Equine-Assisted Services (EAS), which incorporate horses in a variety of ways in an effort to improve human wellbeing, have grown in popularity in recent years. Although much research has been conducted regarding the benefits that horses may provide for human health and wellbeing, little attention has been paid to practitioners’ experiences and perspectives of the field, despite the fact they are uniquely positioned to advance our understanding of this area. This study aimed to explore practitioners’ lived experiences of EAS, focusing on the benefits they observed, possible underlying mechanisms for any health benefits witnessed, and challenges faced in the area. Fifteen EAS practitioners from the UK/Ireland took part in qualitative semi-structured interviews, analysed using reflexive thematic analysis. Five themes were identified, three of which related to the horse’s influence on building connections, relationships, and enriching the process, whilst the remainder explored challenges within the field of EAS. These themes were explored through the practitioners’ lens, where possible linking them to our current understanding of human–animal interactions and related fields in the literature. Findings showed that horses, through EAS, were considered invaluable for building relationships, relational skills, and motivation to engage in whichever service was being provided. However, EAS was also viewed as complex. Concerns regarding competencies to practice, training, and lack of governance were expressed. These areas need further exploration and progress if EAS is to grow in efficacy and attain professional status. Full article

Autistic children often have impaired self-regulation which can impact daily functioning and life outcomes. Occupational Therapy Using Zones of Regulation™ Concepts in an Equine Environment (OT-ZOR-Equine) is a standardized intervention that integrates both the Zones of Regulation™ curriculum and horses into occupational therapy to address self-regulation in autistic children. We aimed to assess the acceptability of OT-ZOR-Equine to participating autistic children. A qualitative descriptive research study was conducted with six autistic children ages 7–9 years who received ten weeks of OT-ZOR-Equine. Children participated in semi-structured interviews that used questions guided by the Theoretical Framework of Acceptability. We analyzed interview transcripts using directed content analysis. Autistic children found OT-ZOR-Equine to be generally acceptable. The children especially enjoyed horse riding but found the Zones of Regulation™ curriculum and certain aspects of riding to be less acceptable. Tailoring OT-ZOR-Equine to integrate each client’s preferences and needs may make this intervention more acceptable to autistic children. The findings of this study support the continued use of and research on OT-ZOR-Equine or similar occupational therapy interventions that integrate horses to influence self-regulation in autistic children. Equine-Assisted Services (EAS) offer significant benefits for individuals with intellectual and/or physical disabilities. However, ensuring the welfare of the horses involved remains a key ethical concern. The objective of this study was to assess the affective states of horses involved in EAS while also establishing behavioural thresholds for indicators of positive or negative affective states. A total of 98 horses were observed over a two-month period, accumulating data about horse behaviour and session characteristics from 830 EAS sessions. Horses participating in coaching programs averaged four sessions in two months; those involved in therapeutic riding averaged 31 sessions. Equine behaviour and field experts defined behavioural thresholds to assess welfare standards. Negative state behaviours were predominantly aligned within excellent or neutral thresholds, while positive state behaviours were observed from unacceptable to excellent. Principal Component Analysis (PCA) of the gathered behavioural data identified six affective states in coaching sessions and four in therapeutic riding, explaining 63% and 73% of the total variation, respectively. Among these states, “comfortable” was the most frequently observed. Multiple linear regression showed several significant variables, highlighting the importance of health, suitable horse selection, and proper management in promoting positive affective states in EAS. Behavioral Signature of Equine Gastric Discomfort? Preliminary Retrospective Clinical Observations

