Cover Story: This study evaluates recruitment strategies for a community-based, medically tailored nutrition intervention targeting underserved populations with early diabetes. A participant-focused, relationship-driven approach significantly outperformed registry-based cold-calling by fostering trust and demonstrating cultural and structural competency. However, even with strong community partnerships, social determinants of health and competing life demands, such as caregiving, transportation, and work, remained barriers to engagement. Findings suggest that while community-centric engagement strategies are critical, a multifaceted, adaptable recruitment strategy is essential to effectively reach and retain underrepresented groups in chronic disease prevention efforts.
