Interpupillary Distance and Peripapillary Myopic Changes: A Pilot Study in a Glaucomatous Cohort

Background/Objectives: Myopia is associated with peripapillary changes, namely, gamma peripapillary atrophy (γPPA) and optic disc ovalization, estimated by the ovality index (OI). These changes have been suggested to be promoted by adduction. Recent studies highlight that near reading significantly contributes to the development and progression of myopia and that the interpupillary distance (IPD) influences vergence amplitudes. While both adduction and convergence are involved during near reading, a potential link between IPD and myopic peripapillary changes has not yet been explored. We, therefore, sought to determine whether IPD is related to the OI or γPPA width. Methods: In this monocentric cross-sectional study, 100 eyes from 100 adults (mean age of 62.6 ± 13.7 years) were analyzed. Axial length (AL), refractive error, and IPD were recorded. The OI and γPPA width were assessed using spectral-domain Optical Coherence Tomography. Pearson correlations and multivariable linear regressions were performed, adjusting for age, gender, and myopia status. Results: IPD showed no significant correlation with the OI (r = 0.001; p = 0.989) or γPPA (r = −0.028; p = 0.789). A weak, non-significant correlation was found between IPD and AL (p = 0.059). In contrast, AL was strongly correlated with both a lower OI and wider γPPA (p < 0.001). Conclusions: These findings suggest that IPD-related biomechanical forces do not influence optic nerve head (ONH) shape or γPPA. Axial elongation remains the key driver of myopic ONH remodeling. This study investigates whether eye movement abnormalities can differentiate between distinct clinical annotations of dyslexia, attention deficit, or school learning difficulties in children. Utilizing a selection of saccade and vergence eye movement data from a large clinical dataset recorded across 20 European centers using the REMOBI and AIDEAL technologies, this research study focuses on individuals annotated with only one of the three annotations. The selected dataset includes 355 individuals for saccade tests and 454 for vergence tasks. Eye movement analysis was performed with AIDEAL software. Key parameters, such as amplitude, latency, duration, and velocity, are extracted and processed to remove outliers and standardize values. Machine learning models, including logistic regression, random forest, support vector machines, and neural networks, are trained using a GroupKFold strategy to ensure patient data are present in either the training or test set. Results from the machine learning models revealed that children annotated solely with dyslexia could be successfully identified based on their saccade and vergence eye movements, while identification of the other two categories was less distinct. Statistical evaluation using the Kruskal–Wallis test highlighted significant group mean differences in several saccade parameters, such as a velocity and latency, particularly for dyslexics relative to the other two groups. These findings suggest that specific terminology, such as “dyslexia”, may capture unique eye movement patterns, underscoring the importance of eye movement analysis as a diagnostic tool for understanding the complexity of these conditions. This study emphasizes the potential of eye movement analysis in refining diagnostic precision and capturing the nuanced differences between dyslexia, attention deficits, and general learning difficulties. Background: To understand if binocular vision disorders are associated with Digital Eye Strain Syndrome (DESS), a study protocol is needed to ensure consistency across observational studies. This study aims to test the feasibility of a protocol to assess DESS, screen time, binocular vision, and dry eye. Methods: DESIROUS is an observational cross-sectional study among Polytechnic students at the Lisbon School of Health Technology, Portugal. The protocol includes three questionnaires (Computer Vision Syndrome Questionnaire [CVS-Q], Convergence Insufficiency Symptom Survey [CISS], and Dry Eye Questionnaire version 5 [DEQ-5]), an assessment of visual acuity and binocular vision (cover test for near and distance, stereopsis, near point convergence (NPC), near point accommodation (NPA), accommodative facility, vergence), and the ocular surface break-up tear (BUT) test. The questionnaires were validated using Cronbach’s alpha. Interobserver variability for BUT was assessed using Cohen’s Kappa, Intraclass Correlation Coefficient (ICC), and Bland–Altman analysis involving three observers (A, B, and C), compared against an expert as the gold standard. Results: A total of 18 students were included in the validation phase (mean age: 21.50 ± 0.62 years; females: 77.8%). The internal consistency of the CVS-Q (α = 0.773) and the CISS (α = 0.756) was considered good, while the DEQ-5 showed a reasonable internal consistency (α = 0.594). Observer A had the highest agreement with the gold standard (Cohen’s Kappa = 0.710 and p < 0.001; ICC = 0.924, p < 0.001). Conclusions: We provide a protocol to assess binocular vision and the ocular surface, with an emphasis on objective measures while integrating other assessment approaches. Further studies are necessary to validate this protocol, potentially incorporating new measures to enhance its validity across different populations. Full article

