Whilst this paper remains a difficult one for the reader to encompass at first read, the authors have tackled a difficult task in two somewhat different regional Wisconsin communities. The authors have shown persistence in pursuing several drafts with the journal's editors and have conscientiously addressed the comments and suggestions made by this referee on the previous draft and have better defined the limitations and gaps in the data collection.