Gastric ulcer disease and other potentially painful gastric conditions are among the most common afflictions adversely affecting the welfare of domestic equids. A large percentage of affected animals may not display the classic signs of gastric disease, such as unexplained weight loss, poor hair coat, and inappetence until the disease becomes severe. As a clinical service within our equine referral hospital, we routinely evaluate 24-h video recorded samples of horses to assist clinicians in identifying subtle discomfort and potential sources or to scan for infrequent neurologic or cardiac-related behavioral events. Empirically, we have recognized discomfort behaviors that appear to be uniquely associated with gastric disease. These include frequent attention to the cranial abdomen (nuzzling, swatting, nipping, and/or caudal gaze focused on the abdomen caudal to the elbow) and/or deep abdominal stretching, often within the context of eating, drinking, and/or anticipating feeding. To systematically evaluate the reliability of these purported gastric discomfort behaviors, we reviewed 30 recent 24-h video behavior evaluation cases for which (1) the clinical video behavior evaluation had been carried out without knowledge of the history and presenting complaint and (2) direct gastric examination had confirmed gastric disease status at the time. Twenty-four of the thirty cases showed gastric discomfort behavior, and all twenty-four had either gastric ulcers (n = 21) and/or gastric impaction (n = 3). Of the six cases not showing gastric discomfort behaviors, four were free of gastric disease, while two had mild lesions. Comparing horses with and without gastric disease, gastric discomfort behaviors were reported in 24 of the 26 (92%) with gastric ulcers or gastric impaction, compared to none of the four gastric disease-free horses. Although a larger prospectively designed study is needed to confidently estimate the sensitivity and specificity or the associations of behavior with the type or severity of gastric disease, these results confirm our long-held clinical impression of a behavioral signature for gastric discomfort in the horse. Equine-assisted services (EAS) is a relatively new area of study within higher education requiring the development of a curriculum that best prepares the undergraduate student for the demands of the industry. Further, coursework targeting the specific management challenges for ensuring the welfare of horses utilized within EAS is limited. Therefore, the objective of this study was to apply an industry-based survey assessment utilizing EAS professionals within the United States to document management practices associated with healthcare and pain management challenges in the EAS horse. Suggested curriculum for undergraduate students targeting a career in EAS is given, utilizing data collected from the survey. Participant responses (n = 30) indicated that all of the surveyed facilities cared for more than three geriatric horses within their program, with the majority (60% responding “definitely yes”) utilizing regularly functionally lame horses, although the prevalence of lameness was not correlated with number of geriatric horses within a program (0.20, p = 0.22). While lameness is a common healthcare challenge within EAS, gastrointestinal conditions (73%) were one of the other leading health concerns for these facilities. The most common equine pain detection method for EAS facilities was behavior assessment (93%), in which this method was positively correlated with years of horse experience (0.51, p = 0.01). Horse pain was primarily managed through annual alternative therapeutic interventions (76%), daily prescriptions (73%), and nutritional supplements (73%). As such, the curriculum for undergraduate EAS degree programs should cover healthcare practices targeting geriatric horse management and associated health issues such as gastrointestinal conditions and lameness, including early detection and pharmaceutical, nutritional, and alternative therapeutic treatments. Equine Assisted Interventions (EAIs) integrate the active participation of horses in therapeutic or educational interventions. A bibliometric analysis was carried out on this topic, using traditional bibliometric laws and recommendations. For this purpose, a search on the Web of Science (WoS) Core Collection database was carried out, obtaining 333 documents. Annual publications followed an exponentially increasing trend (R2 = 86%), pointing out that this topic is a growing interest among researchers, publishers, and journals. The USA was the most productive country worldwide and Jeong-yi Kwon and Ji Lee were the prolific co-authors. The WoS category with the highest number of papers was Rehabilitation (84 papers). The Journal of Alternative and Complementary Medicine, and Pediatric Physical Therapy were the journals with the highest number of publications. The most cited paper was “State of the Evidence Traffic Lights 2019: Systematic Review of Interventions for Preventing and Treating Children with Cerebral Palsy”. The most used author keywords were rehabilitation, balance, and those related to specific populations such as Cerebral Palsy and Autism Spectrum Disorder. These results suggest that EAIs is a topic of increasing interest for researchers, editors, and professionals. Full article

Equine-assisted therapies are becoming increasingly popular for addressing physical and psychological disabilities in clients. The role of the horse’s welfare in equine-assisted service receives increasing attention in research. Several studies have shown that horses are able to perceive human emotions and respond to human stress responses. However, no research has yet looked at the other side of the coin—whether and how humans perceive and react to equine stress levels during equine-assisted services. To fill this gap in the research, we employed a within-subjects design, in which horse-naïve participants had a standardized interaction with both an experimentally stressed horse and an experimentally relaxed horse. We assessed physiological indicators of stress (heart rate, heart rate variability, and salivary cortisol) in participants and horses, as well as psychological indicators of stress (state anxiety and positive and negative affect) in participants. Although our stress and relaxation manipulations were successful (indicated by horses’ physiological indicators of stress), we did not find any difference in the participants’ physiological or psychological indicators of stress between the interaction with a stressed and the interaction with a relaxed horse. Together with results from previous studies, this suggests that humans cannot intuitively recognize the (physiological) stress level of horses, which has important implications for effective communication and bonding between humans and horses and for the safety of equine activities. Equine-Assisted Services (EASs) are commonplace in today’s society, but vary widely in both theoretical and practical applications. Until now, practitioners’ experiences and perspectives in relation to these services have received little attention. To address this, a purpose-designed online questionnaire was distributed to EAS practitioners, exploring issues relating to the nature of the service provided, practice patterns, practitioner education, perceived knowledge, challenges faced and the future direction of these services. An analysis revealed a significant association between practitioners’ backgrounds and the nature of the service offered, as well as perceived knowledge. Median EAS training received to first practice was 20 days of block release over a year; however, nearly half of the sample (42.4%) reported less training than this. Equine-specific training was more limited, with 41.5% of practitioners having no horse-relevant qualifications. The most important challenges reported by practitioners involved client and equine welfare, financial sustainability and raising awareness of EAS. This research highlights the diverse nature of EAS and also raises important challenges and possible opportunities for development. Findings suggest that more progress is needed to professionalise and legitimise the area to support and help practitioners provide the best service for all concerned. Full article

