Dear Colleagues,

The present era in endoscopy is perhaps the most exciting, ushering in a wave of diagnostic and therapeutic breakthroughs. Techniques such as chromoendoscopy and endoscopic ultrasound (EUS) for characterizing submucosal neoplastic lesions, and endoscopic submucosal dissection and full-thickness resection for removing them, have revolutionized the management of malignant and pre-malignant lesions, often omitting the need for a more invasive surgical resection. Therapeutic EUS has facilitated the endoscopic management of luminal and pancreatobiliary anastomoses, the intraluminal drainage of peritoneal and retroperitoneal collections and abscesses, the endoscopic ablation of extraluminal neoplastic lesions, and endohepatology and diagnostics, as well as therapeutic vascular interventions. Endoscopic moyotomy has become a critical tool in the treatment of a wide variety of gastrointestinal motility disorders such as achalasia, gastroparesis, and even Hirschprung disease. Finally, the field of endobariatrics has experienced an amazing growth over the past 10 years, establishing endobariatric treatment as a primary treatment for obesity.

Many of these advances have ocurred with relatively small modifications to the endoscopic tools themselves, using creative problem-solving and enabling the off-label use of available equipment, although few key technologies such as high-resolution optic and ultrasound image processing, endoscopic full-thickness suturing, and lumen-apposing-metal stents have been instrumental. Currently, our field enthusiastically awaits the developement of robotic endoscopes, as well as the increase in the use of artifical intelligence in endoscopy.

This Special Issue of Journal of Clinical Medicine serves as a conduit for understanding the impact of these advancements and many others on patient care. We have collated a high-quality collection of research from researchers exploring advances in endoscopy, and we are excited to share these findings with our readers.
