pathogens in UTIs. Excessive antibiotic use in humans and animals, poor infection control, and increased global travel have accelerated the spread of multidrug-resistant strains (MDR). Carbapenem antibiotics are commonly considered the last line of defense against MDR Gram-negative bacteria; however, their efficacy is now threatened by the increasing prevalence of carbapenem-resistant Enterobacteriaceae (CRE). This comprehensive review aims to explore the biological mechanisms underlying carbapenem resistance and to present a focus on therapeutic alternatives currently available for complicated UTIs (cUTIs). A comprehensive bibliographic search was conducted on the PubMed/MEDLINE, Scopus, and Web of Science databases in December 2023. The best evidence on the topic was selected, described, and discussed. Analyzed with particular interest were the clinical trials pivotal to the introduction of new pharmacological treatments in the management of complicated cUTIs. Additional suitable articles were collected by manually cross-referencing the bibliography of previously selected papers. This overview provides a current and comprehensive examination of the treatment options available for CRE infections, offering a valuable resource for understanding this constantly evolving public health challenge. Prior to undergoing total knee arthroplasty (TKA), surgeons are often confronted with patients with numerous questions regarding the procedure and the recovery process. Due to limited staff resources and mounting individual workload, increased efficiency, e.g., using artificial intelligence (AI), is of increasing interest. We comprehensively evaluated ChatGPT’s orthopedic responses using the DISCERN instrument. Three independent orthopedic surgeons rated the responses across various criteria. We found consistently high scores, predominantly exceeding a score of three out of five in almost all categories, indicative of the quality and accuracy of the information provided. Notably, the AI demonstrated proficiency in conveying precise and reliable information on orthopedic topics. However, a notable observation pertains to the generation of non-existing references for certain claims. This study underscores the significance of critically evaluating references provided by ChatGPT and emphasizes the necessity of cross-referencing information from established sources. Overall, the findings contribute valuable insights into the performance of ChatGPT in delivering accurate orthopedic information for patients in clinical use while shedding light on areas warranting further refinement. Future iterations of natural language processing systems may be able to replace, in part or in entirety, the preoperative interactions, thereby optimizing the efficiency, accessibility, and standardization of patient communication. Periprosthetic infection (PJI) after TAR is a serious complication, often requiring further surgery, including revision arthroplasty, conversion to ankle arthrodesis, or even amputation. This systematic review aims to summarize the current evidence on the management of TAR PJI and provide a comprehensive overview of this topic, especially from an epidemiologic point of view. Three different databases (PubMed, Scopus, and Web of Science) were searched for relevant articles, and further references were obtained by cross-referencing. Seventy-one studies met the inclusion criteria, reporting on cases of TAR PJI. A total of 298 PJIs were retrieved. The mean incidence of PJI was 3.8% (range 0.2–26.1%). Furthermore, 53 (17.8%) were acute PJIs, whereas most of them (156, 52.3%) were late PJIs. Most of the studies were heterogeneous regarding the treatment protocols used, with a two-stage approach performed in most of the cases (107, 35.9%). While the prevalence of ankle PJI remains low, it is potentially one of the most devastating complications of TAR. This review highlights the lack of strong literature regarding TAR infections, thus highlighting a need for multicentric studies with homogeneous data regarding the treatment of ankle PJI to better understand outcomes. Modular megaprostheses (MPs) are commonly used after bone-tumor resection, but they can offer a limb salvage solution in massive bone defects. The aim of this systematic review of the Literature is to provide a comprehensive data collection concerning the use of MPs in non-oncologic cases, and to provide an overview of this topic, especially from an epidemiologic point of view. Three different databases (PubMed, Scopus, and Web of Science) were searched for relevant articles, and further references were obtained by cross-referencing. Sixty-nine studies met the inclusion criteria, reporting on cases of MP in non-oncologic cases. A total of 2598 MPs were retrieved. Among these, 1353 (52.1%) were distal femur MPs, 941 (36.2%) were proximal femur MPs, 29 (1.4%) were proximal tibia MPs and 259 (10.0%) were total femur MPs. Megaprostheses were most commonly used to treat periprosthetic fractures (1158 cases, 44.6%), in particular in the distal femur (859, 74.2%). Overall, complications were observed in 513 cases (19.7%). Type I (soft tissue failures) and type IV (infection) according to the Henderson classification were the most frequent (158 and 213, respectively). In conclusion, patients with severe post-traumatic deformities and/or significant bone loss who have had previous septic complications should be considered as oncologic patients, not because of the disease, but because of the limited therapeutic options available. The benefits of this treatment include relatively short operative times and immediate weight-bearing, thus making MP particularly attractive in the lower limb. Full article

