Atrial fibrillation (AF) is an increasingly recognized comorbidity in patients with cancer. Indeed, cancer patients have a significantly higher incidence of AF than that observed in the general population. A reciprocal relationship between these two diseases has been observed, as much as some assume AF to be a marker for occult cancer screening, especially in older adults. The pathophysiological mechanisms are many and varied, including the underlying pro-inflammatory state, specific treatments (chemo- and radiotherapy), and surgery. The therapeutic management of patients with cancer and AF involves the same rhythm and frequency control strategies as the general population; however, the numerous interactions with chemotherapeutics, which lead to a significant increase in side effects, as well as the extreme fragility of the patient, should be considered. Anticoagulant therapy is also a complex challenge to address, as bleeding and stroke risk scores have not been fully assessed in this subpopulation. Furthermore, in large studies establishing the efficacy of direct oral anticoagulants (DOACs), cancer patients have been underrepresented. In this review, we elaborate on the mechanisms linking AF to cancer patients with a particular focus on the therapeutic challenges in this population. Full article

Stand-off detection of latent traces avoids the scene alteration that might occur during close inspection by handheld forensic lights. Here, we describe a novel sensor, named Crime Light Imaging (CLI), designed to perform high-resolution photography of targets at a distance of 2–10 m and to visualize some common latent traces. CLI is based on four high-power illumination LEDs and one color CMOS camera with a motorized objective plus frontal filters; the LEDs and camera could be synchronized to obtain short-exposure images weakly dependent on the ambient light. The sensor is integrated into a motorized platform, providing the target scanning and necessary information for 3D scene reconstruction. The whole system is portable and equipped with a user-friendly interface. The preliminary tests of CLI on fingerprints at distance of 7 m showed an excellent image resolution and drastic contrast enhancement under green LED light. At the same distance, a small (1 µL) blood droplet on black tissue was captured by CLI under NIR LED, while a trace from 15 µL semen on white cotton became visible under UV LED illumination. These results represent the first demonstration of true stand-off photography of latent traces, thus opening the way for a completely new approach in crime scene forensic examination. Full article

Lung cancer is the leading cause of cancer-related deaths in Europe, with low survival rates primarily due to late-stage diagnosis. Early detection can significantly improve survival rates, but lung cancer screening is not currently implemented in Italy. Many countries have implemented lung cancer screening programs for high-risk populations, with studies showing a reduction in mortality. This review aimed to identify key areas for establishing a lung cancer screening program in Italy. A literature search was conducted in October 2022, using the PubMed and Scopus databases. Items of interest included updated evidence, approaches used in other countries, enrollment and eligibility criteria, models, cost-effectiveness studies, and smoking cessation programs. A literature search yielded 61 scientific papers, highlighting the effectiveness of low-dose computed tomography (LDCT) screening in reducing mortality among high-risk populations. The National Lung Screening Trial (NLST) in the United States demonstrated a 20% reduction in lung cancer mortality with LDCT, and other trials confirmed its potential to reduce mortality by up to 39% and detect early-stage cancers. However, false-positive results and associated harm were concerns. Economic evaluations generally supported the cost-effectiveness of LDCT screening, especially when combined with smoking cessation interventions for individuals aged 55 to 75 with a significant smoking history. Implementing a screening program in Italy requires the careful consideration of optimal strategies, population selection, result management, and the integration of smoking cessation. Resource limitations and tailored interventions for subpopulations with low-risk perception and non-adherence rates should be addressed with multidisciplinary expertise. Full article

Who Is at Higher Risk of SARS-CoV-2 Reinfection? Results from a Northern Region of Italy

The SARS-CoV-2 pandemic continues to spread worldwide, generating a high impact on healthcare systems. The aim of the study was to examine the epidemiological burden of SARS-CoV-2 reinfections and to identify potential related risk factors. A retrospective observational study was conducted in Liguria Region, combining data from National Vaccines Registry and Regional Chronic Condition Data Warehouse. In the study period (September 2021 to May 2022), 335,117 cases of SARS-CoV-2 infection were recorded in Liguria, of which 15,715 were reinfected once. During the Omicron phase (which predominated from 3 January 2022), the risk of reinfection was 4.89 times higher (p < 0.001) than during the Delta phase. Unvaccinated and vaccinated individuals with at least one dose for more than 120 days were at increased risk of reinfection compared with vaccinated individuals with at least one dose for ≤120 days, respectively (odds ratio (OR) of 1.26, p < 0.001; OR of 1.18, p < 0.001). Healthcare workers were more than twice as likely to be reinfected than non-healthcare workers (OR of 2.38, p < 0.001). Lower ORs were seen among people aged 60 to 79 years. Two doses or more of vaccination were found to be protective against the risk of reinfection rather than a single dose (mRNA vaccines: OR of 0.06, p < 0.0001, and OR of 0.1, p < 0.0001; vector vaccines: OR of 0.05, p < 0.0001). Patients with chronic renal failure, cardiovascular disease, bronchopneumopathy, neuropathy and autoimmune diseases were at increased risk of reinfection (OR of 1.38, p = 0.0003; OR of 1.09, p < 0.0296; OR of 1.14, p = 0.0056; OR of 1.78, p < 0.0001; OR of 1.18, p = 0.0205). Estimating the epidemiological burden of SARS-CoV-2 reinfections and the role played by risk factors in reinfections is relevant for identifying risk-based preventive strategies in a pandemic context characterized by a high circulation of the virus and a high rate of pathogen mutations. The numerical crop growth model Italian Vineyard Integrated Numerical model for Estimating physiological values (IVINE) was developed in order to evaluate environmental forcing effects on vine growth. The IVINE model simulates vine growth processes with parameterizations, allowing the understanding of plant conditions at a vineyard scale. It requires a set of meteorology data and soil water status as boundary conditions. The primary model outputs are main phenological stages, leaf development, yield, and sugar concentration. The model requires setting some variety information depending on the cultivar: At present, IVINE is optimized for Vitis vinifera L. Nebbiolo, a variety grown mostly in the Piedmont region (northwestern Italy). In order to evaluate the model accuracy, IVINE was validated using experimental observations gathered in Piedmontese vineyards, showing performances similar or slightly better than those of other widely used crop models. The results of a sensitivity analysis performed to highlight the effects of the variations of air temperature and soil water potential input variables on IVINE outputs showed that most phenological stages anticipated with increasing temperatures, while berry sugar content saturated at about 25.5 °Bx. Long-term (60 years, in the period 1950–2009) simulations performed over a Piedmontese subregion showed statistically significant variations of most IVINE output variables, with larger time trend slopes referring to the most recent 30-year period (1980–2009), thus confirming that ongoing climate change started influencing Piedmontese vineyards in 1980. A new process is described for preparing very pure linear alkanethiols and linear α,ω-alkanedithiols using a sequential alkylation of the title compound, followed by a ring closure to quantitatively give the corresponding 3-methyl[1,3]thiazolo[3,2-a]-[3,1]benzimidazol-9-ium salt and the alkanethiol derivative under mild conditions. The alkanethiol and the heteroaromatic salt are easily separated by a simple extraction process. The intermediate thiazolium quaternary salts resulting from the first reaction step can be isolated in quantitative yields, affording an odourless protected form of the thiols. Full article