This study examines the effect of volunteering within a Professional Association of Therapeutic Horsemanship International (PATH Intl) premiere accredited center by exploring the experiences of volunteers leading horses in adaptive riding lessons. Adaptive Riding lessons are horseback riding lessons for individuals ages four through the lifespan, with special needs, varying from cognitive, physical, social-emotional, or other challenges. Volunteers directly impact the rider-horse bond by increasing accessibility to horseback riding for individuals with disabilities, fostering a meaningful bond between the rider and horse. The research questions were as follows: (1) do saliva measures of cortisol and alpha-amylase (stress), and oxytocin (affiliative bonding) change over time for volunteers; and (2) how satisfied are volunteers with volunteering for Equine Assisted Services (EAS)? Forty-one volunteers participated in Reining in Anxiety, an intervention combining adaptive riding and cognitive behavioral therapy. Physiological data (i.e., pooled saliva, saliva combined from various glands throughout the mouth, resting under the tongue prior to collection) were collected pre/post riding session at four time points during the 10-session intervention, measuring oxytocin, cortisol, and alpha-amylase. Post-intervention, volunteers completed a survey about their experiences as volunteers and as participants in the study. All saliva samples were collected successfully. There was a non-significant, positive trend in oxytocin and alpha-amylase, while cortisol remained level. The responses in the survey suggested that volunteers perceive their role positively, with nuanced experiences of a sense of responsibility to ensure safety, and enjoyment in assisting the riders. Volunteers are vital to the safety of the rider and horse. While their perceived and internalized responsibility is evidenced by an increase in stress (e.g., cortisol remaining level and an increase in alpha-amylase), it is not necessarily negative stress, as there is simultaneously affiliative bonding expressed (oxytocin). The complex emotions and experiences of volunteers are important to understand to create meaningful, sustainable volunteer engagement. This is particularly important in the EAS industry, which is reliant on volunteerism. Full article

Equine-assisted services (EASs) are being increasingly used as complementary interventions for military veterans who have experienced trauma. However, there is limited evidence of benefit for this population and almost no literature describing the desired potential outcomes and possible mechanisms of action. The aim of this article is to address these gaps by reviewing the extant literature of animal-assisted interventions in general, and equine-assisted services in particular, with the goal of providing guidance for future investigations in the field. Currently, the field is in the early stage of scientific development, but published results are promising. Interventions that enhance treatment compliance and/or outcomes could benefit this population. Preliminary results, reviewed herein, indicate that EAS interventions might benefit the military veteran population by enhancing treatment engagement and therapeutic alliance, as well as by contributing to symptom reduction and resulting in various transdiagnostic benefits. It is recommended that future studies include exploration of potential beneficial outcomes discussed herein, as well as investigate suggested mechanisms of action. Full article

A significant number of institutionalized children have behavior problems. Socio-emotional skills are fundamental for their adaptation and success throughout life and are usually weakened in this population. Equine-assisted services (EAS) are a form of therapeutic mediation that facilitates and requires the practitioner’s participation, contributing to the promotion of various psychomotor and socio-emotional dimensions. This study was developed during 17 sessions of EAS with a psychomotor intervention, which took place individually and weekly and lasted approximately 45 min, with three institutionalized children. A quantitative and qualitative assessment was carried out before and after the intervention to study the effects of an EAS intervention on socio-emotional competencies in the three institutionalized children. There was an improvement in skills, with an impact on intrapersonal skills and marked improvement in self-regulation and self-control, in addition to an improvement in the intentionality of movement and adequacy of gesture to the context. This type of intervention underlies a renewed educational and therapeutic approach, contributing to mental health promotion in this population. Full article

