Interns’ Abuse Across the Healthcare Specialties in Saudi Arabian Hospitals and Its Effects on Their Mental Health

Healthcare abuse is a critical human rights and public health issue, particularly impacting medical interns and trainees who are vulnerable to mistreatment during their formative professional years. This cross-sectional study, conducted from February to June 2024, evaluated the prevalence and psychological impact of harassment and discrimination among 463 healthcare interns in Saudi Arabia from various specialties, including medicine, nursing, pharmacy, and dentistry. Using a self-administered online questionnaire, we found that mistreatment was widely reported, with female interns experiencing significantly higher rates of sexual harassment and gender-based discrimination. Common perpetrators included residents, lecturers, professors, nurses, and patients, with incidents most frequently occurring in surgical and internal medicine departments. Despite high prevalence, only 9% of interns reported the abuse due to mistrust in reporting systems or failure to recognize the behavior as abuse. These experiences were associated with significant psychological distress, including frustration, reduced motivation to learn, and higher DASS scores, particularly among female interns. The study underscores the need for institutional reforms, including policy development, cultural change, and effective reporting systems to ensure a safe and supportive learning environment for future healthcare professionals. Addressing abuse in medical training is essential for individual well-being and the sustainability and integrity of healthcare systems. Full article

Managers often need to stay motivated and effectively motivate others. Therefore, they should rely on evidence-based interventions to effectively motivate and self-motivate. This research investigated how self-determination theory-based interventions affect employees’ motivation dynamics and motivational consequences within short time frames (i.e., within an hour, within a few weeks or months) in two empirical studies. Study one focused on assessing the effectiveness of a one-day training workshop in helping to improve managers’ work motivation, basic psychological needs satisfaction/frustration, subordinates’ motivation, and perceptions of managers’ needs-supportive/thwarting behaviors within a few weeks. Results support the effectiveness of the training, as managers were rated by their direct subordinates as having fewer needs-thwarting behaviors and reported self-improvement in needs satisfaction and frustration six weeks after completing the training program. Study two used the mean and covariance structure analysis and tested the impact of three types of basic psychological needs-supportive/thwarting and control conditions (3 × 2 × 1 factorial design) on participants’ situational motivation, vitality, and general self-efficacy for playing online word games within 30 min. Multi-group confirmatory factor analysis (CFA) confirmed the scalar measurement invariance, then latent group mean comparison results show consistently lower controlled motivation across the experimental conditions. During a quick online working scenario, the theory-based momentary intervention effectively changed situational extrinsic self-regulation in participants. Supplementary structural equation modeling (SEM; cross-sectional) analyses using experience samples supported the indirect dual-path model from basic needs satisfaction to vitality and general efficacy via situational motivation. We discussed the theoretical implications of the temporal properties of work motivation, the practical implications for employee training, and the limitations. Full article

