Family Members’ Explanatory Models of Cancer Anorexia–Cachexia

The experience of bearing witness to the lack of appetite and involuntary weight loss that characterizes cancer anorexia–cachexia syndrome (CACS) is reported to be stressful for family members. Research identifies that family members engage in a wide range of behaviors in response to a relative who shows minimal interest in eating and is literally ‘wasting away’ before their eyes. Some families, though concerned about the symptoms of CACS, do not dwell excessively on the patient’s nutritional intake while others continually harass the patient to eat and petition health care providers for aggressive nutritional interventions to eat in an attempt to stave off further physical deterioration. While studies have detailed how family members respond to a terminally ill relative with CACS, empirical work explicating the explanatory models of CACS that they hold is lacking. Explanatory models (EMs) reflect the beliefs and ideas that families have about why illness and symptoms occur, the extent to which they can be controlled, how they should be treated, and how interventions should be evaluated. To address this gap in the literature, a grounded theory study guided by Kleinman’s Explanatory Model questions was conducted with 25 family members of advanced cancer patients. The core category of ‘Wayfaring’ integrates the key categories of the model and maps onto Kleinman’s questions about CACS onset, etiology, natural course, physiological processes/anatomical structures involved, treatment, and the impacts of disease on patient and family. Findings suggest that a divergence between some biomedical constructions of CACS and explanatory models held by family members may fuel the family–health care provider conflict, thereby providing direction for communication with families about care of the patient with anorexia–cachexia. The present study aims to establish a mixed lubrication model for staved stern tube bearings lubricated with water, in which the average Reynolds equation and a KE elastic–plastic contact model are introduced to calculate the hydrodynamic pressure and contact pressure, respectively. The difference in the mixed lubrication behaviors between circular- and flat-staved bearings is compared; moreover, the effects of the number of staves on the mixed-lubrication performance of these two kinds of staved bearing are investigated. The mechanism of action of the number of staves in staved bearings on the mixed-lubrication performance is revealed. The numerical results show that the number of staves has a significant effect on the mixed-lubrication performance in circular- and flat-staved bearings. Furthermore, there is an optimal value for the number of staves, shown to be 30 in the current simulation, for improving the mixed-lubrication performance of flat-staved stern tube bearings lubricated with water. All propeller-driven ships employ a drive shaft supported by journal bearings. To avoid water pollution, these bearings are generally lubricated by the surrounding water, removing the need for a rear seal. Such bearings, commonly referred to as Cutlass bearings, usually have an inner grooved nitrile rubber lining. The grooves (called flutes) allow debris to be flushed out and the bearing surface to be cooled. The remaining area is divided into a number of load-carrying areas called staves. At present, no rigorous design guide exists for these bearings. This paper presents a methodology to predict the minimum film thickness between the journal and the most heavily-loaded stave, an approach not hitherto reported in the literature. The method includes a new, 3D, finite element (FE) approach for soft elasto-hydrodynamic (EHL) predictive modelling of generated pressures in cutlass bearings. Model predictions compare favourably with experimental data. It is shown that the modulus of elasticity of the rubber has no influence on the minimum film thickness. An equation relating dimensionless film thickness to dimensionless load, clearance ratio and numbers of staves is presented. For a nominally circular bearing, increasing the clearance ratio or increasing the numbers of staves reduces load-carrying capacity. It is shown that distortion due to loading can increase load-carrying capacity. Full article