This study evaluated binocular visual function using automatic dual rotational Risley prisms (ADRRPs) to measure phoria and vergence ranges. Thirty-nine (mean age: 21.82 ± 1.10 years; age range: 20–24 years) healthy adults with normal binocular vision participated. Each underwent baseline refraction exams followed by phoria and vergence tests conducted using both a phoropter with Maddox rods and the ADRRPs. The results revealed a strong positive correlation between the two instruments for distance phoria (r = 0.959, p < 0.001) and near-phoria measurements (r = 0.968, p < 0.001). For vergence testing, positive fusional vergence (PFV) at distance showed a moderate-to-strong correlation for break points (r = 0.758, p < 0.001) and a moderate correlation for recovery points (r = 0.452, p < 0.001). Negative fusional vergence (NFV) at distance demonstrated a strong correlation for break points (r = 0.863, p < 0.001) and a moderate correlation for recovery points (r = 0.458, p < 0.01). Near-vergence testing showed moderate-to-strong correlations for break points (r = 0.777, p < 0.001) and recovery points (r = 0.623, p < 0.001). The inclusion of Bland–Altman analysis provides a more comprehensive evaluation of agreement between ADRRPs and the phoropter. While strong correlations were observed, systematic bias and LoA indicate that these methods are not perfectly interchangeable. The ADRRPs demonstrated potential for binocular vision assessment but require further validation for clinical application. Full article

Recent studies applying machine learning (ML) to saccade and vergence eye movements have demonstrated the ability to distinguish individuals with dyslexia, learning disorders, or attention disorders from healthy individuals or those with other pathologies. Stroke patients are known to exhibit visual deficits and eye movement disorders. This study focused on saccade and vergence measurements using REMOBI technology V3 and the Pupil Core eye tracker. Eye movement data were automatically analyzed with the AIDEAL V3 (Artificial Intelligence Eye Movement Analysis) cloud software developed by Orasis-Ear. This software computes multiple parameters for each type of eye movement, including the latency, accuracy, velocity, duration, and disconjugacy. Three ML models (logistic regression, support vector machine, random forest) were applied to the saccade and vergence eye movement features provided by AIDEAL to identify stroke patients from other groups: a population of children with learning disorders and a population with a broader spectrum of dysfunctions or pathologies (including children and adults). The different classifiers achieved macro F1 scores of up to 75.9% in identifying stroke patients based on the saccade and vergence parameters. An additional ML analysis using age-matched groups of stroke patients and adults or seniors reduced the influence of large age differences. This analysis resulted in even higher F1 scores across all three ML models, as the comparison group predominantly included healthy individuals, including some with presbycusis. In conclusion, ML applied to saccade and vergence eye movement parameters, as measured by the REMOBI and AIDEAL technology, is a sensitive method for the detection of stroke-related sequelae. This approach could be further developed as a clinical tool to evaluate recovery, compensation, and the evolution of neurological deficits in stroke patients. Full article