Background: Fundamental movement skills (FMS)—including muscular strength, endurance, and mobility—are linked to better health-related quality of life and higher physical activity in children. Rural children often score lower on FMS tests than urban peers due to resource limitations rather than ability. Thus, increasing access to FMS activities in under-resourced rural areas is essential. The primary objective was to test the feasibility of Hoosier Strength in a rural middle school sample, and the secondary objective was to observe the preliminary changes in FMS-related outcomes pre- to post-intervention and at follow-up. The exploratory objective was to explore how participants responded to different coaches on the Hoosier Strength coaching team (i.e., gender, coaching style during activities). Methods: This study used a Hybrid Type 3 design to evaluate feasibility and FMS outcomes, integrating qualitative and quantitative data. The four-week intervention included a test group (n = 24; 14 females, 10 males; mean age: females 12.4 ± 0.5 years, males 12.7 ± 0.4 years) and a control group (n = 12; 8 females, 4 males; mean age: females 12.9 ± 0.3 years, males 12.7 ± 0.3 years). Data analysis included descriptive statistics for feasibility indicators (Acceptability of Intervention Measures [AIM], Intervention Appropriateness Measure [IAM], and Feasibility of Intervention Measure [FIM]), linear regression for mobility and muscular endurance changes, t-tests for psychological need satisfaction and frustration, and regression analysis for squat knowledge and post-intervention confidence. Results: (1) There was high feasibility across the 4-week Hoosier Strength intervention and at follow-up; (2) there were no statistically significant changes in squat performance; (3) participants’ confidence in their ability to squat at the end of the intervention was significantly predicted by their squat knowledge at baseline; and (4) participants prioritized leadership and team management over tactical analysis, highlighting a preference for coaches who foster teamwork. Conclusions: The findings offer a transparent approach for evaluating the feasibility and preliminary outcomes of the Hoosier Strength intervention in an under-resourced rural middle school, thereby encouraging further investigation into strength training interventions in rural schools. Background/Objectives: Boarding refers to the period when patients deemed stable in the emergency department (ED) are temporarily monitored, wait to be admitted, and receive appropriate care. As life expectancy increases, so does the importance of understanding the dynamics and experiences of older adults with frailty in emergency settings. The absence of a care environment tailored to specific needs could diminish the overall quality of care provided, threatening the health and well-being of this population. To our knowledge, how frail older adults experience this has not yet been synthesized in a qualitative systematic review. The aim of this study was to explore the lived experiences of frail older adults during the emergency department (ED) boarding phase Methods: This systematic review was conducted using PubMed, OVID, and Scopus in October 2024. No time restrictions were settled and only articles published in English were included. Following the predefined inclusion criteria, two researchers independently extracted and synthesized the data using the Joanna Briggs Institute (JBI) meta-aggregation methodology and instruments. Results: Seven studies were included. Thirty-one findings were identified and grouped into seven categories and three themes regarding the lived experiences of frail people in ED boarding areas. The themes we identified were discomfort, distress, frustration, the experience of positive/negative attitudes of healthcare providers, and the supportive role of family members during ED LOS (length of stay). Conclusions: Older frail adults experience significant physical and psychological distress during ED boarding. It is necessary to manage their specific needs through targeted actions aimed at improving their overall experience and quality of care in emergency settings. Full article

Cyberbullying can be prevalent across different life stages, with lasting traces on mental health across the lifespan. This study aims to (a) explore how cyberbullying is emotionally experienced across three distinct age groups and (b) analyze the influence of cyberbullying on mental health across the lifespan. This study included 883 participants divided into three age groups: 18–39, 40–59, and 60+. In-depth semi-structured interviews were conducted to gather participants’ experiences and perspectives. The data were then subjected to content analysis, which revealed a number of themes. The first objective revealed the following themes: For ages 18–39: (a) feeling ashamed or humiliated (92.4%), (b) withdrawing from friends and family, and (c) experiencing harassment as positive and difficulties with rules. For ages 40–59: (a) losing interest in hobbies (89.5%), (b) questioning about things they did or did not do, and (c) experiencing a sense of missing out. For ages 60+: (a) negative thoughts and self-talk (91.3%), (b) feeling judged negatively, and (c) feeling financially vulnerable. The second objective showed: For 18–39: (a) depressive symptoms (79.7%), (b) easy anger, and (c) suicidal behavior. For 40–59: (a) anxiety (93.2%), (b) low self-esteem, and (c) the use of substances. For 60+: (a) frustration (78.1%), (b) isolation, and (c) disturbances in sleep and eating patterns. This study highlights the significant psychological and emotional impact of cyberbullying across age groups, emphasizing the need for targeted interventions that address the unique challenges faced by individuals at different life stages. The findings underscore the importance of developing age-specific strategies to mitigate the effects of cyberbullying and to have perpetrators take responsibility for their reckless disregard for others, and ultimately, themselves. Type 1 diabetes (T1D) is a chronic disease that influences all health aspects. The self-determination theory (SDT) suggests that three psychological needs of competence, autonomy, and relatedness are necessary for motivation to engage in healthy behaviors. Through medical specialty camps, these needs can be met by educating campers on how to manage T1D and realize they are not alone. The volunteer-based, five-day, inaugural tween–teen camp for youth with T1D was held on the campus of a university. The entire camp experience was engineered around the three basic needs described by the SDT. These needs were promoted by physical and educational activities and meeting friends. The Basic Psychological Needs and Frustration Scale was administered as a pre-, post-, and follow-up test. Two of the four measures were significant, the autonomy pretest (M = 3.93; SD = 0.75) to posttest (M = 4.49; SD = 0.56), with t(25) = −6.258; p ≤ 0.001) and relatedness pretest (M = 4.31; SD = 0.79) to posttest (M = 4.53; SD = 0.49), with t(25) = −2.168; p = 0.040; however, mean scores declined at the 3-month follow-up. Campers’ blood glucose levels were collected through an online platform that allowed staff to monitor blood glucose levels, with the week’s average being 150 mg/dL, and almost 71% remaining in the TIR. The results can be helpful for practitioners who want to use the SDT to structure and examine the effectiveness of medical specialty camps for youth with T1D. Full article

