Lightning strikes pose a significant risk during outdoor activities. The connection between conventionally used rescue blankets in alpine emergencies and the risk of lightning injury is unclear. This experimental study investigated whether rescue blankets made of aluminum-coated polyethylene terephthalate increase the likelihood of lightning injuries. High-voltage experiments of up to 2.5 MV were conducted in a controlled laboratory setting, exposing manikins to realistic lightning discharges. In a balanced test environment, two conventionally used brands were investigated. Upward leaders frequently formed on the edges along the fold lines of the foils and were significantly longer in crumpled rescue blankets (p = 0.004). When a lightning strike occurred, the thin metallic layer evaporated at the contact point without igniting the blanket or damaging the underlying plastic film. The blankets diverted surface currents and prevented current flow to the manikins, indicating potentially protective effects. The findings of this experimental study suggest that upward leaders rise from the edge areas of rescue blankets, although there is no increased risk for a direct strike. Rescue blankets may even provide partial protection against exposure to electrical charges. Full article

High-voltage electrical injuries, especially from lightning strikes, can cause life-threatening complications due to extreme temperature and voltage exposure. While burns and cardiac complications have been widely described, the documentation of metabolic imbalances, particularly hypokalemia, has not been as prevalent. This report focuses on a patient with profound transient hypokalemia following a lightning strike, alongside a review of three similar cases of transient hypokalemia from the literature. Our patient, a previously healthy young man, was struck by lightning and subsequently suffered transient hypokalemia with lower extremity sensory changes, which resolved after the normalization of serum potassium levels. While the exact underlying mechanisms of transient hypokalemia following high-voltage electrical injuries are unknown, we propose a multifactorial mechanism, which includes massive intracellular shifts of potassium due to elevated epinephrine levels and the prevention of potassium efflux through the electrical disruption of voltage-gated potassium channels. Our report underscores the importance of recognizing hypokalemia in patients with high-voltage electrical injuries and contributes to the understanding of the complex mechanisms involved. Further research is necessary to understand the connection between cellular changes induced by high-voltage exposure and their effects on metabolism, particularly in relation to hypokalemia. Bangladesh has seen a significant number of fatalities and injuries related to lightning in the past few years, which indicates that lightning has become a deadly hazard. This cross-sectional study aims to determine university students’ self-rated status about lightning. Additionally, it evaluates these students’ views toward lightning through knowledge, attitude, and practices (KAP). A total of 1274 university students participated in an online KAP survey. Where appropriate, the Kruskal–Wallis or Mann–Whitney U test, Spearman’s rank correlation, and logistic regression models were performed. About 90% of university students perceive lightning as a dangerous event, and 38% rated their places unsafe. More than half of the survey population reported frequent lightning; most (84%) did not have lightning safety precautions, and a small portion (26%) received warning messages. Individuals encountering frequent lightning consider lightning-prone areas much more dangerous compared to the individuals encountering occasional lightning. Students living in tin sheds assessed lightning as a dangerous event (4.78 ± 0.53) and having unsafe surroundings (2.44 ± 0.98). Many individuals have enough knowledge (63%), developed positive attitudes (93%), and effective preventative practices (77%). The logistic regression analysis indicated that having adequate information and a good attitude can assist individuals in practicing lightning safety; also, student’s Gender, living with family, residential unit, university type, study year, major field, and having lightning-related subjects in university curricula as significant predictors. Females demonstrated better lightning practice than males. Additionally, lightning-related courses in university curricula are critical for educating students about lightning. Behavioral improvements among these students will require substantial lightning campaign actions coupled with effective education. Full article

Background: Lightning strikes are a risk during mountain-sport activities. Yet little is known about the prevalence of injuries related to lightning strikes during mountain hiking, backcountry skiing, or high-altitude mountaineering. This study therefore examined the occurrence and characteristics of lightning-strike-related emergencies during mountain-sport activities in the Swiss Alps. Methods: We analyzed 11,221 alpine emergencies during mountain hiking, 4687 during high-altitude mountaineering, and 3044 during backcountry skiing in the observational period from 2009 to 2020. Identified cases were analyzed in detail regarding age, sex, the time of occurrence, altitude, location, the severity of the injury as quantified by its NACA Score (National Advisory Committee for Aeronautics Score), and injury pattern. Results: We found no cases related to backcountry skiing. Eight cases of lightning strikes during mountain hiking (four female and four male) were identified. The mean age was 32.5 ± 17.5 years, the mean NACA Score was 2.5 ± 1.9, and the mean altitude was 1883.8 ± 425.7 m. None of these cases were fatal, and only one victim was seriously injured. Fifteen cases were identified during high-altitude mountaineering (four female and 11 male). The mean age was 38.7 ± 5.2 years, the mean NACA Score was 3.1 ± 2.5, and the mean altitude was 3486.4 ± 614.3 m. Two lightning strikes were fatal. In these two cases, rope partners were injured by a lightning strike (NACA Score = 4). Most cases were on relatively exposed terrain, such as the Matterhorn Hörnligrat or the Eiger Mittellegigrat. Discussion: The typical victims were 30–40-year-old men. It is possible that the lightning strikes are a consequence of a lower risk aversion among these alpinists, which is be supported by the fact that most of the events occurred on famous mountains such as the Matterhorn or Eiger. Furthermore, since most of the locations were on relatively exposed terrain where one could not quickly find shelter, we recommend careful tour planning with serious consultation of the weather forecast and the likelihood of thunderstorms before climbing exposed sections to prevent emergencies related to lightning strikes. Despite the significant number of fatalities and injuries, there is currently a lack of data on public knowledge, attitudes, and practices regarding lightning in Bangladesh. This study aims to assess the public’s knowledge, attitude, and practices (KAP) towards lightning in the country. A total of 1641 individuals participated in an online KAP survey. Where appropriate, the Kruskal–Wallis or Mann–Whitney U test, Spearman’s rank correlation, and logistic regression models were performed. About 65% of the survey population reported frequent lightning; most (72.64%) did not receive any warning messages, and small proportions (22.12%) had lightning safety precautions. Individuals with numerous lightning experiences perceived considerably unsafe places against lightning compared to infrequent lightning experiences (p < 0.05). Respondents reported good knowledge (56.06%), positive attitudes (82.27%), and good preventive practices (72.33%). The logistic regression demonstrated that having good knowledge and positive attitudes can help people practice lightning safety. Females demonstrated better lightning attitudes and practices compared to males. In comparison to urban residents, rural residents had poor practices. In addition, individuals’ educational levels might also play a crucial role in preparing them for lightning. Overall, extensive lightning campaign activities combined with effective education are required for the behavioral changes in this lightning-vulnerable society. Full article

Electrical trauma can be caused by low-voltage current (from 60 to 1000 V, usually 220 or 360 V), high-voltage (more than 1000 V) current, lightning, and voltaic arc. Often victims are little children, teenagers, and working-age adults. Electrical injuries and clinical manifestations can vary a lot and range from mild complaints undemanding serious medical help to life-threatening conditions. Lightning causes serious injuries in 1000–1500 individuals every year worldwide. The case fatality rate is about 20–30%, with as many as 74% of survivors experiencing permanent injury and sequela. The primary cause of death in victims of lightning strike or other electrical trauma is cardiac or respiratory arrest. That is why appropriate urgent help is essential. Subsequently electrical burns, deep-tissue and organ damage caused by electricity, secondary systemic disorders often demand intensive care and prompt, usually later multistage surgical treatment; therefore, prevention of electrical trauma, which would help to reduce electrical injuries in children and working-age population, is very actual. The most important is to understand the possible danger of electricity and to avoid it. Full article
