The underground mining industry, and some above ground operations, rely on the use of heavy equipment that articulates to navigate corners in the tight confines of the tunnels. Poor line of sight (LOS) has been identified as a problem for safe operation of this machinery. Proximity detection systems, such as a video system designed to provide a 360 degree view around the machine have been implemented to improve the available LOS for the operator. A four-camera system was modeled in a computer environment to assess LOS on a 3D cad model of a typical, articulated machine. When positioned without any articulation, the system is excellent at removing blind spots for a machine driving straight forward or backward in a straight tunnel. Further analysis reveals that when the machine articulates in a simulated corner section, some camera locations are no longer useful for improving LOS into the corner. In some cases, the operator has a superior view into the corner, when compared to the best available view from the camera. The work points to the need to integrate proximity detection systems at the design, build, and manufacture stage, and to consider proper policy and procedures that would address the gains and limits of the systems prior to implementation. In the present study, pilot simulations of the phenomena of blast wave and fireball generated by the rupture of a high-pressure (35 MPa) hydrogen tank (volume 72 L) due to fire were carried out. The computational fluid dynamics (CFD) model includes the realizable k-ε model for turbulence and the eddy dissipation model coupled with the one-step chemical reaction mechanism for combustion. The simulation results were compared with experimental data on a stand-alone hydrogen tank rupture in a bonfire test. The simulations provided insights into the interaction between the blast wave propagation and combustion process. The simulated blast wave decay is approximately identical to the experimental data concerning pressure at various distances. Fireball is first ignited at the ground level, which is considered to be due to stagnation flow conditions. Subsequently, the flame propagates toward the interface between hydrogen and air. The stake-holders in the quad-bike (QB) industry in Australia have failed to reach a satisfactory resolution of the present impasse that exists with respect to the causes and mitigation of the trauma suffered by riders due to QB instability. In an effort to provide purchasers with data enabling them to discriminate between safer and less safe machines, static longitudinal and lateral tests have been conducted by various interested parties; quasi-static lateral tests have also been conducted under some operational conditions. It is argued that while these static tests are valid, under many operating conditions QBs will not reach such unstable slopes due to poor traction. Further, these tests do not include the quasi-static and dynamic factors which also influence the processes associated with operational instability. For these reasons, the static tests do not provide an adequate basis for discrimination between safer and less safe machines. Full article

Prior to 2016, motorcycle licensing in Victoria, Australia, required off-road (range) skills testing only, focusing on vehicle-handling skills. The objective of this research was to develop an education and assessment curriculum commensurate with best practice that included on-road components and increased focus on awareness, judgment, and decision-making skills. No single best-practice curriculum was identified in the published literature. Therefore, to guide development of a new curriculum, a best-practice novice driver education framework, Goals for Driver Education, was adapted into the Goals for Rider Education framework. Applying Training Needs Analysis, the target population of learner motorcyclists was identified as largely male and aged under 30 years, with the target crash problem including a high proportion of single-vehicle loss-of-control crashes. Tailored content was developed based on exemplary Australian and international curricula, behaviour change theory, and adult learning principles; including transitioning from training to coaching and from testing to competency-based assessment. The result is Victoria’s new Motorcycle Graduated Licensing System (M-GLS) education and assessment curriculum, comprising three stages: pre-learner (Motorcycle Permit Assessment), learner (Check Ride), and pre-licence (Motorcycle Licence Assessment). Subject to potential refinements and on-going evaluation, this work lays the foundation for establishing a best-practice approach to novice motorcyclist education for licensure. Full article