With the current rapid increase in digital technology adoption, understanding the relationship between technological proficiency and self-esteem is crucial to older adults. This study explores the mechanisms through which technological proficiency influences self-esteem, particularly considering its associations with technological engagement, self-efficacy, social participation, and autonomy. By using a narrative review approach, this study synthesizes recent findings from gerontology and psychology to examine these relationships. The analysis shows that higher technological proficiency enhances self-esteem by boosting self-efficacy, fostering social participation, and promoting greater autonomy. Older adults who are skilled in using digital tools exhibit greater confidence in managing daily activities and maintaining their social networks, contributing to their psychological resilience. Conversely, lower technological proficiency is linked to frustration, social exclusion, and diminished self-esteem, exacerbated by the digital divide. However, despite these insights, much remains unexplored; for example, no longitudinal study has been conducted to capture the dynamic relations between technological proficiency and self-esteem. These findings suggest that further research is needed to learn more about how technological proficiency influences self-esteem and recommend clinical ways of supporting older adults in their current stage. Background/Objectives: As fitness apps increasingly incorporate social interaction features, users may find themselves overwhelmed by an excess of received support, struggling to effectively manage it. Highlighting a novel recipient-centric perspective, we aim to investigate the impact of social support overload on users’ life burnout and discontinuance within fitness apps. Methods: Utilizing Social Support Theory and Basic Psychological Needs Theory, we develop a model to examine how emotional, network, and informational support overload affect life burnout and discontinuance through the frustration of basic psychological needs: autonomy, competence, and relatedness. A total of 443 fitness app users were included in our study, and we employed Structural Equation Modeling (SEM) to empirically test this model. Results: The results highlight the significant mediating role of the frustration of basic psychological needs between social support overload and life burnout/discontinuance. Network and informational support overload positively correlate with frustration of all needs, whereas emotional support overload shows a complex relationship. All need frustrations are linked to life burnout, but only autonomy and relatedness frustrations significantly lead to discontinuance. Additionally, gender and app use proficiency are significant control variables impacting discontinuance. Conclusions: This study adopts a novel recipient-centric perspective to explore social support overload, examining its effects on life burnout and discontinuance and offering practical implications for both users and app managers. Multidisciplinary collaboration is key to strengthening the evidence base for multifaceted illness prevention interventions. We bring together health economics and behavioral science to explore the well-being benefits and social cost–benefit of volunteer gardening at an accredited botanic garden, Wales, UK. A health economics-informed social return on investment (SROI) evaluation was combined with the assessment of volunteers’ basic psychological needs (autonomy, competence, and relatedness), connection to nature, and their interrelatedness in this innovative nature-based intervention study. Pre- and post-volunteering outcome data were collected using the Short Warwick-Edinburgh Mental Well-being Scale (SWEMWBS), the ICEpop CAPability measure for Adults (ICECAP-A), the 12-item diary version of the Basic Psychological Need Satisfaction and Frustration Scale (BPNSNF), the Nature Connection Index (NCI), and a bespoke Client Service Receipt Inventory (CSRI). Results indicate that volunteer gardening can provide well-being benefits to participants and cost savings to the NHS. The well-being benefits observed were estimated to generate social value in the range of GBP 4.02 to GBP 5.43 for every GBP 1 invested. This study contributes to the evidence base that simple nature-based interventions such as volunteer gardening could offer low-cost supportive environments that deliver significant well-being benefits and associated social value to local communities, including a reduced burden on overstretched local healthcare services. Background/Objectives: Basic psychological needs are essential for fostering motivation, self-regulated behaviors, and overall