Background and Objectives: Tooth impaction is a common developmental dental anomaly characterized by the failure of eruption within the expected physiological timeframe. Permanent canines represent the second most frequently impacted teeth after third molars and may lead to functional, esthetic, and orthodontic complications. This systematic review aimed to synthesize current evidence regarding the incidence, anatomical distribution, etiological determinants, and diagnostic evaluation of permanent canine impaction. Materials and Methods: A systematic literature search was conducted in PubMed, PubMed Central, and ScienceDirect for studies published between December 2009 and December 2025. Studies reporting prevalence data, anatomical positioning, etiological factors, or imaging characteristics of permanent canine impaction were included. Study selection followed PRISMA 2020 guidelines, and 31 studies were included in the qualitative synthesis. Two independent reviewers screened titles, abstracts, and full texts. Methodological quality was assessed using the Joanna Briggs Institute Critical Appraisal Tools. Results: Thirty-one studies met the inclusion criteria and were included in the qualitative synthesis. The reported prevalence of maxillary canine impaction ranged from 0.97% to 7.10%, while mandibular impaction occurred less frequently. Palatal displacement represented the most common positional pattern. Major etiological factors included retained deciduous canines, dental arch constriction, supernumerary teeth, odontomas, and genetic anomalies such as lateral incisor agenesis. Cone-Beam Computed Tomography (CBCT) demonstrated superior diagnostic accuracy compared with panoramic radiography. Conclusions: Permanent canine impaction is a multifactorial condition predominantly influenced by local anatomical and environmental factors, with genetic predisposition acting as a secondary contributor. Early diagnosis and appropriate imaging assessment are essential to prevent complications such as root resorption and to optimize treatment outcomes. Full article

Pediatric obstructive sleep apnea (OSA) is a highly prevalent, multifactorial, and often underdiagnosed condition with significant consequences for cognitive and behavioral development. Early detection and timely multidisciplinary interventions are essential, particularly in children with craniofacial anomalies or syndromes associated with increased OSA risks, to prevent long-term complications. This narrative review explores the orthodontists’ role in the interdisciplinary management of pediatric OSA, focusing on early screening for craniofacial risk factors and implementing interceptive orthodontic interventions that support favorable airway development and growth modulation. Through early and frequent interaction with pediatric patients, orthodontists are well-positioned to identify clinical signs of airway-related abnormalities and craniofacial risk factors such as mandibular and maxillary retrognathism, maxillary constriction, and high-arched palatal vaults. Orthodontic interventions such as rapid maxillary expansion (RME), mandibular advancement, and myofunctional therapy may improve airway patency in selected cases. These approaches should be coordinated and integrated within the multidisciplinary team, including orthodontists, pediatricians, sleep specialists, ENT specialists, and speech-language pathologists. Furthermore, caregivers’ involvement and patients’ compliance are keys to success. Despite encouraging clinical observations, current evidence is limited by heterogeneity and a lack of long-term outcome data. Future research should prioritize well-designed prospective trials, explore the effectiveness of combined therapeutic strategies, and support the development of standard diagnostic protocols. Equally important is a stronger focus on early diagnosis and preventive measures to enhance patient outcomes and long-term treatment strategies. Integrating orthodontists into early OSA care is essential for optimizing outcomes and reducing long-term morbidity. Full article

Background/Objectives: Currently, clear aligners are preferred to conventional appliances, especially among adult patients. However, the use of aligners for treating maxillary constriction is still debated in the literature. Therefore, the purpose of this study was to assess maxillary dentoalveolar expansion following clear aligner therapy in adults using CBCT scans. Methods: The study sample encompassed 50 non-growing patients (27 females and 23 males) aged 20 to 42 undergoing clear aligner orthodontics without dental extractions or auxiliaries. Transverse linear distances were measured on initial and final CBCTs and, subsequently, analysed through paired t-test and ANOVA. We considered alveolar bone measurements and interdental widths measured at the buccal apices and cusps from canines to second molars. Results: The buccal alveolar ridge width showed the greatest expansion (1.01 ± 0.38 mm), followed by the palatal alveolar ridge and maxillary alveolar bone. Statistically significant improvements were observed for all interdental measurements. The most considerable changes occurred in the interpremolar cusp distances, while the least changes were seen in the intermolar apex distances. At the cusp level, the average interpremolar widths increased by 3.44 ± 0.22 mm for the first premolars and 3.14 ± 0.27 mm for the second ones. Conclusions: Clear aligner treatment can effectively manage a constricted maxillary arch. We found significant changes in the maxillary alveolar bone. Both inter-apex and inter-cusp widths increased in all teeth, with the highest values in the premolars. Moreover, the increases in interdental distances at both apex and cusp levels were related to tooth position. Full article