Background: Visual and oculomotor skills are essential components of athletic performance, particularly in sports that demand rapid visual processing and precise motor execution. This study aimed to provide a descriptive analysis of the visual and oculomotor abilities of federated university athletes and explore their relationship with self-perceived performance. Methods: A total of 52 federated university athletes (34 men and 18 women), aged 18 to 37 years, participated in this observational and descriptive study. Visual assessments were conducted using the digital COI-Sport Vision system, evaluating static and dynamic visual acuity, stereopsis, vergence, contrast sensitivity, visual memory, hand-eye coordination, anticipation time, peripheral awareness, and identification. Results: The findings revealed patterns in the visual and oculomotor skills of this cohort, particularly in parameters such as dynamic visual acuity, stereopsis, and hand-eye coordination, which are relevant to the demands of their sports. Athletes reporting a higher satisfaction with their performance exhibited faster response times in visual memory (p = 0.039) and anticipation tasks (p = 0.022). These results suggest a potential relationship between self-perceived performance and specific visual skills, although further research is needed to establish causal links. Conclusions: This study provides valuable insights into the visual and oculomotor abilities of federated university athletes, emphasizing the potential influence of self-perceived performance on visual skill execution. While the results do not allow for direct comparisons with other populations, they lay the groundwork for future research into sport-specific visual demands. Incorporating validated psychological assessments and exploring longitudinal designs could further enhance an understanding of the interplay between visual skills, motivation, and athletic performance. Background/Objectives: Ocular proprioception is implicated in balance control and heterophoria is associated with abnormal posture, though previous research focused mainly on the role of vertical phoria and the use of vertical prisms. This study aims to evaluate whether ocular misalignment and prismatic correction of horizontal phoria affect posture. Methods: Sixty-nine (N = 69) young healthy subjects were included and equally divided by horizontal distance phoria: orthophoria (n = 23), esophoria (n = 23) and exophoria (n = 23). A prism of low power (two-diopter) was placed base out on the non-dominant eye, reducing misalignment in esophorics and increasing it in exophorics more than in orthophorics. Dynamic computerized posturography was performed with the sensory organization test protocol (SOT) of the EquiTest® NeuroCom® version 8 platform both without and with prism, always maintaining subjects unaware of prism use. A mixed model for repeated measures analysis of variance was run to evaluate the main effect of prism and the interaction effect of prism with baseline phoria. Results: Composite movement strategy score without prism was 88.1 ± 2.8% (ankle-dominant strategy) and slightly increased to 89.0 ± 3.1% with prism insertion (p = 0.004), further shifting toward ankle strategy. Composite equilibrium score without prism was 80.3 ± 6.5% and remained stable with prism insertion (81.3 ± 8.2%, p = 0.117), medio-lateral and antero-posterior projection of center of gravity did not displace significantly under prism insertion (p = 0.652 and p = 0.270, respectively). At baseline, posturographic parameters were statistically independent of individual phoria, and no significant interaction between prism insertion and individual phoria was documented for any parameters (p > 0.05 for all). Secondary analysis and pairwise comparisons confirmed that the effect of prism was strongly selective on condition SOT 5 (eyes-closed, platform sway-referenced) with improvement of equilibrium (70.4 ± 9.7% with prism vs. 65.7 ± 11.6% without) and more use of ankle strategy (81.6 ± 5.3% with prism vs. 78.2 ± 6.0% without), without any interaction of phoria and ocular dominance, while the other conditions were comparable with and without prism. Conclusions: A two-diopter prism base out on the non-dominant eye induces the body to use the ankle joint more independently of individual phoria, suggesting a small improvement in postural control, while maintaining oscillations of the center of gravity unaltered. Prism seems to enhance the function of vestibular system selectively. Phoria adjustments with prismatic correction enable intervention in postural behavior. Extraocular muscles could act as proprioceptors influencing postural stability. Full article

Eccentric photorefractometry is widely used to measure eye refraction, accommodation, gaze position, and pupil size. While the individual calibration of refraction and accommodation data has been extensively studied, gaze measurements have received less attention. PowerRef 3 does not incorporate individual calibration for gaze measurements, resulting in a divergent offset between the measured and expected gaze positions. To address this, we proposed two methods to calculate the physiological vergence angle based on the visual vergence data obtained from PowerRef 3. Twenty-three participants aged 25 ± 4 years viewed Maltese cross stimuli at distances of 25, 30, 50, 70, and 600 cm. The expected vergence angles were calculated considering the individual interpupillary distance at far. Our results demonstrate that the PowerRef 3 gaze data deviated from the expected vergence angles by 9.64 ± 2.73° at 25 cm and 9.25 ± 3.52° at 6 m. The kappa angle calibration method reduced the discrepancy to 3.93 ± 1.19° at 25 cm and 3.70 ± 0.36° at 600 cm, whereas the linear regression method further improved the accuracy to 3.30 ± 0.86° at 25 cm and 0.26 ± 0.01° at 600 cm. Both methods improved the gaze results, with the linear regression calibration method showing greater overall accuracy. Full article