well-being. For cancer survivors, fulfilling these needs is crucial for coping with the various challenges of survivorship and for enhancing psychosocial health. This study aimed to assess the validity and reliability of the Korean version of the Basic Psychological Needs Satisfaction and Frustration Scale (K-BPNSFS) in a cancer survivor population. Methods: A cross-sectional design was employed, involving 367 community-dwelling cancer survivors. Construct validity was assessed using confirmatory factor analysis with multiple fit indices, while convergent validity was examined through Pearson’s correlation coefficients. Reliability was evaluated using internal consistency, inter-item correlations, and item-total correlations. Results: The findings confirmed a robust six-factor structure of the K-BPNSFS, which includes satisfaction and frustration dimensions corresponding to autonomy, relatedness, and competence needs. Convergent validity was supported by significant correlations with relevant constructs, aligning with the scale’s theoretical underpinnings. Reliability analysis demonstrated high internal consistency across all dimensions, with strong Cronbach’s alpha values and substantial item-total and inter-item correlations. Conclusions: This study establishes the K-BPNSFS as a valid, reliable, and culturally relevant instrument for assessing the basic psychological needs of Korean cancer survivors. Application of this scale provides critical insights into the unique psychological needs of this population, supporting the development of targeted healthcare strategies to enhance intrinsic motivation, self-care, and overall quality of life. This study aimed to explore the nature of contextual differences in child–parent attachment relationships and examine how these experiences relate to children’s psychological outcomes. A theoretically informed qualitative study was conducted using semi-structured interviews with 15 participants across four groups of Taiwanese youths and parents, representing different contextual attachment combinations. Data were analyzed using a hybrid thematic analysis approach, integrating both inductive and deductive methods. The findings revealed two key characteristics of secure attachment across contexts: (1) parental timely and sensitive responsiveness enhances children’s openness to share personal thoughts, and (2) parental empathic and genuine concern fosters children’s empathy. Conversely, insecure attachment across contexts was marked by (1) inappropriate parental responses reducing children’s willingness to share their inner feelings, and (2) parental lack of empathy frustrating children and discouraging communication. In addition, parental beliefs about children’s achievement behaviors (e.g., expectation primarily centered on academic achievement and viewing athletic achievement as an alternative pathway to higher education) and parenting practices (e.g., performance- vs. mastery-oriented achievement goals, conditional negative regard, and psychological control through invalidating feelings and demeaning the child) significantly influenced children’s psychological outcomes, particularly in terms of basic psychological needs satisfaction and frustration (autonomy and competence) across academic and sports domains. These findings advance our theoretical understanding of contextual attachment dynamics and provide practical insights for fostering optimal parenting strategies, particularly in academic and sport-related contexts. Methods: We conducted semi-structured interviews with YA cancer survivors in peak years for frontal neurocognitive development (ages 18–30), and we asked them to describe their experiences with cognitive and mental health changes during and after treatment for their cancer. An applied thematic analysis was used to identify themes. Results: The participants were 20 YA cancer survivors who were, on average, 23 years old at diagnosis and 26 years old when interviewed. Five themes emerged regarding the connection between cognitive and mental health: depressed mood after diagnosis that decreased mental engagement, cancer-related anxiety consumed cognitive resources during treatment, a bidirectional “downward spiral” during treatment, frustration with forgetfulness, and self-criticism about long-term cognitive limitations. Additionally, participants described receiving minimal information about evidence-based strategies for managing CRCI. Conclusions: Future research is needed to examine common neurological mechanisms among symptoms of depression, anxiety, and CRCI and to address gaps in survivorship care for YA survivors with CRCI. Cognitive–behavioral interventions may address both cognitive and mental health. Full article

