Special Issue Information

Dear Colleagues,

The ankle joint complex, with its bony, muscular and capsule-ligamentous structures, is frequently affected by acute traumatic events in the general population and especially in the sports population; in some sports, such as basketball and volleyball, ankle injuries account for around 30% of all injuries.

Through clinical experience and thanks to the ever-increasing number of publications on the subject, significant progress has been made in diagnostic and instrumental classification, in conservative and surgical treatment in acute and in the management of sensory–motor rehabilitation.

Despite this, swelling, instability with a feeling of weakness and pain during walking and sporting activities persist in many patients long after the initial occurrence of trauma, negatively impacting quality of life and socio-economic loss in a working-age population.

We invite authors active in ankle pathology research to contribute reviews or original articles on new developments, emerging ideas, and mid- to long-term clinical and functional results in the diagnosis, treatment, and prevention of bone or soft tissue trauma of the ankle joint complex.

Background and Objectives: The treatment of end-stage ankle osteoarthritis (OA) and associated hindfoot deformities remains a major challenge for orthopedic surgeons. Numerous techniques and surgical approaches have been proposed for tibiotalar (TT) and tibiotalocalcaneal (TTC) arthrodesis, from arthroscopic to open, as well as numerous devices proposed for internal fixation (retrograde intramedullary nails, cannulated screws, and plating systems). The aim of this study was to retrospectively analyze the results, with at least 18 months of follow-up, with SilverbackTM TT/TTC Plating System Paragon28 in a group of 20 patients with severe OA and hindfoot deformities (mainly secondary post-traumatic OA). Materials and Methods: The demographic characteristics and past medical history of the patients were collected and analyzed to identify the cause of the pathology. The degree of OA and deformity were quantified based on foot and ankle weight-bearing radiography and CT examination. Pre- and post-operative clinical and functional scores (ROM, VAS, AOFAS, FFI, and SF-36) and radiographic parameters (anterior distal tibial angle, tibiotalar angle, coronal tibiotalar angle, and hindfoot alignment angle) were evaluated. Results: All of the patients showed clinical and radiographic fusion at an average of 14 weeks (range 12–48), with improvement in pain and functional scores, without major surgical complications and/or infections. Conclusions: Despite the limitations of our study, the results with this new plating system showed good results in terms of bone consolidation, post-operative complications, and improvement of pain and quality of life in patients with severe OA and deformities of the ankle and hindfoot. Background and Objectives: Ankle injuries are the most common type of injury in healthy active individuals. If not treated properly, recurrent sprains can lead to a condition of chronic ankle instability (CAI). The aim of the present review is to evaluate the effects of Kinesio Taping (or KT) on sports performances and ankle functions in athletes with CAI. Materials and Methods: This systematic review with meta-analysis was carried out following the criteria of the Prisma Statement system (registered on Open Science Framework, number: 10.17605/OSF.IO/D8QN5). For the selection of the studies, PubMed, Scopus and Web of Science were used as databases in which the following string was used: (“kinesiology tape” OR “tape” OR “taping” OR “elastic taping” OR “kinesio taping” OR “neuro taping”) AND (unstable OR instability) AND (ankle OR (ankle OR “ankle sprain” OR “injured ankle” OR “ankle injury”)). The Downs and Black Scale was used for the quality analysis. The outcomes considered were gait functions, ROM, muscle activation, postural sway, dynamic balance, lateral landing from a monopodalic drop and agility. Effect sizes (ESs) were synthesised as standardized mean differences between the control and intervention groups. Calculation of the 95% confidence interval (CI) for each ES was conducted according to Hedges and Olkin. Results: In total, 1448 articles were identified and 8 studies were included, with a total of 270 athletes. The application of the tape had a significant effect size on gait functions, ROM, muscle activation and postural sway. Conclusions: The meta-analysis showed a significant improvement in gait functions (step velocity, step and stride length and reduction in the base of support in dynamics), reduction in the joint ROM in inversion and eversion, decrease in the muscle activation of the long peroneus and decrease in the postural sway in movement in the mid-lateral direction. It is possible to conclude that KT provides a moderate stabilising effect on the ankles of the athletes of most popular contact sports with CAI. Full article

Background and Objectives: Foot deformities are the basis of numerous disorders of the locomotor system. An optimized method of classification of foot deformities would enable an objective identification of the type of deformity since the current assessment methods do not show an optimal level of objectivity and reliability. The acquired results would enable an individual approach to the treatment of patients with foot deformities. Thus, the goal of this research study was the development of a new, objective model for recognizing and classifying foot deformities with the application of machine learning, by labeling baropodometric analysis data using computer vision methods. Materials and Methods: In this work, data from 91 students of the Faculty of Medicine and the Faculty of Sports and Physical Education, University of Novi Sad were used. Measurements were determined by using a baropodometric platform, and the labelling process was carried out in the Python programming language, using functions from the OpenCV library. Segmentation techniques, geometric transformations, contour detection and morphological image processing were performed on the images, in order to calculate the arch index, a parameter that gives information about the type of the foot deformity. Discussion: The foot over which the entire labeling method was applied had an arch index value of 0.27, which indicates the accuracy of the method and is in accordance with the literature. On the other hand, the method presented in our study needs further improvement and optimization, since the results of the segmentation techniques can vary when the images are not consistent. Conclusions: The labeling method presented in this work provides the basis for further optimization and development of a foot deformity classification system. Full article

Background and Objective: Acute Achilles tendon rupture (AATR) is a common injury with a significant impact on daily living. Although various systematic reviews and meta-analyses have been written on the topic, no actual consensus exists on the best treatment. We aimed to collect the highest quality of evidence on the subject and to produce a document to which to refer, from the diagnosis to the final treatment. Material and Methods: Inclusion criteria were systematic reviews discussing Achilles tendon rupture, concerning either diagnostic criteria, classification, or treatment; English language; clearly stated inclusion and exclusion criteria for patients’ selection. Results: Thirteen systematic reviews were included in the study. A strong consensus exists about the higher risk of re-rupture associated with non-operative treatment and a higher risk of complications associated with surgical repair. Conclusions: The combination of minimally invasive repair and accelerated functional rehabilitation seems to offer the best results in the treatment of Achilles tendon rupture. Full article