Objectives: This study examines the association between a predominantly soft-textured diet and clinical signs of maxillary bone hypoplasia, such as maxillary constriction and related malocclusions like crossbite. A secondary aim is to assess whether this diet correlates with dental caries and higher plaque index in children with early mixed dentition. Methods: A total of 106 pediatric patients (4–12 years) were enrolled during routine visits (June 2022–February 2024), divided into 53 “cases” (maxillary hypoplasia and malocclusions) and 53 “controls” (normal maxillary development, no malocclusions). Patients with congenital craniofacial malformations were excluded. Dietary habits were assessed using a food questionnaire categorizing foods into four consistencies (“Semi-Liquid”, “Creamy”, “Soft”, and “Solid”). The DMFT index was calculated, considering only missing teeth due to caries. The plaque index (PI) was recorded at the first visit to evaluate the relationship between food texture and plaque accumulation. Statistical analyses included Student’s t-test, Z-test, Chi-square test, and Fisher’s exact test. Results: Mixed breastfeeding was common in both groups with no significant difference. However, natural breastfeeding was significantly more frequent in the non-crossbite group. A high-arched palate was more prevalent in the crossbite group (p = 0.042 *). Soft food consumption was significantly higher in the crossbite group compared to controls (p = 0.032 *). A statistically significant association was found between caries prevalence (DMFT > 0) and posterior crossbite (p = 0.04). Furthermore, the relationship between the dental plaque index and food consistency demonstrated a statistically significant result. In particular, there was a correlation between soft food and semi-solid foods and the plaque index (χ2 = 3.55, p = 0.04). Conclusions: Posterior crossbite is associated with increased consumption of soft foods, potentially reducing the mechanical stimulation essential for maxillary growth. Conversely, non-crossbite subjects consume more hard foods and are more frequently breastfed naturally, reinforcing their role in craniofacial development. Additionally, posterior crossbite is associated with higher caries prevalence, indicating a potential connection between occlusion and oral health. Dietary texture also influences oral hygiene, with soft and semi-solid foods correlating with increased plaque accumulation, while no association was found with solid or hard foods. Full article

Class III malocclusion prevalence varies significantly among racial groups, with the highest prevalence observed in southeast Asian populations at 15.80%. These malocclusions often involve maxillary retrognathism, mandibular prognathism, or both, accompanied by maxillary constriction and crossbites. Comprehensive treatment should address anteroposterior, transverse, and vertical imbalances. Microimplant-assisted rapid palatal expansion (MARPE) has shown high success rates for transverse maxillary expansion in late adolescents and adults, presenting a viable alternative to surgically-assisted rapid palatal expansion (SARPE). This case report aims to demonstrate the successful treatment of a young adult female with borderline Class III malocclusion using MARPE and mandibular backward rotation (MBR) techniques. A 21-year-old female presented with a Class III skeletal pattern, anterior/posterior crossbites, and mild dental crowding. Despite her concerns about a concave facial profile, the patient declined orthognathic surgery due to a negative experience reported by a friend. The treatment plan included MARPE to correct maxillary transverse deficiency and MBR to alleviate Class III malocclusion severity. Lower arch distalization was performed using temporary anchorage devices (TADs) on the buccal shelves, and Class II elastics were used to maintain MBR and prevent retroclination of the lower labial segment during anterior retraction. Significant transverse correction was achieved, and the severity of Class III malocclusion was reduced. The lower dentition was effectively retracted, and the application of Class II elastics helped maintain MBR. The patient’s final facial profile was harmonious, with well-aligned dentition and a stable occlusal relationship. The treatment results were well-maintained after one year. The MARPE with MBR approach presents a promising alternative for treating borderline Class III cases, particularly for patients reluctant to undergo orthognathic surgery. This case report highlights the effectiveness of combining MARPE and MBR techniques in achieving stable and satisfactory outcomes in the treatment of Class III malocclusion. Full article