The potential uses of 3D scan data in the construction industry have been extensively researched in the last 20 years, with many benefits over traditional methods proclaimed by researchers. However, despite their advocated benefits, their implementation in actual construction sites remains low. This research aims to discover the potential uses of 3D scan data for construction management purposes and the limitations and barriers to their implementation and widespread adoption. Previous research into the topic was analysed to discover what technologies were used for generating 3D scan data, for what purpose and what issues were identified. These discoveries were then used to specify the potential uses of 3D scan data for, primarily, progress monitoring and quality control, which were then cross-referenced with all known limitations and barriers from the literature and the researchers’ own experience. Research has shown that, currently, there are numerous issues with both the capabilities of current technical solutions and with the construction industry’s readiness, which hinder mass adoption. Potential for breakthroughs, fortunately, does exist; however, greater impetus from the construction industry is needed to drive forward the demand for better technical solutions, which would resolve current issues and lead to the widespread adoption of 3D scan data for construction management-related uses. Fungal prosthetic joint infection (fPJI) is a rare complication; nonetheless, it represents a significant diagnostic and therapeutic challenge. There are no official guidelines on the most effective approach to identify and treat fPJIs. This systematic review aims to review the current literature on fPJI management and provide a comprehensive overview of this topic, especially from an epidemiologic point of view. Studies eligible for this systematic review were identified through an electronic systematic search of PubMed, Scopus, and Web of Science until 30 September 2022. Further references were obtained by cross-referencing. Sixty-three studies met the inclusion criteria, reporting on 372 cases of fPJI; such cases were described mostly in case reports and small case series with only a few larger cohort studies. Diagnosis of fPJI is challenging because of its chronic and indolent clinical course; it is further complicated by the technical difficulty of harvesting fungal cultures. A two-stage revision was the primary procedure in 239 (64.2%) patients whereas DAIR and one-stage approaches were reported in 30 (8.0%) and 18 (4.8 %) cases. In conclusion, our study highlights the heterogeneity of the reported treatments of fPJI, particularly in terms of medical management. With concern to a surgical approach, a two-stage revision arthroplasty is generally suggested, considering fPJI a delayed or late infection. The need for multicenter, prospective studies to provide standardized protocols and improve the treatment of fungal PJI clearly emerges. Full article

Artificial intelligence (AI), a rousing advancement disrupting a wide spectrum of applications with remarkable betterment, has continued to gain momentum over the past decades. Within breast imaging, AI, especially machine learning and deep learning, honed with unlimited cross-data/case referencing, has found great utility encompassing four facets: screening and detection, diagnosis, disease monitoring, and data management as a whole. Over the years, breast cancer has been the apex of the cancer cumulative risk ranking for women across the six continents, existing in variegated forms and offering a complicated context in medical decisions. Realizing the ever-increasing demand for quality healthcare, contemporary AI has been envisioned to make great strides in clinical data management and perception, with the capability to detect indeterminate significance, predict prognostication, and correlate available data into a meaningful clinical endpoint. Here, the authors captured the review works over the past decades, focusing on AI in breast imaging, and systematized the included works into one usable document, which is termed an umbrella review. The present study aims to provide a panoramic view of how AI is poised to enhance breast imaging procedures. Evidence-based scientometric analysis was performed in accordance with the Preferred Reporting Items for Systematic reviews and Meta-Analyses (PRISMA) guideline, resulting in 71 included review works. This study aims to synthesize, collate, and correlate the included review works, thereby identifying the patterns, trends, quality, and types of the included works, captured by the structured search strategy. The present study is intended to serve as a “one-stop center” synthesis and provide a holistic bird’s eye view to readers, ranging from newcomers to existing researchers and relevant stakeholders, on the topic of interest. Diving or hyperbaric oxygen therapy with increased partial pressures of oxygen (pO2) can have adverse effects such as central nervous system oxygen toxicity or pulmonary oxygen toxicity (POT). Prevention of POT has been a topic of interest for several decades. One of the most promising techniques to determine early signs of POT is the analysis of volatile organic compounds (VOCs) in exhaled breath. We reanalyzed the data of five studies to compose a library of potential exhaled markers for the early detection of POT. GC-MS data from five hyperbaric hyperoxic studies were collected. Wilcoxon signed-rank tests were used to compare baseline- and postexposure measurements; all ion fragments that significantly varied were compared by similarity using the National Institute of Standards and Technology (NIST) library. All identified molecules were cross-referenced with open-source databases and other scientific publications on VOCs to exclude compounds that occurred as a result of contamination, and to identify the compounds most likely to occur due to hyperbaric hyperoxic exposure. After identification and removal of contaminants, 29 compounds were included in the library. This library of hyperbaric hyperoxic-related VOCs can help to advance the development of an early noninvasive marker of POT. It enables validation by others who use more targeted MS-related techniques, instead of full-scale GC-MS, for their exhaled VOC research. Full article

