Assessment of Hydrogels for Intra-Articulate Application, Based on Sodium Hyaluronate Doped with Synthetic Polymers and Incorporated with Diclofenac Sodium

The intra-articular application of drugs has gained considerable interest with regard to formulations for advanced drug delivery systems. It has been identified as a potential route for local drug delivery. A drug agent is usually incorporated into the hydrogel to prolong and control the drug release. This study aimed to design and evaluate an intra-articular hydrogel based sodium hyaluronate, which was modified with an additional polymer to enable the sustained release of the incorporated anti-inflammatory agent, diclofenac sodium (NaDic). Viscosity studies, drug release tests and FTIR−ATR measurements, as well as DSC analysis, were carried out to evaluate the obtained formulations. The viscosity measurements were performed using a rotational viscometer. The drug release was carried out by employing the apparatus paddle over the disk. The concentration of the released drug was obtained spectrophotometrically. The results revealed that the addition of the second polymer to the matrix influenced the dynamic viscosity of the hydrogels. The highest viscosity of (25.33 ± 0.55) × 103 cP was observed when polyacrylic acid (PA) was doped in the formulation. This was due to the hydrogen bond formation between both polymers. The FTIR−ATR investigations and DSC study revealed the hydrogen bond formation between the drug and both polymers. The drug was released the slowest from hydrogel doped with PA and 17.2 ± 3.7% of NaDic was transported to the acceptor fluid within 8 h. The hydrogel based on hyaluronan sodium doped with PA and containing NaDic is a promising formulation for the prolonged and controlled intra-articulate drug delivery of anti-inflammatory agents. Alternative Hydraulic Modeling Method Based on Recurrent Neural Networks: From HEC-RAS to AI

The present study explores the application of RNNs for the prediction and propagation of flood waves along a section of the Bârsa River, Romania, as a fast alternative to classical hydraulic models, aiming to identify new ways to alert the population. Five neural architectures were analyzed as follows: S-RNN, LSTM, GRU, Bi-LSTM, and Bi-GRU. The input data for the neural networks were derived from 2D hydraulic simulations conducted using HEC-RAS software, which provided the necessary training data for the models. It should be mentioned that the input data for the hydraulic model are synthetic hydrographs, derived from the statistical processing of recorded floods. Performance evaluation was based on standard metrics such as NSE, R2 MSE, and RMSE. The results indicate that all studied networks performed well, with NSE and R2 values close to 1, thus validating their capacity to reproduce complex hydrological dynamics. Overall, all models yielded satisfactory results, making them useful tools particularly the GRU and Bi-GRU architectures, which showed the most balanced behavior, delivering low errors and high stability in predicting peak discharge, water level, and flood wave volume. The GRU and Bi-GRU networks yielded the best performance, with RMSE values below 1.45, MAE under 0.3, and volume errors typically under 3%. On the other hand, LSTM architecture exhibited the most significant instability and errors, especially in estimating the flood wave volume, often having errors exceeding 9% in some sections. The study concludes by identifying several limitations, including the heavy reliance on synthetic data and its local applicability, while also proposing solutions for future analyses, such as the integration of real-world data and the expansion of the methodology to diverse river basins thus providing greater significance to RNN models. The final conclusions highlight that RNNs are powerful tools in flood risk management, contributing to the development of fast and efficient early warning systems for extreme hydrological and meteorological events. This study aims to demonstrate how mathematics, especially calculus concepts, can be expanded to include semi-entities and how these can be applied to sampling activities. Here, the multivalued logic uses pseudo-complemented lattices, instead of Boolean algebras. Truth values can express the intensity of a property: for example, the property of being heavy intensifies as weight increases. They can also express the state-of-the-art knowledge of an individual about a certain thing. To express that a number 𝑥 approaches 𝑎 is to say that the statement “𝑥 = 𝑏” is not fully true but approaches the full-true value as 𝑏 − 𝑎 approaches zero. This approach generalizes the concept of a limit and the concepts derived from it, such as differentiation and integration. A Monte Carlo algorithm replaces one function with another with finite domain, preferably its finite part, by sampling the domain and calculating its map. The discussion extends to integration over an unbounded interval, integral transforms, and differential equations. This study then covers strategies for producing Monte Carlo estimates of respective problems and determining their crucial truth values. In the discussion, a topic related to axiomatizing set theory is also suggested. Full article