Pressure differential systems have the purpose of maintaining tenable conditions in protected spaces for different types of building safe places, like escape routes, firefighting access routes, lobbies, stairwells and refuge areas. The aim of pressure differential systems is to establish airflow paths from protected spaces at high pressure to spaces at lower or ambient pressure, preventing the spread of toxic gas released during a fire. This strategy ought to be supported by a detailed design of the necessary air supply, considering also the cycle of opening and closing doors during the egress phase. The paper deals with the design of a simple pressure differential system intended to be used in a building as a pressurized smokeproof enclosure. Specifically, experimental tests and numerical modelling are conducted with the objective of characterizing the pressure evolution in a small compartment under different conditions and through a cycle of door opening. Experimental tests are conducted in a simple 3-m side cubic enclosure with two doors and no vent openings. While a centrifugal fan blows constant airflow inside the structure, the pressure trend in time is recorded during steady state and transient conditions; additionally, the velocity of the airflow across the doors has been measured by means of an anemometer. Numerical CFD (computational fluid dynamics) simulations are carried out to reproduce the same smokeproof enclosure configuration (both geometrical and boundary conditions) using the fire dynamics simulator (FDS). Furthermore, specific attention is paid to the modelling of the leakage across the doors, directly inserted in the model through a localized HVAC (heating and venting air conditioning) advanced leakage function. Comparisons between experimental tests and numerical simulations are provided. Once the model was correctly calibrated, other geometrical and mechanical configurations have been studied, looking for convenient and efficient positions of the fan in order to fulfill the requirements of the pressure differential, airflow velocity and door handle force. The paper highlights some fundamental aspects on the pressurization and depressurization during steady state and transient phases, trying to identify if there are airflow profiles typical of some geometrical configurations. One way to avoid detection of law enforcement officials if you are engaging in illegal activities is to wear a seatbelt. Therefore, an unintended consequence of laws allowing people to possess marijuana for medical purposes is that seatbelt use may decline among groups whose possession of marijuana is now legal. We find a decrease in seatbelt use among middle-aged males, providing evidence that drivers use seatbelts as a means to avoid police interaction. We find no such reduction in seatbelt use among those less likely to possess medical marijuana cards. Our evidence supports the contention that drivers use seatbelts more if they fear interaction with law enforcement officials, which is consistent with evidence of heightened seatbelt use among drunk drivers. These findings are important in understanding how to best design traffic safety laws and enforce them. Full article

Prior research on extrication of victims entrapped in stored grain has measured the vertical forces applied to a representative victim (i.e., mannequin) using an anchor point that is mounted directly overhead. However, primarily because of how most grain storage structures are designed, the forces applied would more likely be coming from an angle. This is due to the victim typically being entrapped in the center of the grain mass while the access point is at the perimeter of the roof. The objective of this study was to measure the peak forces required to pull a mannequin at an angle and compare it to vertical pull. An adult-size mannequin was pulled out of dry corn and soybean masses from various depths and at various angles. It was found that both corn and soybeans were comparable regardless of depth or angle with the exception of 15° angle. It was also found that, at pulling angles of 60° and 75°, the loads were comparable to those required at 90° or directly overhead. Only at the sharper angles of 15° and 30° did the peak forces significantly increase. The results highlight that there is some flexibility in the placement of the anchor point and that extrication forces on the body will increase at sharper pull angles. Full article

