(1) Background: osteopathic manipulation of the sphenopalatine ganglia (SPG) blocks the action of postganglionic sensory fibres. This neuromodulation can reduce nasal obstruction and enhance upper airway stability. We investigated the manipulation of the SPG in 31 patients with obstructive sleep apnoea syndrome (OSAS); (2) Methods: Randomised, controlled, double-blind, crossover study. Participants received active (AM), then sham manipulation (SM), or vice versa. The primary endpoint was apnoea-hypopnoea index (AHI). Secondary endpoints were variation of nasal obstruction evaluated by peak nasal inspiratory flow (PNIF) and upper airways stability evaluated by awake critical closing pressure [awake Pcrit]), at 30 min and 24 h. Schirmer’s test and pain were assessed immediately post-manipulation. Tactile/gustatory/olfactory/auditory/nociceptive/visual sensations were recorded. Adverse events were collected throughout. (3) Results: SPG manipulation did not reduce AHI (p = 0.670). PNIF increased post-AM but not post-SM at 30 min (AM-SM: 18 [10; 38] L/min, p = 0.0001) and 24 h (23 [10; 30] L/min, p = 0.001). There was no significant difference on awake Pcrit (AM-SM) at 30 min or 24 h). Sensations were more commonly reported post-AM (100% of patients) than post-SM (37%). Few adverse events and no serious adverse events were reported. (4) Conclusions: SPG manipulation is not supported as a treatment for OSAS but reduced nasal obstruction. This effect remains to be confirmed in a larger sample before using this approach to reduce nasal congestion in CPAP-treated patients or in mild OSAS. Full article

Exploring future physicians’ attitudes toward vaccination is crucial as physicians’ recommendation is the top predictor for individuals to receive vaccines. This study explored the uptake of COVID-19 vaccines and the intention for future booster dose uptake among students at the New York Institute of Technology College of Osteopathic Medicine (NYITCOM). Predictors for actual vaccine and intended booster uptake were also examined. An electronic survey was distributed to Osteopathic Medical Students (OMS I-IV) in the Spring of 2021. A total of 1331 students received the survey, with 316 responses received (24%). In total, 95.3% (301/316) of the respondents reported that they already received vaccines, while 3.1% (13/316) reported that they had not yet received a vaccine. Moreover, 88.9% of the respondents (281/316) were in favor of a booster dose, which was a strong predictor for actual vaccine uptake. We identified that the Asian race, pharmaceutical mistrust, building immunity via vaccines, adequate vaccine testing, and willingness to get non-U.S. manufactured vaccines are the most significant predictors for willingness to accept a booster dose. A very high COVID-19 vaccine uptake among NYITCOM OMS was found in our study. The study also observed a high acceptance of an additional dose of the COVID-19 vaccine in the future. Full article

Musculoskeletal rehabilitation has been receiving growing attention in the scientific research field taking into account the functional consequences of chronic pain that have been frequently underestimated and undertreated. The usual rehabilitative care of a wide range of diseases affecting physical function and independence in activities of daily living needs to be improved with novel approaches. In this scenario, the recent literature has highlighted the great advantages of multidisciplinary and comprehensive pain management. The Special Issue highlights the importance of advancements in musculoskeletal rehabilitation in terms of instrumental physical therapies, therapeutic exercise, osteopathic manual therapy, innovative approaches and the correlation with dentistry. Physicians should be aware of the presence of novel therapeutic approaches that are changing the clinical scenario of musculoskeletal rehabilitation. Full article

Fluoroquinolone medications have been shown to contribute to tendinopathies, cardiotoxicity, and neurotoxicity. Low back pain is a common musculoskeletal condition for which chiropractic treatment is most often sought. This case report details a patient presenting with low back pain and a history of fluoroquinolone toxicity. The patient was initially treated with chiropractic manipulation, which increased her symptoms. She was then referred to an osteopathic physician who treated the patient with intravenous antioxidants and amino acids, an elimination diet, and probiotic supplementation. Within 4 months of therapy, the patient reported a decrease in pain, a resolution of her dizziness, shortness of breath, panic attacks, tachycardia, and blurred vision. After an additional 8 weeks of antioxidant therapy, she reported further reductions in pain and improved disability. People susceptible to fluoroquinolone toxicity may present with common musculoskeletal symptoms. A past medical history and medication history may help to identify this population of patients. People presenting with fluoroquinolone toxicity may have unidentified contributing factors that predispose them to this anomaly. This patient reported improvements in pain and disability following antioxidant amino acid therapy for a total of 6 months. The natural history of fluoroquinolone toxicity is unknown and may account for the observed improvements. Full article