Virtual reality (VR) can challenge the visual system, leading to temporary oculomotor changes, though the degree of change varies among individuals. While the vergence and accommodation system plays a crucial role in VR perception, it remains unclear whether individuals whose visual functions fall outside clinical norms experience larger changes. Thus, our study aimed to investigate whether changes in vergence and accommodation responses following VR gameplay differ between individuals with and without non-strabismic binocular and accommodative disorders. To assess this, both subjective and objective measurements were conducted before and after 20 min of playing Beat Saber. Results revealed significant alterations across both subjective measurements—near point of convergence and near point of accommodation—and objective measurements, including eye refraction, accommodation lag, and accommodative microfluctuations at far. Moreover, individuals with non-strabismic binocular and accommodative disorders exhibited larger accommodative microfluctuations at far compared to the control group. Overall, these findings indicate that considering individual differences in vergence and accommodation responses is important when evaluating the impact of VR on the visual system and can be helpful in the design and use of VR systems, particularly for individuals with binocular and accommodative disorders. The Indo-Burma Range (IBR), as one of the youngest accreted units in the Eastern Neotethys, plays a crucial role in understanding the interactive relationships between the Gondwana supercontinent and its rifted microcontinents in SE Asia. However, its basement nature and tectonic evolution remain debated. Here, we conducted a comprehensive structural analysis across six sections within the IBR and correlated Late Triassic flysch units between the Western IBR (Pane Chaung Formation) and the Tethyan Himalaya. Within the Mindat section, the eastern segment of the Pane Chaung Formation unit displays top-to-east vergent overturned folds, indicating eastward backthrusting, in contrast to the prevailing top-to-west vergence structures in Kalemyo, Natchaung, Magwe and the western segment of the Mindat flysch unit. By reconstruction of this backthrust sheet, a megathrust separates the Pane Chaung Formation unit in the footwall to the west from schist units in the hanging wall to the east. The Pane Chaung Formation unit in the Western IBR and its counterparts in the Tethyan Himalaya share common characteristics, including herringbone cross-beddings, Carnian–Norian Halobia fossils, and dominant detrital zircons of 220–280, 500–620, 900–1000, and 1100–1140 Ma. Alongside the Paleozoic strata and Precambrian one-stage model ages of Mesozoic dikes, as evidenced by ɛNd (t) (−13.4 to −0.1) and ɛHf (t) (−24.2 to −0.1) in the Tethyan Himalaya, these facts suggest that the major tectonic units of the Western IBR–Tethyan Himalaya are the result of the amalgamation of a microcontinent with the West Burma Block. The transition from OIB to E-MORB and N-MORB, the rapid deepening of sedimentary waters, and the presence of the 155–152 Ma Indian ocean crust collectively indicate that the microcontinent rifted from the host East Gondwana as a fragment of the Argoland archipelago in the Late Jurassic. This identification sheds light on the orogenic processes of the doublet subduction zones in the Indo-Myanmar orogenic belt. Full article

The vestibular function is in synergism with the oculomotor vergence. Vertigo may be related to vergence disorders and conversely, vestibular pathologies may affect vergence. To consolidate this hypothesis, we conducted a study at the vestibular orthoptic clinic of the Bastogne Hospital. Fourteen patients with vertigo history appearing 2 weeks to 8 years ago, aged 30 to 65 years were studied; at the moment of the eye movement study, no patient had acute attack of vertigo. The origin of vertigo varied (Meniere’s disease, organic pathology, sensitivity to visual movement). An assessment with objective measurement of vergence (single-step protocol) was carried out with the REMOBI technology coupled with binocular video-oculography in sitting and standing positions. Four neuro-rehabilitation sessions of vergence eye movements were performed with the double-step in-depth protocol, alternating sitting and standing positions to involve different postural and vestibular functions. An assessment of vergence was done again 1 to 2 months later. The initial assessment revealed problems of magnitude and/or speed or variability of vergence for 11 of the patients relative to controls (published by the group in previous studies). After neuro-rehabilitation, an improvement was observed in eight patients. Patients reported a clear improvement of their self confidence in moving in the space. Posture measures done before rehabilitation comparing eyes fixating or closed or while making near–far vergence eye movements indicated lower medio-lateral acceleration when doing vergence eye movements in patients with vertigo history of functional origin. The results are in favor of the hypothesis of a symbiosis between vergence and vestibular function and the interest of diagnosis and rehabilitation of the vergence disorder in patients with vertigo history in the absence of acute vertigo attack. The Interaction between Vergence and Accommodation Cues in the Assessment of Fusional Vergence Range

Fusional vergence range tests are commonly used in optometric practice. The aim of this study was to investigate the possible contribution of CA/C, AC/A, and proximal cues (PCT) to the magnitude and presence of blur and recovery during the measurement of fusional vergence ranges and to determine whether the occurrence of blur is influenced by these vergence and accommodation cues. A total of 27 participants with normal binocular vision were included and AC/A, CA/C, and PCT ratios were evaluated. Blur, break, and recovery values in convergence and divergence were assessed with base-out and base-in prisms, respectively. No statistical correlations were found between AC/A, CA/C, and PCT ratios and the magnitude of blur, break, and recovery values in neither far, near, convergence, nor divergence testing conditions. However, better near point of convergence values were related to higher break values in convergence at far distances, but not at near distances. In addition, for convergence and far distance, a statistical difference was found between groups reporting and not reporting blur in AC/A stimulus and PCT ratios. The present results cannot confirm whether vergence and accommodation cues, such as AC/A, CA/C, and PCT ratios, may play an active role during the assessment of fusional vergence range. Full article

