Special Issue Information

Dear Colleagues,

The growing complexity of chronic disease management in the 21st century calls for innovative approaches that place the individual at the centre of care while leveraging the potential of digital technologies. Digital health literacy—the ability to seek out, understand, and use digital health information—has become a critical factor in promoting self-care, adherence to treatment, and health outcomes.

We are pleased to invite you to contribute to this Special Issue, which will explore innovations in digital health literacy and person-centred care, focusing on their integration into chronic disease prevention, monitoring, and management. This topic is highly relevant to the scope of Healthcare, as it aligns with interdisciplinary approaches to public health, clinical practice, education, and policy.

For this Special Issue, submissions of original research articles and reviews are welcome. Research areas may include (but are not limited to) digital tools for health empowerment, patient–professional communication, eHealth interventions, mHealth, telemonitoring, educational strategies, equity in access to digital health resources, and innovative models for chronic disease management.

Contributions addressing integrated care pathways, digital self-management solutions, or person-centred strategies for long-term condition management are particularly encouraged.

We look forward to receiving your valuable contributions.