This article explores the concept of textuality as embedded within contemporary architecture, understood as the capacity of buildings to generate meanings, narratives, and interpretations that transcend their physical and functional dimensions. An interdisciplinary approach is adopted, integrating architectural theory, semiotics, hermeneutics, and cultural studies, positioning architecture as a form of symbolic production deeply intertwined with current social and technological contexts. The primary aim is to demonstrate how certain paradigmatic buildings operate as open texts that engage in dialogue with their users, urban surroundings, and cultural frameworks. The methodology combines theoretical analysis with an in-depth study of three emblematic cases: the Guggenheim Museum in Bilbao, the Centre Pompidou in Paris, and the Seattle Public Library. The findings reveal that these buildings articulate multiple layers of meaning, fostering rich and participatory interpretive experiences that influence both the perception and construction of public space. The study concludes that contemporary architecture functions as a narrative and symbolic device that actively contributes to the shaping of collective imaginaries. The article also identifies the study’s limitations and proposes future research directions concerning architectural textuality within the context of emerging digital technologies. Full article

Anterior Redisplacement After Intramedullary Nail Fixation for Trochanteric Femoral Fractures: Incidence and Risk Factors in 598 Older Patients

Background/Objectives: Anterior redisplacement, defined as a postoperative anterior shift of the distal fragment despite intraoperative reduction, is occasionally observed after cephalomedullary nailing for trochanteric femoral fractures. However, its incidence and associated risk factors remain unclear. This study aimed to determine the incidence of anterior redisplacement following intramedullary nail fixation in geriatric trochanteric fractures, and to identify independent risk factors. Methods: This study retrospectively reviewed data from 598 consecutive hips in 577 patients (aged ≥ 65 years) who underwent intramedullary nail fixation for trochanteric fractures at a single center (2012–2023). Sagittal reduction on the lateral radiographic view was classified as posterior, anatomical, or anterior according to the position of the distal fragment, and was recorded preoperatively and postoperatively. Anterior redisplacement, the primary outcome, was defined as a change in alignment from a posterior or anatomical position postoperatively to an anterior position on any subsequent follow-up radiograph. Independent risk factors were identified by logistic regression. Results: Among the 543 hips reduced posteriorly (n = 204) or anatomically (n = 339), anterior redisplacement occurred in 73 (13.4%). The incidence of anterior redisplacement was significantly higher following anatomical compared to posterior reduction (19.5% vs. 3.4%; p < 0.001), and also higher in fractures that were anteriorly aligned preoperatively (18.0%) compared to anatomical (8.5%; p < 0.01) and posterior (6.2%; p < 0.01) alignment. Multivariate analysis revealed two independent predictors: preoperative anterior alignment (odds ratio [OR] 1.87, 95% confidence interval [CI] 1.24–2.81; p = 0.003) and postoperative anatomical (vs. posterior) reduction (OR 6.49, 95% CI 2.92–14.44; p < 0.001). Age, sex, Arbeitsgemeinschaft für Osteosynthesefragen/Orthopaedic Trauma Association classification, Evans–Jensen classification, nail length, and canal-filling ratio were not associated with redisplacement. No lag‑screw cutout occurred during the follow-up. Conclusions: Anterior redisplacement occurred in one of seven geriatric trochanteric fractures despite apparently satisfactory fixation. An anatomical sagittal reduction—traditionally considered “ideal”—increases the risk more than sixfold, whereas a deliberate posterior-buttress is protective. Unlike patient-related risk factors, sagittal reduction is under the surgeon’s control. The study findings provide evidence that choosing a slight posterior bias can significantly improve stability. Sperm count, motility, and morphology are semen parameters that directly affect male fertility. The presence of cytokines in seminal plasma negatively or positively influences these parameters. Interleukins and prostaglandins are proinflammatory cytokines present in human seminal plasma and play crucial roles in fertilization, in general and after intracytoplasmic sperm injection (ICSI) procedures. This study aimed to investigate the possible influence of interleukins IL-17 and IL-18, and prostaglandins PGE2 and PGF2α on male infertility. Semen samples were collected from 58 males who underwent the ICSI procedure. An enzyme-linked immunosorbent assay (ELISA) was used to determine the levels of IL-17, IL-18, PGE2, and PGF2α, and these concentrations were then correlated with semen parameters and the rate of fertilization. Furthermore, the chromatin integrity of the sperm was evaluated with an Acridine Orange (AO) assay. The results showed an inversely proportional relationship between the AO binding intensity and fertilization rate (r = −0.394; p ≤ 0.002). Furthermore, a negative correlation was observed between the IL-18 concentration and positive AO (p ≤ 0.021). Moreover, the IL-18 concentration was positively correlated with the fertilization rate (p ≤ 0.05). In contrast, IL-17 did not significantly correlate with any semen parameters or with the fertilization rate. Seminal PGE2 levels were significantly correlated with embryo cleavage at 72 h (p ≤ 0.05). To conclude, this study revealed that denaturation of sperm nuclear deoxyribonucleic acid (DNA) contributes to low fertilization rates. In addition, this study proposed a potential role for IL-18 in fertilization. PGE2 likely influences embryo development, but further studies are needed to examine the impact of seminal PGE2 on the oocyte to fully elucidate its contribution to this complex biological process. The rising demand for energy resources and industrial goods presents significant challenges to sustainable development. Sunflower oil, commonly utilized in the food sector, biofuels, and various industrial applications, is notably affected by this demand. In Bulgaria, it serves as a primary source of vegetable fats, ranking second to butter in daily consumption. The aim of this study is to evaluate and propose methods to improve energy and resource efficiency in sunflower oil production in Bulgaria. The analysis is based on data from an energy audit conducted in 2023 at an industrial sunflower oil production facility. Reconstruction and modernization initiatives, which included the installation of high-performance, energy-efficient equipment, led to a 34% increase in energy efficiency. The findings highlight the importance of adjusting the technological parameters such as temperature, pressure, grinding level, and pressing time to reduce energy use and operational costs. Additionally, resource efficiency is improved through more effective raw material utilization and waste reduction. These strategies not only enhance the economic and environmental performance of sunflower oil production but also support sustainable development and competitiveness within the industry. The improvement reduces hexane use by approximately 2%, resulting in energy savings of 12–15 kWh/t of processed seeds and a reduction in CO2 emissions by 3–4 kg/t, thereby improving the environmental profile of sunflower oil production. Combining products of natural origin with different mechanisms of action on insect herbivores may provide an alternative among methods of plant protection against pests that are less risky for the environment. The aim of the study was to evaluate the effectiveness of mixtures of Thuja occidentalis L. essential oil and diatomite (EO + DE) compared to each substance separately in reducing economically important pests such as black bean aphid (BBA) Aphis fabae Scop., Colorado potato beetle (CPB) Leptinotarsa decemlineata Say., and pea leaf weevil (PLW) Sitona lineatus L. The effects on mortality (all pests) and foraging intensity (CPB and PLW) were tested. The improvement in effectiveness using a mixture of EO + DE versus single components against BBA was dose- and the developmental stage-dependent. The effect of enhancing CPB foraging inhibition through DE addition was obtained at a concentration of 0.2% EO (both females and males of CPB) and 0.5% EO (males) in no-choice experiments. In choice experiments, mixtures EO + DE with both 0.2% and 0.5% EO concentrations resulted in a significant reduction in CPB foraging. A significant strengthening effect of EO 0.5% through the addition of DE at a dose of 10% against PLW males was observed in the no-choice experiment, while, when the beetles had a choice, the synergistic effect of a mixture of EO 0.5% and DE 10% was also apparent in females. In conclusion, the use of DE mixtures with EO from T. occidentalis appears to be a promising strategy. The results support the idea of not using doses of EO higher than 0.5%. Full article