Job Satisfaction Among First-Generation Migrant Physicians in Anesthesiology and Intensive Care Medicine in Germany

Background/Objectives: This study examines job satisfaction, burnout, and well-being among first-generation migrant physicians in anesthesiology and intensive care medicine in Germany, comparing them to their native German counterparts. Methods: A cross-sectional survey design was utilized, collecting data from 513 physicians, 110 of whom identified as having a migration background. Job satisfaction was measured using the Warr-Cook-Wall (WCW) Job Satisfaction Scale, burnout was assessed with the Copenhagen Burnout Inventory (CBI), and well-being was evaluated using the WHO-5 Well-Being Index. Results: The job satisfaction ratings revealed no significant differences between migrant and German physicians in most dimensions, including physical workload, freedom to choose work methods, satisfaction with colleagues, responsibility, income, skill utilization, and variety in work tasks. However, migrant physicians reported significantly higher satisfaction with recognition received for their work and lower dissatisfaction with working hours. Burnout assessments showed that migrant physicians experienced higher psychological strain, perceiving every work hour as more exhausting and having significantly less energy for family and friends. Migrant physicians reported higher difficulty and frustration in working with patients. Well-being items indicated that migrant physicians felt less energetic and active but found their daily life more filled with interesting activities. Notably, the multivariate analyses of the total scale scores did not show significant associations between migration background and the overall outcome scales. Conclusions: The findings indicate unique challenges faced by migrant physicians, particularly in terms of recognition and patient-related burnout. These results highlight the need for targeted interventions to support migrant physicians, including cultural competence training and flexible working hours to enhance their job satisfaction and overall well-being. Addressing these issues is crucial for maintaining the quality of patient care and the occupational health of migrant physicians in Germany. Full article

Recreational football has shown growing evidence that it could be played safely in adults aged 60+ and that it is physically beneficial. Less is known about the psychological aspects, except for the lived experiences of players. The aim of the present study was to analyze both physiological and psychological effects of short-term recreational football. Fifteen participants took part in a six-week training program of recreational football played at a walking pace with two sessions of 1 h and 30 m per week. Physical fitness was assessed before and after the training period and psychological questionnaires were given at the same time. Body mass and body mass index were significantly decreased, but no other significant effects were found on physical fitness. Participants experienced less frustration related to psychological needs (autonomy and competence). Six weeks were too short to observe significant physical improvements while psychological benefits were already experienced. In this short period, psychological aspects seem predominant. These effects may encourage to long-term adhesion. The activity has the potential to keep adults 60+ exercising, which is important for maintaining good global health and seeing physical changes later. Full article

The aim of this study is to examine the relationship between the psychological needs of athletes who attend gyms, the spirituality index of well-being, and motivation in sports, and to determine the mediating role of the spirituality index of well-being between psychological needs in sports and motivation in sports. Although sports spirituality is rapidly developing in the international literature, sports and spirituality have not been studied together in the national literature before. Therefore, this study holds the distinction of being one of the pioneering research efforts on the subject of spirituality in athletes in Türkiye. In this context, the data of 422 athletes, 176 women and 246 men, who have been attending gym facilities in Antalya for at least one year, were included in the research. Participants were administered a personal information form, Psychological Need States. It was found that the spirituality index of well-being is positively related to all satisfaction subscales and negatively related to all frustration subscales of psychological needs and that the life scheme is related to many subscales of sport motivation. Furthermore, it was also found that the spirituality index of well-being is a mediating variable between psychological needs in sports and motivation in sports. The data draw attention to the importance of sports spirituality. It is recommended to integrate spirituality into healthcare for athletes. Full article