One of the important parameters in orthodontics is the perimeter of the dental arch. Precise assessment is necessary in cases of maxillary constriction treated with a rapid maxillary expander (RME). The orthodontic software allows customization of the processes from diagnosis to manufacturing of the treatment device. The aim of the present study is to evaluate a relationship between the parameters of the dental arch—perimeter, width, and length—and to follow the changes during treatment. The study is based on the digital measurements of 3D models of 33 patients treated with a digitally planned and printed RME. In the results an increase of 3.99 mm in perimeter was achieved. The rest of the parameters were changed as follows: The width of the dental arch was increased in the premolar area by an average of 3.3 mm; in the area of the first molars, the increase was 4.41 mm; the length of the dental arch in the anterior segment was reduced by an average of 0.54 mm; and the whole length by 0.52 mm. Correlation between the studied variables was described by linear equations. In conclusion, rapid maxillary expansion is a reliable method for gaining predictable space in the dental arch. (1) Background: A large number of patients of orthodontic clinics are diagnosed with improper jaw relationships. Intraoral scanners have become an important part of orthodontic practice and provide an opportunity to measure the changes in the width of dental arches. The purpose of the study was to evaluate the impact of removable appliances used over a 10-month period on growth changes in children with narrowed jaw dimensions. (2) Methods: Twenty four patients were included in the study (a study group—patients, treated with removable appliances in the upper dental arch for a minimum of 10 months; a control group—patients with no craniofacial abnormalities and who did not require orthodontic treatment). A panoramic radiograph and digital intraoral scan were taken, followed by palatal width measurements in Ortho-CAD before treatment, and after a period of 10 months of treatment with removable appliances. (3) Results: After a period of 10 months of the treatment, the study group had a statistically significantly greater mean change in the anterior width of the upper dental arch than the control group. (4) Conclusions: The use of removable appliances in children with narrowed maxillary transverse dimension contributes to offsetting growth changes in comparison to children with normal occlusion. The abnormal growth of the craniofacial bone leads to skeletal and dental defects, which result in the presence of malocclusions. Not all causes of malocclusion have been explained. In the development of skeletal abnormalities, attention is paid to general deficiencies, including of vitamin D3 (VD3), which causes rickets. Its chronic deficiency may contribute to skeletal malocclusion. The aim of the study was to assess the impact of VD3 deficiency on the development of malocclusions. The examination consisted of a medical interview, oral examination, an alginate impression and radiological imaging, orthodontic assessment, and taking a venous blood sample for VD3 level testing. In about 42.1% of patients, the presence of a skeletal defect was found, and in 46.5% of patients, dentoalveolar malocclusion. The most common defect was transverse constriction of the maxilla with a narrow upper arch (30.7%). The concentration of vitamin 25 (OH) D in the study group was on average 23.6 ± 10.5 (ng/mL). VD3 deficiency was found in 86 subjects (75.4%). Our research showed that VD3 deficiency could be one of an important factor influencing maxillary development. Patients had a greater risk of a narrowed upper arch (OR = 4.94), crowding (OR = 4.94) and crossbite (OR = 6.16). Thus, there was a link between the deficiency of this hormone and the underdevelopment of the maxilla. Full article

Background and Objectives: Palatal expansion is a common orthodontic technique able to increase the transverse changes for subjects with constricted maxillary arches. The aim of the present investigation was to evaluate through a systematic review the tomography effectiveness of different palatal expander approaches. Materials and Methods: The database used to perform the screening and determine the eligibility of the clinical papers was PubMed (Medline). Results: The database search included a total of 284 results, while 271 articles were excluded. A total of 14 articles were included for the qualitative assessment. Conclusions: The effectiveness of the present studies reported that skeletal expansion was a useful approach to increase the transverse changes for subjects with constricted maxillary arches.