Image registration is an important research topic in medical image-guided therapy, which is dedicated to registering the high-dose imaging sequences with low-dose/faster means. Registering computer tomography (CT) scanning sequences with cone beam computer tomography (CBCT) scanning sequences is a typical application and has been widely used in CBCT-guided radiotherapy. The main problem is the difference in image clarity of these two image sequences. To solve this problem, for the single projection image sequence matching tasks encountered in medical practice, a novel local quality based curved section encoding strategy is proposed in this paper, which is called the high-quality curved section (HQCS). As an optimized cross-section regularly encoded along the sequence of image, this curved section could be used in order to solve the matching problem. Referencing the independent ground truth provided by medical image physicians, with an experiment combined with the four most widely used indicators used on image registration, matching performance of HQCS on CT/CBCT datasets was tested with varying clarity. Experimental results show that the proposed HQCS can register the CT/CBCT effectively and outperforms the commonly used methods. Specifically, the proposed HQCS has low time complexity and higher scalability, which indicates that the application enhanced the task of diagnosis. Full article

Osteochondritis dissecans (OCD) is a chronic and painful joint condition that can occur from childhood through to adult life. Microtrauma, vascular insufficiency, or abnormal endochondral ossification are the most common causes of OCD. Reconstructive techniques for OCD of the knee are typically necessary when either non-operative or reparative/regenerative operative treatments fail, or when the OCD is irreversible. To analyze the clinical outcomes and failure rates of fresh osteochondral allograft transplantation (FOCA) used as a reconstructive strategy in OCD patients, an in-depth search was carried out on the PubMed, Scopus, and Web of Science databases concerning the existing evidence related to the use of FOCA for OCD patients in the knee joint. A total of 646 studies were found through the search and 2 studies were added after a cross-referenced examination of the articles within the bibliography. Six studies with a total of 303 OCD lesions treated with FOCA, with a mean follow-up of 6.3 years, were included. Although a limited number of low-level evidence studies on this topic are available in previous research, satisfactory clinical results and survival rates of the reconstruction are reported. However, to better define the real advantages of FOCA in the healing process of OCD lesions, comparative studies with different techniques are needed. Full article

Limiting the increase in CO2 concentrations in the atmosphere, and at the same time, meeting the increased energy demand can be achieved by applying carbon capture, utilization and storage (CCUS) technologies, which hold potential as the bridge for energy and emission-intensive industries to decarbonization goals. At the moment, the only profitable industrial large-scale carbon sequestration projects are large-scale carbon dioxide enhanced oil recovery (CO2-EOR) projects. This paper gives a general overview of the indirect and direct use of captured CO2 in CCUS with a special focus on worldwide large-scale CO2-EOR projects and their lifecycle emissions. On the basis of scientific papers and technical reports, data from 23 contemporary large-scale CO2-EOR projects in different project stages were aggregated, pointing out all the specificities of the projects. The specificities of individual projects, along with the lack of standardized methodologies specific for estimating the full lifecycle emissions resulting from CO2-EOR projects, pose a challenge and contribute to uncertainties and wide flexibilities when estimating emissions from CO2-EOR projects, making the cross-referencing of CO2-EOR projects and its comparison to other climate-mitigation strategies rather difficult. Pointing out the mentioned project’s differentiations and aggregating data on the basis of an overview of large-scale CO2-EOR projects gives useful information for future work on the topic of a CO2-EOR project’s lifecycle emissions. Artificial intelligence (AI) is bringing about enormous changes in everyday life and today’s society. Interest in AI is continuously increasing as many countries are creating new AI-related degrees, short-term intensive courses, and secondary school programs. This study was conducted with the aim of identifying the interrelationships among topics based on the understanding of various bodies of knowledge and to provide a foundation for topic compositions to construct an academic body of knowledge of AI. To this end, machine learning-based sentence similarity measurement models used in machine translation, chatbots, and document summarization were applied to the body of knowledge of AI. Consequently, several similar topics related to agent designing in AI, such as algorithm complexity, discrete structures, fundamentals of software development, and parallel and distributed computing were identified. The results of this study provide the knowledge necessary to cultivate talent by identifying relationships with other fields in the edutech field. Full article
