Ultrasonographic Diagnosis and Computed Tomographic Confirmation of a Scapular Body Stress Fracture in an Elite Boxer: A Case Report

Background and Clinical Significance: Scapular stress fractures are exceptionally rare in athletes and are notoriously difficult to diagnose due to their subtle presentation and poor sensitivity on initial radiographs. This case report describes the diagnostic challenge of a scapular body stress fracture in an elite boxer who initially presented with wrist pain. Case Presentation: A 19-year-old right-hand-dominant female elite boxer presented with a three-month history of bilateral wrist pain. Initial examination and MRI were consistent with a triangular fibrocartilage complex (TFCC) injury. Despite conservative management, her symptoms persisted, and she subsequently developed mechanical right shoulder pain and a sensation of instability. Physical examination revealed scapular dyskinesis, with a positive push-up test and weakness on punch protraction. Plain radiographs of the scapula were unremarkable. Point-of-care musculoskeletal ultrasound (MSK US) identified a cortical irregularity at the medial scapular border. A subsequent computed tomography (CT) scan obtained at three-month follow-up definitively confirmed a stress fracture at this site. Treatment focused on scapular stabilization via prolotherapy and activity modification, leading to symptomatic resolution and a successful return to sport. Conclusions: This case underscores the importance of evaluating the entire kinetic chain in athletes presenting with focal complaints. It demonstrates the utility of MSK US as an effective initial screening tool for cortical stress fractures and highlights the necessity of CT for definitive confirmation. Clinicians should maintain a high index of suspicion for scapular stress injuries in overhead athletes with unexplained shoulder dysfunction. Full article

Comparing Single and Dual Plating in Displaced Scapular Body Fractures: A Retrospective Study of Clinical and Functional Outcomes

Background: Scapular body fractures, when significantly displaced or malunited, can cause glenohumeral discomfort and functional disability. This study compares single- and dual-plating techniques in terms of pain, function, and active range of motion (aROM) in patients with scapular body fractures. Methods: Twenty-eight patients with scapular fractures were retrospectively analyzed, with sixteen undergoing single plating treatment and twelve dual plating treatment. The mean age was 44.9 years, and the mean follow-up was 14 months for single plating and 13.8 months for dual plating. Outcomes included Disabilities of the Arm, Shoulder and Hand (DASH) scores, the Visual Analog Scale (VAS) for pain, aROM measurements, and the time to return to work. Functional outcomes were assessed using two-way ANOVA with Šidák’s multiple comparisons test at 2 weeks, 4 weeks, 3 months, 6 months, and 1 year. The time to return to work was analyzed with survival analysis and a log-rank test. Results: The single plating group had higher DASH scores than the dual plating group at 2 weeks (44.88 ± 10.81 vs. 32.75 ± 6.05, p = 0.005), 4 weeks (28.50 ± 5.91 vs. 22.83 ± 4.24, p = 0.033), and 3 months (19.63 ± 2.45 vs. 16.00 ± 2.45, p = 0.004), indicating greater disability. VAS scores were also higher in the single plating group at 2 weeks (4.00 ± 1.21 vs. 2.33 ± 0.88, p = 0.002) and 4 weeks (2.50 ± 1.03 vs. 1.17 ± 0.94, p = 0.008), suggesting faster pain relief in the dual plating group. However, differences were no longer significant after 3 months. At 1 year, the dual plating group demonstrated better external rotation (73 ± 3° vs. 63 ± 5°, p = 0.032), with no significant differences in internal rotation, abduction, or forward flexion. Dual plating patients returned to work earlier (Hazard Ratio = 3.346, 95% CI: 1.208 to 9.269, p = 0.020). Conclusions: In the current cohort, dual plating for scapular fractures offers superior early pain relief and functional outcomes compared to single plating, along with better external rotation at 1 year and an earlier return to work. These findings suggest that dual plating may facilitate faster recovery and enhanced active range of motion in selected patients, a hypothesis that warrants further investigation through future randomized trials. Background: Scapula body fractures are rare injuries with an incidence of 1% of all fractures accounting for 3% to 5% of all upper extremity fractures. Fractures of the scapula commonly result from high-energetic trauma and fall from great height. While several studies focused on concomitant injuries of chest and head as well as the cervical spine, up to now in the common literature, no study exists analyzing the prevalence of concomitant intra-articular glenohumeral injury following extra-articular scapular fracture. Objectives: The aim of this study was to analyze the prevalence of concomitant intra-articular glenohumeral injuries in acute fractures of the scapula by performing magnetic resonance imaging (MRI) of the shoulder joint. Study Design and Methods: This prospective cohort study was performed at our academic Level I trauma center from November 2014 to October 2016. According to our clinical algorithm, all patients suffering from an acute scapula body fracture primarily underwent computed tomography (CT) for assigning the fracture according to the Orthopedic Trauma Association (OTA)-classification and therapy planning. In addition, 3 T MRI-scans of all patients were performed within seven days after trauma. Results: Twenty-one (16 male/5 female, mean age 53 years (25–83 y) patients with scapula body fractures (OTA 14.A3.2 80.1%, OTA 14.A3.1 4.8%, OTA14.B3.1 4.8%, OTA14.C3 9.5%) were enrolled. MRI revealed 11 acute intra-articular injuries in 8 of 21 patients (38%). In all 21 patients, hematoma of the rotator cuff and periarticular muscles was present. Three patients (14.3%) presented a partial bursa sided tear of the supraspinatus tendon, whereas in 5 (23.8%), a partial articular sided supraspinatus tendon tear and in 2 (9.5%) patients, a subtotal tear was observed. One patient (4.8%) showed a complete transmural supraspinatus tendon tear. Conclusions: Traumatic concomitant glenohumeral injuries in scapula body fractures seem to be more frequent than generally expected. Subsequent surgical treatment of these formerly missed but therapy-relevant injuries may increase functional outcome and reduce the postoperative complication rate following scapula body fractures. The shoulder is the most mobile joint of the human body, but it is very fragile; several pathologies, and especially muscular degenerations in the elderly, can affect its stability. These are more commonly called rotator cuff fractures. In the case of this type of pathology, the mobility of the shoulder decreases and pain appears. In order to restore mobility and reduce pain, implantation of an inverted shoulder prosthesis is recommended. Unfortunately, over time a notch phenomenon has been observed. In the lower position of the arm, part of the implant comes into contact with the scapula and therefore causes deterioration of the bone. Among the solutions adopted is the lateralized method with bone grafting. However, a main disadvantage of this method concerns the reconstruction of the graft in the case of prosthesis revision. In this context, the aim of the present work was to reconstruct the shoulder joint in 3D in order to obtain a bio-faithful geometry, and then study the behavior of different types of biomaterials that can replace bone grafting. To this end, three arm abduction motions were examined for three individuals. From the results obtained, it appears that grafts in ultra-high molecular weight polyethylene (UHMWPE) exhibit a behavior closer to that of bones. Full article