Self-supervised learning (SSL) has gained significant attention in the past decade for its capacity to utilize non-annotated datasets to learn meaningful data representations. In the medical domain, the challenge of constructing large annotated datasets presents a significant limitation, rendering SSL an ideal approach to address this constraint. In this study, we introduce a novel pretext task tailored to stimulus-driven eye movement data, along with a denoising task to improve the robustness against simulated eye tracking failures. Our proposed task aims to capture both the characteristics of the pilot (brain) and the motor (eye) by learning to reconstruct the eye movement position signal using up to 12.5% of the unmasked eye movement signal patches, along with the entire REMOBI target signal. Thus, the encoder learns a high-dimensional representation using a multivariate time series of length 8192 points, corresponding to approximately 40 s. We evaluate the learned representation on screening eight distinct groups of pathologies, including dyslexia, reading disorder, and attention deficit disorder, across four datasets of varying complexity and size. Furthermore, we explore various head architecture designs along with different transfer learning methods, demonstrating promising results with improvements of up to approximately 15%, leading to an overall macro F1 score of 61% and 61.5% on the Saccade and the Vergence datasets, respectively. Notably, our method achieves macro F1 scores of 64.7%, 66.1%, and 61.1% for screening dyslexia, reading disorder, and attention deficit disorder, respectively, on clinical data. These findings underscore the potential of self-learning algorithms in pathology screening, particularly in domains involving complex data such as stimulus-driven eye movement analysis. Full article

This cross-sectional study examined eye movement performance in patients aged 4 to 16 years. Measurements of eye movements were obtained before and after performing therapy for inhibition of four primitive reflexes, asymmetric tonic neck reflex, symmetric tonic neck reflex, labyrinthine tonic reflex and Moro reflex. Subsequently the scores of the four primitive reflexes were compared with the results of five variables: fixation maintenance, % mean saccade size, motility excursions, fixations during excursions and mean duration of fixations. The comparisons showed a significant reduction in evidence of fixation maintenance as well as mean saccade size due to the inhibition of the four primitive reflexes. There was also a significant increase in ocular motility while fixations per saccade and average duration of fixations also decreased significantly. Visual balance between values of both eyes improved in all tests. A device called VisagraphTM III, which measures eye movements, was used for data collection. These results suggest that the oculomotor improvements reflect the involvement of other maturational processes such as the emergence and inhibition of primitive reflexes, the whole reorganization being key to future reading and attentional processes. Background: Dyslexia, a neurodevelopmental disorder affecting reading skills, poses significant challenges to children’s academic performance and quality of life. Despite its rising prevalence and adverse effects, understanding of its relationship with vision anomalies remains limited, particularly in low-resource settings like Nigeria. This study aims to assess the prevalence of binocular vision anomalies (BVAs) among children with and without dyslexia in Kano, Nigeria. Methods: This is a hospital-based, cross-sectional, matched-paired, controlled study conducted at the Aminu Kano Teaching Hospital (AKTH) Eye Clinic in Northern Nigeria. The study included school children who visited the AKTH Eye Clinic from January 2018 to December 2022. Visual acuity tests, external eye examinations and accommodative, binocular vision and oculomotor skills tests were conducted. Descriptive statistics, independent t-tests, Mann–Whitney U tests and Fisher’s exact tests were conducted, with a significance level set at p < 0.05. Results: Forty-four children aged 12 ± 2 years participated. Children with dyslexia reported higher rates of visual symptoms than those without dyslexia, Blurring vision, visual distortion and eye strain were the most prevalent (p < 0.05) BV symptoms. Accommodative insufficiency), was the most common visual abnormality, and was significantly higher in children with dyslexia than those without dyslexia (45.5% vs. 18.2%). However, other visual anomalies showed no significant difference between groups. There was a high prevalence of binocular vision anomalies in both groups. Binocular test findings showed dyslexic children had significantly lower distance positive fusional vergence recovery values (p = 0.005). All cases of convergence insufficiency alone were found in the non-dyslexic group. Conclusions: The study found that children with dyslexia residing in Northern Nigeria demonstrated higher rates of visual symptoms, more accommodative insufficiency and lower distance positive fusional vergence recovery values compared to their non-dyslexic counterparts.