Background/Objectives: Curcumin (CUR) is well known for its therapeutic properties, particularly attributed to its antioxidant and anti-inflammatory effects in managing chronic diseases such as arthritis. While CUR application for biomedical purposes is well known, the phytochemical has several restrictions given its poor water solubility, physicochemical instability, and low bioavailability. These limitations have led to innovative formulations, with nanocarriers emerging as a promising alternative. For this reason, this study aimed to address the potential advantages of associating CUR with nanocarrier systems in managing arthritis through a scoping review. Methods: A systematic literature search of preclinical (in vivo) and clinical studies was performed in PubMed, Scopus, and Web of Science (December 2024). General inclusion criteria include using CUR or natural derivatives in nano-based formulations for arthritis treatment. These elements lead to the question: “What is the impact of the association of CUR or derivatives in nanocarriers in treating arthritis?”. Results: From an initial 536 articles, 34 were selected for further analysis (31 preclinical investigations and three randomized clinical trials). Most studies used pure CUR (25/34), associated with organic (30/34) nanocarrier systems. Remarkably, nanoparticles (16/34) and nanoemulsions (5/34) were emphasized. The formulations were primarily presented in liquid form (23/34) and were generally administered to animal models through intra-articular injection (11/31). Complete Freund’s Adjuvant (CFA) was the most frequently utilized among the various models to induce arthritis-like joint damage. The findings indicate that associating CUR or its derivatives with nanocarrier systems enhances its pharmacological efficacy through controlled release and enhanced solubility, bioavailability, and stability. Moreover, the encapsulation of CUR showed better results in most cases than in its free form. Nonetheless, most studies were restricted to the preclinical model, not providing direct evidence in humans. Additionally, inadequate information and clarity presented considerable challenges for preclinical evidence, which was confirmed by SYRCLE’s bias detection tools. Conclusions: Hence, this scoping review highlights the anti-arthritic effects of CUR nanocarriers as a promising alternative for improved treatment. Full article

