Round 1

Reviewer 1 Report

Title: Informative title that tells the reader what to expect

Background: Informative and well-written background. Could potentially add a small section to address the impacts of covid-19 on nursing and people with intellectual disabilities. This would make this section even stronger.

Aim: The aim of the study should be stated more clearly. The last sentence of this section is very long and is hard to understand.

Methods: This section is very thorough, with many details that are important and some that could be taken out. The mini literature review seemed out of place, maybe it should have been placed at the beginning of the methods section.

Results: well done.

Discussion: It would be interesting to compare the findings from Derek’s case to the literature and see if there are any discrepancies or similarities. More connection to the literature would strengthen this section.

Limitations: There is also an important limitation of geographical location and the resources that are available to Derek and this health system.

Conclusion: This section should clearly state that the conclusions made are based on one patient’s case.

Reviewer 2 Report

On the basis of a single case, the authors compare 4 different scenarios of the functioning of medical care for
intellectual disability and associated multiple chronic and complex health problems patients.
Although the analysis seems suggestive, the conclusions are intentional obvious,
the methodology of single-case research has great limitations as to the generalization of such research.
It would be useful to extend this study to a few or even several cases.

1. The methodology of single-case research has great limitations as to the generalization of such research. Please note that this method is a way to lead to methodological limitations when it comes to the resulting generalization and the conclusion.    2. Make it clear in the abstract that the analysis is based on one case (n = 1).   3. It would be useful to extend this study to a few or even several cases.  Explain why the study cannot be completed with a few additional cases. What motivated the authors to choose the single-case method.