Inhalation of organic dusts in agricultural environments causes airway inflammatory diseases. Despite advances in understanding the airway response to dust-induced inflammation, less is known about the transition from lung injury to repair and recovery. The objective of this study was to define the post-inflammation homeostasis events following organic dust-induced lung injury. Using an established protocol, mice were intranasally treated with swine confinement facility organic dust extract (ODE) daily for 3 weeks (repetitive exposure) or treated daily with ODE for 3 weeks followed by no treatment for 1-4 weeks (recovery period) whereupon lavage fluid, lung tissue, and sera were processed. During recovery period, a significant decrease was observed in ODE-induced neutrophil levels after 1 week, lymphocytes at 2 weeks, and macrophages at 4 weeks in the lavage fluid. ODE-induced lung cellular aggregates and bronchiolar compartment inflammation were diminished, but persisted for 4 weeks post-injury. Alveolar inflammation resolved at 3 weeks. ODE-induced lung neutrophils were cleared by 3 weeks, B-cells by 2 weeks, and CD3+CD4+ and CD3+CD8+ T cells by 4 week recovery period. Amphiregulin levels increased post-ODE exposure to the 4 week recovery period. Ex vivo amphiregulin production was demonstrated in lung type 2 innate lymphoid cells and macrophages isolated from the 4 week recovery (post-ODE exposure) animals. Collectively, these results identify important processes during recovery period following agricultural dust-induced inflammation, and present possible strategies for improving lung repair and resolution. Although driver education (DE) is widely accepted as an effective teen driver safety measure and widely available in the United States, Canada and elsewhere, evaluations have generally failed to show that such formal programs actually produce safer drivers. To address the issue of safety effects as part of a larger investigation, two studies were conducted to examine whether the Oregon Department of Transportation (ODOT)-approved DE program was associated with reductions in collisions and convictions. In the first study, DE status among a relatively small sample of teens who completed an online survey was not found to have a significant effect on collisions and convictions. In the second study, of a much larger population of teen drivers, DE status was associated with a lower incidence of collisions and convictions. On balance, this suggests that the safety effects of DE are either neutral, based on the results of the first Oregon study, or cautiously optimistic based on the results of the second study. The implications of these findings are discussed in terms of making improvements in DE that are evidence-based, and the need for further evaluation to establish that improved and new programs meet their safety objectives. The research reported here sought to more fully understand the types and causative factors of injury/fatality incidents resulting from the rearward-movement of tractors and other self-propelled agricultural machinery, with the view that such findings might lead to the development, improvement, and/or better utilization of safety procedures, design principles, and technologies that would prevent—or at least markedly reduce—their occurrence. Thus, the scope of this study focused only on rearward-travel (not mechanical malfunction) incidents, and principally on agricultural equipment (although cases involving similar equipment in industrial or construction settings were also drawn upon). Applying these two criteria, a search of published and online sources uncovered more than 100 documented cases, 35 of which could clearly be identified as rearward-movement incidents, of which 28 (80%) were fatal. Each of these 35 cases were then assessed, based on the type of machine, type of worksite, and type/description of incident (i.e., ‘scenario’), which fell into one of three distinct categories or classifications—(1) co-worker run over/crushed/otherwise injured because operator loses visual contact with co-worker; (2) bystander run over/crushed/otherwise injured because operator is unaware of bystander’s presence; and (3) operator run over/crushed/otherwise injured because operator loses visual contact with, or is unaware of, a stationary object or a hazard. Then, from each scenario, a representative incident (i.e., case study) was selected for a more in-depth analysis. The collective findings, from these three case studies and all 35 machinery rearward-movement incidents, were as follows: (1) The ‘victim’ could be the machine operator as well as a co-worker or a bystander; (2) The specific site of the co-worker or bystander injury/fatality was at the base of the machine’s rear tires or tracks, at the hitching point, or behind a towed implement; (3) The specific cause was loss of visual contact between the operator and co-worker/bystander due to visual obstruction, the operator’s physical limitations, or the operator’s and/or bystander’s lack of alertness. To reduce the likelihood of future occurrences of agricultural machinery rearward travel-related incidents, preventive measures aimed at addressing the key causative factors for each scenario are offered. Building information modelling (BIM) enables the creation of a digital representation of a designed facility combined with additional information about the project attributes, performance criteria, and construction process. Users of BIM tools point to the ability to visualize the final design along with the construction process as a beneficial feature of using BIM. Knowing the construction process in relationship to a facility’s design benefits both safety professionals when planning worker safety measures for a project and designers when creating a project’s design. Success in using BIM to enhance safety partly depends on the familiarity of project personnel with BIM tools and the extent to which the tools can be used to identify and eliminate safety hazards. In a separate, ongoing study, the authors investigated the connection between BIM and safety to document the opportunities, barriers, and impacts. Utilizing an on-line survey of project engineers who work for construction firms together with a comprehensive literature review, the study found those who use BIM feel that it aids in communication of project information and project delivery, both of which have been found to have positive impacts on construction site safety. Further, utilizing the survey results, the authors apply the binary logistic regression econometric framework to better understand the factors that lead to safety professionals believing that BIM increases safety in the work place. In addition, according to the survey results, a large percentage of the engineers who use BIM feel that ultimately it helps to eliminate safety hazards and improve worker safety. The study findings suggest that improvements in safety performance across the construction industry may be due in part to increased use of BIM in the construction industry. Full article