Reining in Anxiety (RiA) is a therapeutic program for youth with mild to moderate anxiety delivered in a therapeutic riding setting by Certified Therapeutic Riding Instructors. RiA was developed after a review of the evidence base for youth anxiety, is manualized, and includes five core CBT components: in vivo exposure, cognitive restructuring, youth psychoeducation, relaxation, and caregiver psychoeducation about anxiety. This study extended findings from a prior RCT that examined (1) the feasibility of collecting saliva samples from horses and children to measure stress (cortisol) and relaxation (oxytocin); (2) whether changes in stress and relaxation occurred both during each lesson and over the course of the 10-week intervention for horses and youth; (3) whether changes in anxiety symptoms, emotional regulation, and self-efficacy found in the first trial were comparable; and (4) if fidelity to the program was reliable. Youth participants (n = 39) ages 6–17 with caregiver-identified mild-to-moderate anxiety participated in a ten-week therapeutic intervention (RiA), which combined adaptive riding and components of CBT. Physiological data and self-report measures were taken at weeks one, four, seven, and ten for the youth and horses. Saliva assays assessed cortisol as a physiological marker of stress and anxiety, and oxytocin as a measure of relaxation. Fidelity data were recorded per session. Anxiety, as measured by caregiver self-reporting, significantly decreased from pre- to post-test, while emotional regulation scores increased. No significant changes in self-efficacy from pre- to post-test were observed. Saliva samples obtained from participants before and after riding sessions showed a consistent decrease in cortisol and a significant increase in oxytocin at two of the four timepoints (Week 1 and Week 7), but no overall pre- to post-test changes. Horse saliva data were collected using a modified bit; there were no significant changes in oxytocin or cortisol, suggesting that the horses did not have an increase in stress from the intervention. RiA may be a promising approach for reducing anxiety and stress among youth, as measured both by self-reported and by physiological measures. Collection of salivary assays for both youth and horses is feasible, and the intervention does not increase stress in the horses. Importantly, RiA can be delivered by adaptive/therapeutic horseback riding instructors in naturalistic (e.g., non-clinic-based) settings. As youth anxiety is a growing public health problem, novel interventions, such as RiA, that can be delivered naturalistically may have the potential to reach more youth and thus improve their quality of life. Further research is needed to examine the comparative value of RiA with other animal-assisted interventions and to assess its cost-effectiveness. Full article

(1) Background: Accounting for the well-being of equine partners is a responsibility of those engaged in Equine-Assisted Services (EAS). Researchers took heed of this call to action by developing an innovative way to collect data to assess the physiological indicators of stress in equine participants. The collection of saliva is considered to be a minimally invasive method of data collection and is typically performed using a cotton swab; however, in equines, the introduction of a foreign object may induce stress; (2) Methods: Researchers used a modified bit to collect pooled saliva in an effort to further reduce stress during the saliva collection process. Additionally, the collection of pooled saliva, via the bit, increases the opportunity to consider additional analyses, such as oxytocin, which is more reliable in pooled saliva than site-specific saliva captured with a swab; (3) Results: A data analysis demonstrated that ample saliva was captured using the modified bit. Observational data supported that the horses demonstrated fewer physical stress signals to the bit than to the swab. Thus, the modified bit is a feasible and valid method for equine salivary sample collection; (4) Conclusions: The results suggest that the modified bit provides a viable method to collect equine saliva and supports national calls to prioritize animal welfare analysis, specifically for horses used within EAS. Future research should enhance methodological rigor, including in the process and timing, thereby contributing to the bit’s validation. Full article

The foundation of healthy workplace design is an understanding of work practices. Volunteers comprise the majority of the workforce in care centers using horses to address human health issues. Documentation is lacking on protections for worker well-being in equestrian microenvironments which are known to have the potential for dust exposures. Climate acts as a master variable in equestrian facility design and ventilation usage to address dust and temperature concerns. Using climate as an independent variable, our objective was to characterize space usage, safety, environmental control, and organizational practices through a national survey of equine assisted programs. We found that more fully enclosed indoor arena spaces were in cold/very cold and mixed-humid climates (p = 0.0114). Annually more volunteers (p = 0.0073) work in these two climate groups averaging 100 volunteers per location. A total of 34% of all facilities, regardless of climate, do not use mechanical ventilation systems (e.g., fans). As volunteer worker time in the arena increased, time in the barn microenvironment tended to decrease (p = 0.0538). We identified facility designs, ventilation usage, and worker arrangements to refine the scalability of future air contaminant monitoring and to provide frameworks for education, workplace design, and prevention of exposure to dust. While human benefits of animal-assisted therapy programs have been documented, relatively little research has been conducted on behavioral factors that predict a successful equine-assisted services (EAS) horse. This study compares the behavior of experienced and non-experienced EAS horses as well as horses selected for future EAS work in a series of sociability and temperament tests. No significant differences were found between experienced and non-experienced horses in the sociability measures or for most of the temperament tests; however, significant differences were found between groups in the brushing test, with non-experienced horses showing more affiliative behaviors towards the familiar handler and unfamiliar persons. No significant differences were found between selected and non-selected horses in the temperament tests. However, non-selected horses were found to show significantly more affiliative behaviors towards a familiar person during a sociability test compared with selected horses. These findings suggest that the social behavior and temperament of EAS horses may not be significantly different from other available horses not selected for EAS work. Instead, these decisions may primarily reflect subjective impressions of fit. Interestingly, on measures where significant differences were identified, the horses not actively engaged in or selected for therapy were the ones that showed greater affiliative responses to familiar and unfamiliar humans.
