When Research Evidence and Healthcare Policy Collide: Synergising Results and Policy into BRIGHTLIGHT Guidance to Improve Coordinated Care for Adolescents and Young Adults with Cancer

Background/Objectives: BRIGHTLIGHT was the national evaluation of adolescent and young adult (AYA) cancer services in England. BRIGHTLIGHT results were not available when the most recent healthcare policy (NHSE service specifications for AYA Cancer) for AYA was drafted and therefore did not consider BRIGHTLIGHT findings and recommendations. We describe the co-development and delivery of a Policy Lab to expedite the implementation of the new service specification in the context of BRIGHTLIGHT results, examining the roles of multi-stakeholders to ensure service delivery is optimised to benefit AYA patients. We address the key question, “What is the roadmap for empowering different stakeholders to shape how the AYA service specifications are implemented?”. Methods: A 1-day face-to-face policy lab was facilitated, utilising a unique, user-centric engagement approach by bringing diverse AYA stakeholders together to co-design strategies to translate BRIGHTLIGHT evidence into policy and impact. This was accompanied by an online workshop and prioritisation survey, individual interviews, and an AYA patient workshop. Workshop outputs were analysed thematically and survey data quantitatively. Results: Eighteen professionals and five AYAs attended the face-to-face Policy Lab, 16 surveys were completed, 13 attended the online workshop, three professionals were interviewed, and three AYAs attended the patient workshop. The Policy Lab generated eight national and six local recommendations, which were prioritised into three national priorities: 1. Launching the service specification supported by compelling communication; 2. Harnessing the ideas of young people; and 3. Evaluation of AYA patient outcomes/experiences and establishing a national dashboard of AYA cancer network performance. An animation was created by AYAs to inform local hospitals what matters to them most in the service specification. Conclusions: Policy and research evidence are not always aligned, so when emerging evidence does not support current guidance, further exploration is required. We have shown through multi-stakeholder involvement including young people that it was possible to gain a different interpretation based on current knowledge and context. This additional insight enabled practical recommendations to be identified to support the implementation of the service specification. Full article

Longitudinal Cohort Study of the Relationship Between Illness Perception, Perceived Social Support, and Psychosocial Quality of Life in Adolescents and Young Adults Newly Diagnosed with Cancer: Outcomes from a BRIGHTLIGHT Study

Background/objectives: Social support can enhance psychosocial health-related quality of life (PSQOL) in adult cancer patients. Adolescents and young adults (AYAs) with cancer face unique psychosocial challenges that intersect with key developmental milestones. Theoretical models propose that illness perceptions and social support are key determinants of coping strategies and long-term health outcomes in this context. These may be especially salient for AYAs, for whom peer relationships and identity formation are central. Methods: We explored how perceived social support and illness perceptions influence PSQOL over time in AYA cancer patients through a secondary analysis of the BRIGHTLIGHT longitudinal cohort study. Results: BRIGHTLIGHT followed 830 young people aged 13–24 across five time points (6–36 months post-diagnosis). Multi-level modelling revealed that PSQOL improved over time but remained consistently lower in females (mean: 69.62, 95% CI: 70.69 to −68.55). Greater perceived support from friends was associated with poorer PSQOL (β: −0.77, 95% CI: −1.007 to −0.54) and linked to negative illness perceptions, longer hospital stays (β: 0.01, 95% CI: 0.00 to −0.02), longer diagnostic intervals (β: −0.009, 95% CI: −0.02 to −0.00), and poorer clinical communication (β: 0.52, 95% CI: 0.01 to −1.03). A patient interpretation exercise with BRIGHTLIGHT’s Young Advisory Panel contextualized these findings. Conclusions: While peer support could promote normalcy, it could also intensify distress through emotional pressure or social isolation. Future research should address not only access to social support but its quality and relevance to AYAs’ unique psychosocial needs. Full article

Background/Objectives: AYAs face unique psychosocial challenges compared to younger children and older adults. Little published data exists about symptoms of psychological distress in AYAs, particularly in large cohorts over time. This study aimed to (1) explore relationships between distress and variables known to impact mental health, (2) examine distress over time, and (3) consult with a patient and public involvement group (Young Advisory Panel, YAP) to contextualize the results. Methods: Secondary data analyses were conducted from a multicentre longitudinal cohort study, which recruited 830 participants aged 13–24 years newly diagnosed with different cancers. The mean age was 20.1 (SD 3.3) and 45% were female. Bespoke surveys, including a validated measure of psychosocial distress, were administered five times over 3 years following diagnosis. Participatory workshops were conducted with the YAP to gather their impressions of the study results and inform conclusions. Results: At the first time point, higher social support was associated with higher distress scores (β = 0.202, p < 0.001) and AYAs with higher depressive symptoms were more likely to see a mental health professional (p < 0.01). Over time, anxiety symptoms remained stable (p = 0.090); however, depressive symptoms improved (p < 0.001). The YAP highlighted the mixed effects of social support and the under-recognition and undertreatment of distress. Conclusions: This information identifies key targets, such as social support, that can inform the design of personalized screening and intervention tools. It also highlights specific timepoints at which these efforts can be directed to mitigate distress and improve quality of life. Future research should include longer-term follow-up, qualitative explorations of distress along the disease trajectory, and ongoing co-production efforts. Full article

In order to effectively solve the problems of the complex growth state of dragon fruit and how the picking process is mostly manual, this study designed a picking and selecting integrated remote-operation-type dragon-fruit-picking device. Based on SOLIDWORKS 2020 software for the three-dimensional digital design and overall assembly of key components, the structure and working theory of the machine are introduced. By improving the high-recognition-rate dragon fruit target detection algorithm based on YOLOv5, better recognition and locating effects were achieved for targets with a small size and high density, as well as those in bright-light scenes. Serial communication, information acquisition, and the precise control of each picking action were realized by building the software and hardware platforms of the picking device control system. By analyzing the working principle of the mechanical system and the mechanics of the machine picking process, the critical factors affecting the net picking rate and damage rate of the dragon-fruit-picking device were confirmed. Based on the force and parameter analysis of the test results, it was confirmed that the machine had an optimal picking influence when the flexible claw closing speed was 0.029 m/s, the electric cylinder extending speed was 0.085 m/s, and the mechanical arm moving speed was 0.15 m/s. The net picking rate of the device reached 90.5%, and the damage rate reached 2.9%. The picking device can complete the picking of a single dragon fruit, as well as a plurality of fruits grown at a growing point, and integrates the integration of picking fruits, removing bad fruits, and sorting fruits, which can improve the efficiency of dragon fruit harvesting and replace manual work. True randomness is necessary for the security of any cryptographic protocol, including quantum key distribution (QKD). In QKD transceivers, randomness is supplied by one or more local, private entropy sources of quantum origin which can be either passive (e.g., a beam splitter) or active (e.g., an electronic quantum random number generator). In order to better understand the role of randomness in QKD, I revisit the well-known “detector blinding” attack on the BB84 QKD protocol, which utilizes strong light to achieve undetectable and complete recovery of the secret key. I present two findings. First, I show that the detector-blinding attack was in fact an attack on the receiver’s local entropy source. Second, based on this insight, I propose a modified receiver station and a statistical criterion which together enable the robust detection of any bright-light attack and thus restore security.