Aims: Exposure to dusts/bioaerosols in concentrated animal feeding operations (CAFOs) results in inflammatory lung diseases in workers. Hog CAFOs dust extract (HDE) increases expression of intercellular adhesion molecule-1 (ICAM-1), neutrophil adhesion, and TNFα release in bronchial epithelial cells. Alcohol consumption is increasingly recognized to impair lung immunity. We hypothesized that alcohol impairs HDE-induced TNFα, ICAM-1 expression, and neutrophil adhesion by directly inhibiting TNFα converting enzyme (TACE) activity. Methods: Bronchial epithelial cells (BEAS-2B) and primary human bronchial epithelial cells were pretreated with ethanol (EtOH) or TACE inhibitor. ICAM-1 surface expression; TNFα release; and TACE activity were analyzed following HDE stimulation. The effect of alcohol and TACE inhibition on HDE-regulated epithelial cell/neutrophil adhesion interactions was investigated. Finally; utilizing an established animal model; C57BL/6 mice were fed ad libitum ethanol (20%) in drinking water for 8 weeks followed by daily intranasal inhalation of HDE or saline during the final two weeks. Mice were sacrificed and lung sections immunostained for ICAM-1. Results: Pretreatment with alcohol or TACE inhibitor significantly decreased HDE-induced ICAM-1 expression and TNFα release. HDE augmented neutrophil adhesion to epithelial cells, which was decreased with alcohol (32% decrease) or TACE inhibitor (55% decrease) pretreatment. TACE activity increased following HDE exposure, but TACE activity was inhibited following alcohol pretreatment. Alcohol-fed mice demonstrated decreased HDE-induced airway epithelium ICAM-1 expression. Conclusions: Alcohol diminishes HDE-induced ICAM-1 expression, TNFα release, and neutrophil adhesion via inhibition of TACE activity. These results suggest that alcohol may be an important modulator of lung innate immune responses following CAFO exposure. Full article

The US Department of Labor (US DOL) oversees the Agricultural Hazardous Occupations Orders (AgHOs), which identifies specific tasks that youth are prohibited from performing for hire on American farms and ranches. An educational exemption from this public policy is currently in place that allows youth, 14–15 years old, to complete a certification program prior to engaging in agricultural work involving tractors and machinery. However, limited guidance is provided in the legislation regarding the format or content of the tractor and machinery certification exemption. Four AgHOs (tractor and machinery) studies were identified and included in this meta-analysis publication. The research goals of this analysis were to determine basic trends of learning outcomes, and identify educational content to be delivered as a result of these studies. Within each of the four studies, expert panels were used to identify educational learning outcomes. The analysis revealed that 48.0% (n = 184) of all learning outcome items fell within the Tractor-based (Tractor) learning outcome category, 29.8% (n = 114) within General Safety and Health (General), and 22.2% (n = 85) of items in the Machinery-based (Machinery) category. Ultimately, sound educational methods and understanding of fundamental student competence are essential components for any training program, including youth who complete AgHOs tractor and machinery certification programs. As farm demographics change, it is important to create relatable, research-based safety and health materials, and deliver information via preferable methods. Three data collection methods (focus groups, surveys via National Agricultural Statistics Service, and face-to-face interviews) were administered to farmers in seven Midwestern states. Farmers were coded as retired, organic, residential/lifestyle, limited resource (<$249 K gross annual income), or large resource (>$250 K gross annual income). Issues addressed hazard perceptions, injury prevention measures, resource preferences and delivery, and preferred communication channels. Findings indicated that gender, age, and farming operation influence Personal Protective Equipment (PPE) use and communication preferences. Retired farmers and those with a higher percentage of income from farming are more likely to use radio as a communication medium. Research results will inform the creation of web-based, customizable educational materials for use by safety and health professionals and the public. Audio Public Service Announcements (PSAs) were recorded to reach farmers who indicated a preference for radio. Full article