Objectification of the Functional Myodiagnosis Muscle Test

Objective: This study aimed to investigate whether the subjective assessments of strong and weak muscles in the Functional Myodiagnosis muscle test (FMD-MT) can be objectively and reproducibly verified using physically measurable parameters. Additionally, we sought to evaluate the reliability of the manual muscle test in order to reinforce the scientific evidence supporting this accepted, yet not widely adopted, complementary medicine method. Methods: In a crossover observational study, three experienced medical practitioners conducted the FMD-MT of the rectus femoris muscle on 24 healthy participants using a specially designed therapy bench, with all measurements recorded via an oscillogram. The study investigated the force–time integral, joint angle change, additional force load, mean force turning point 1, as well as the interrater reliability and validity of both examiner assessments and instrumental analyses for the two muscle reaction variants: strong and weak. Results: A significant difference between the response pattern of strong and weak muscles was identified for the force–time integral (p = 0.005), the change in joint angle (p < 0.001), and the additional force load (p = 0.001). No difference between strong and weak muscles could be detected regarding the force turning point 1 (p = 0.972). The examiners demonstrated 100% accuracy in identifying weak muscle reactions as weak, and 99.2% accuracy in identifying strong muscle reactions as strong (p = 0.316). The overall intra-class correlation coefficient was 0.984. The oscillogram correctly visualized weak muscle reactions in weak muscles with an accuracy of 81.7%, and strong muscle reactions in strong muscles with an accuracy of 86.7% (p = 0.289). Conclusions: The Functional Myodiagnosis muscle test (FMD-MT) enables a clear and objective differentiation between strong and weak muscles, with statistically significant differences observed in the force–time integral, additional force load, and joint angle changes. Under rigorously standardized testing conditions, the FMD-MT of the rectus femoris muscle demonstrates a validity rate of 99.6% and an excellent reliability (ICC 0.984). Self-Reported Weight Gain After the Age of 20 and Risk of Steatotic Liver Disease

Background/Objectives: We aimed to identify questionnaire items associated with an increased risk of developing hepatic steatosis in the general population. Methods: A total of 15,063 individuals aged ≥20 years who underwent general health checkups and had no hepatic steatosis at baseline were included. The relationship between questionnaire data at baseline and hepatic steatosis incidence over a median 4.2-year follow-up was investigated across body mass index (BMI) categories. Results: Among 15,063 individuals (mean [SD] age, 47.1 [10.2] years; 6769 [44.9%] male; mean [SD] BMI, 21.4 [2.6] kg/m2), 1889 individuals (12.5%) developed hepatic steatosis during follow-up. After adjusting for age, sex, and factors related to metabolic diseases and liver injury, the strongest questionnaire-based risk factor for hepatic steatosis was self-reported weight gain of 10 kg or more after the age of 20 across all BMI categories: total population (hazard ratio [HR], 2.11; 95% confidence interval [CI], 1.90–2.34; p < 0.001), Category 1 (BMI < 22) (HR, 2.33; 95% CI, 1.86–2.91; p < 0.001), Category 2 (BMI 22 to <25) (HR, 1.43; 95% CI, 1.25–1.63; p < 0.001), and Category 3 (BMI ≥ 25) (HR, 1.41; 95% CI, 1.12–1.77; p = 0.003). Conclusions: In this cohort study, self-reported weight gain of 10 kg or more after the age of 20 was associated with an increased risk of hepatic steatosis, independent of baseline BMI. Questionnaires capturing weight gain history may support universal screening efforts to identify individuals at elevated risk. Full article

