Abstract

Perioperative management of blood coagulation: “old” and “new” anticoagulants. The perioperative management of patients receiving “old” and “new” oral anticoagulants is challenging and requires an interdisciplinary approach. Surgeons, anaesthetists, haematologists and cardiologists should elaborate guidelines that are mandatory for their institution. The risk and harmfulness of bleeding, the thromboembolic risk, the urgency of the procedure, and special circumstances (e.g., pregnancy, history of heparin- induced thrombocytopenia) have to be taken into consideration. Based on two case scenarios, the management of patients under treatment with direct oral anticoagulants and with vitamin K antagonists is discussed.