In this paper the results from a field experiment (conducted in 2012) are presented, in which 3 regional police units in the Flemish region of Belgium each tested a particular combination of a speed control (with mobile radar in an anonymous car) with communicative signage. The goal of this paper was to scientifically evaluate frequently used field set-ups. The following set-ups were tested in one week: (1) police unit 1: speed control with and without a static feedback sign placed after the control; (2) police unit 2: speed control with and without a digital feedback sign placed after the control; (3) police unit 3: speed display followed by a second speed display further along the road section. During certain time frames, speed control took place in between both signs. All tested field set-ups generated significant reductions in the speed level. Studying the effect of the different variations for each set-up reveals that the effect of the speed control is generally larger in combination with the signage alongside the road. After the period during which the police activities took place, speed levels again reached their initial level. Based on the before–after analysis, only in police area 2 was a small ‘time halo-effect’ found. To generalize results from this pilot study, repetition of tested set-ups in the field is recommended as well as testing on a larger scale. Full article

An increasing number of countries are requiring an extended learner permit prior to independent driving. The question of when drivers begin the learner permit period, and how long they hold the permit before advancing to independent licensure has received little research attention. Licensure timing is likely to be related to “push” and “pull” factors which may encourage or inhibit the process. To examine this question, we recruited a sample of 90 novice drivers (49 females and 41 males, average age of 15.6 years) soon after they obtained a learner permit and instrumented their vehicles to collect a range of driving data. Participants completed a series of surveys at recruitment related to factors that may influence licensure timing. Two distinct findings emerged from the time-to-event analysis that tested these push and pull factors in relation to licensure timing. The first can be conceptualized as teens’ motivation to drive (push), reflected in a younger age when obtaining a learner permit and extensive pre-permit driving experience. The second finding was teens’ perceptions of their parents’ knowledge of their activities (pull); a proxy for a parents’ attentiveness to their teens’ lives. Teens who reported higher levels of their parents’ knowledge of their activities took longer to advance to independent driving. These findings suggest time-to-licensure may be related to teens’ internal motivation to drive, and the ability of parents to facilitate or impede early licensure. Full article

We conducted individual and ecologic analyses of prospectively collected data from 839 injured bicyclists who collided with motorized vehicles and presented to Bellevue Hospital, an urban Level-1 trauma center in New York City, from December 2008 to August 2014. Variables included demographics, scene information, rider behaviors, bicycle route availability, and whether the collision occurred before the road segment was converted to a bicycle route. We used negative binomial modeling to assess the risk of injury occurrence following bicycle path or lane implementation. We dichotomized U.S. National Trauma Data Bank Injury Severity Scores (ISS) into none/mild (0–8) versus moderate, severe, or critical (>8) and used adjusted multivariable logistic regression to model the association of ISS with collision proximity to sharrows (i.e., bicycle lanes designated for sharing with cars), painted bicycle lanes, or physically protected paths. Negative binomial modeling of monthly counts, while adjusting for pedestrian activity, revealed that physically protected paths were associated with 23% fewer injuries. Painted bicycle lanes reduced injury risk by nearly 90% (IDR 0.09, 95% CI 0.02–0.33). Holding all else equal, compared to no bicycle route, a bicycle injury nearby sharrows was nearly twice as likely to be moderate, severe, or critical (adjusted odds ratio 1.94; 95% confidence interval (CI) 0.91–4.15). Painted bicycle lanes and physically protected paths were 1.52 (95% CI 0.85–2.71) and 1.66 (95% CI 0.85–3.22) times as likely to be associated with more than mild injury respectively. Full article