Euterpe precatoria Mart. is an increasingly important palm for subsistence and income generation in central and western Amazonia with growing demand for its fruit pulp, which is an alternative source of açaí juice for domestic and international markets. This study synthesizes current knowledge on its systematics, ecology, fruit production in natural populations, fruit quality, uses, population management, and related areas, identifying critical research gaps. A systematic literature survey was conducted across databases including Web of Science, Scopus, Scielo, CAPES, and Embrapa. Of 1568 studies referencing Euterpe, 273 focused on E. precatoria, with 90 addressing priority themes. Genetic diversity studies suggest the E. precatoria may represent a complex of species. Its population abundance varies across habitats: the highest variability occurs in terra firme, followed by baixios and várzeas. Várzeas exhibit greater productivity potential, with more bunches per plant and higher fruit weight than baixios; no production data exist for terra firme. Additionally, E. precatoria has higher anthocyanin content than E. oleracea, the primary commercial açaí species. Management of natural populations and cultivation practices are essential for sustainable production; however, studies in these fields are still limited. The information is crucial to inform strategies aiming to promote the sustainable production of the species. Full article

“The Internet of Things” is a relatively new idea that refers to objects that can connect to the Internet and exchange data. The Internet of Things (IoT) enables novel interactions between objects and people by interconnecting billions of devices. While there are many IoT-related products, challenges pertaining to their effective implementation, particularly the lack of knowledge and confidence about security, must be addressed. To provide IoT-based enterprises with a platform for efficiency and sustainability, this study aims to identify the critical elements that influence the growth of a successful company integrated with an IoT system. This study proposes a decision support tool that evaluates the influential features of IoT using the Pythagorean Fuzzy and Interval Grey approaches within the Analytical Hierarchy Process (AHP). This study demonstrates that security, value, and connectivity are more critical than telepresence and intelligence indicators. When both strategies are used, market demand and information privacy become significant indicators. Applying the Pythagorean Fuzzy approach enables the identification of sensor networks, authorization, market demand, and data management in terms of importance. The application of the Interval Grey approach underscores the importance of data management, particularly in sensor networks. The indicators that were finally ranked are compared to obtain a good coefficient of agreement. These findings offer practical insights for promoting sustainability in enterprise operations by optimizing IoT infrastructure and decision-making processes. Full article

The interfacial transition zone (ITZ) significantly influences the mechanical properties and durability of lightweight aggregate concrete (LWAC), yet existing research on the ITZ in LWAC remains fragmented due to varied characterization techniques, inconsistent definitions of ITZ thickness and porosity, and the absence of standardized performance metrics. This review focuses primarily on structural LWAC produced with artificial and natural lightweight aggregates, with intended applications in high-performance civil engineering structures. This review systematically analyzes the microstructure, composition, and physical properties of the ITZ, including porosity, microhardness, and hydration product distribution. Quantitative data from recent studies are highlighted—for instance, incorporating 3% nano-silica increased ITZ bond strength by 134.12% at 3 days and 108.54% at 28 days, while using 10% metakaolin enhanced 28-day compressive strength by 24.6% and reduced chloride diffusion by 81.9%. The review categorizes current ITZ enhancement strategies such as mineral admixtures, nanomaterials, surface coatings, and aggregate pretreatment methods, evaluating their mechanisms, effectiveness, and limitations. By identifying key trends and research gaps—particularly the lack of predictive models and standardized characterization methods—this review aims to synthesize key findings and identify knowledge gaps to support future material design in LWAC. Comparison of the Serodiagnostic Accuracy Tests for Lyme Disease in Adults and Children: A Network Meta-Analysis

As direct detection methods of Borrelia burgdorferi are limited, serology plays an important role in diagnosing Lyme disease (LD). There are various types of Lyme serological tests with varying diagnostic accuracy, so it is necessary to compare and rank them. The aim of this study is to compare the accuracy of various serological diagnostic methods for LD using network meta-analysis (NMA). We searched the Cochrane Library and PubMed databases for all serological diagnostic accuracy studies published from the discovery of LD until June 2024. After screening, we assessed the quality of the included studies with QUADAS-C and extracted relevant data. We calculated the Q* index of the receiver operating characteristic curve for each diagnostic test. Meta-disc 2.0 and Stata 15.0 were used to perform traditional meta-analysis and NMA with the gold standard (the comprehensive evaluation) as a reference. We then compared the Q* index values between different methods using two-by-two comparisons and ranked them accordingly. A total of 52 studies with 181,032 participants, including 5318 patients with LD, were included. These studies covered 14 diagnostic methods. The results of the NMA suggest that modified two-tiered testing (MTTT), C6 enzyme immunoassay (EIA), and standard two-tiered testing (STTT) rank in the top three among the 14 methods in terms of Q* index, with MTTT being the highest, followed by C6 EIA and STTT. MTTT and C6 EIA have higher overall diagnostic performance, and their accuracy is not inferior to that of the widely used STTT (PROSPERO CRD42022378326). Full article