Hog production in the United States is a large industry that has seen dramatic changes over the last few decades. Concentrated animal feeding operations (CAFOs) are growing in number throughout the country. This pilot study explores the perception of risk, receipt of work-related training, provision and usage of personal protective equipment (PPE), and prevention preferences of Latino immigrant hog CAFO workers in Missouri. Forty workers (M age = 36.08 years, SD = 10.04; 92.5% male; 70.0% Mexican) were interviewed. Results indicate that most workers did not perceive their job as dangerous. Limited English proficient workers were significantly less likely to report receiving any work-related training. Although most workers had access to employer provided PPE, usage was inconsistent. As the demographic composition of the farmworker population in the Midwest becomes increasingly comprised of hired immigrant workers, it will be imperative to develop occupational safety and health educational and outreach efforts focused on the needs of these workers. Purpose: To review primary literature on ocular and orbital injury secondary to all-terrain vehicles (ATVs). Methods: A Medline search of English language literature. Results: Very few studies detail ocular and orbital manifestations of ATV crashes. The most common ocular injuries included orbital fractures and eyelid lacerations. Less common but more severe injuries included traumatic optic neuropathy, retinal detachment, optic nerve laceration, and ruptured globe. Associated facial and skull bone fractures, traumatic brain injury, and subdural/subarachnoid hemorrhage were not uncommon. Depending on the mechanism and force of injury, complete loss of vision has been documented. Conclusion: Ocular and orbital trauma can be found in many cases of ATV-related injury. Various interventions may decrease the frequency of such injuries, including use of head and eye protection. Full article

Agriculture is one of the most dangerous industries in the United States. Farmworkers, including migrant farmworkers, are at risk for work-related injuries. This study explores the association between stress, depression, and occupational injury among migrant farmworkers in Nebraska. Occupational injury was hypothesized to significantly increase the odds of farmworkers being stressed and depressed. Two hundred migrant farmworkers (mean age = 33.5 years, standard deviation (SD) = 12.53; 93.0% men, 92.9% of Mexican descent) were interviewed. In bivariate analyses, results indicated that stress and depression were positively associated with occupational injury. Two logistic regression models were developed. Occupational injury was a significant factor for depression, but not for stress. Participants who had been injured on the job were over seven times more likely to be depressed. These results highlight the interconnection between the work environment and mental health. More must be done to foster well-being in rural, agricultural communities. Improving occupational health and safety information and training, integrating behavioral health services into primary care settings, and strengthening the protections of the Migrant and Seasonal Agricultural Worker Protection Act may improve conditions for migrant farmworkers in the rural Midwest. Stress can have serious implications on road safety and evidence suggests that it could lead to increases in driving errors, lapses, and even crashes. Motorcyclists are a vulnerable road user group, and lapses in attention and risky behaviours resulting from stress could increase the risk of collision. However, few safety interventions for reducing stress have been developed and evaluated, especially in motorcyclists. The purpose of this research was to develop and pilot a Cognitive Behavioural Therapy (CBT) course for the treatment of rider stress. Five motorcyclists experiencing a range of life and work stressors completed the CBT course between January and March 2015. Findings from the Driver Stress Inventory and Driver Behaviour Questionnaire showed positive trends in the overall reduction of rider stress traits, such as aggression, thrill seeking, and dislike of riding. Qualitative data showed that participants engaged well with the intervention and believed it had aided them in their riding-related stress. Although these results are promising, the results warrant further investigation in order to validate CBT as a viable means of reducing the collision risk both for this already vulnerable road user group and other driver categories. The Pike River mine (PRM), an underground coal mine in New Zealand (NZ), exploded in 2010. This paper analyses the causes of the disaster, with a particular focus on the systems engineering and organisational contributions. Poor systems-engineering contributed via poorly designed ventilation, use of power-electronics underground, and placement of the main ventilation fan underground. Management rushed prematurely into production even though the technology development in the mine was incomplete. Investment in non-productive infrastructure was deprioritised resulting in inadequate ventilation, and the lack of a viable second emergency egress. The risk assessments were deficient, incomplete, or not actioned. Warnings and feedback from staff were ignored. Risk arises as a consequence of the complex interactions between the components of the sociotechnical system. Organisations will need to strengthen the integrity of their risk management processes at engineering, management, and board levels. The systems engineering perspective shows the interacting causality between the engineering challenges (ventilation, mining method, electrical power), project deliverables, management priorities, organisational culture, and workers’ behaviour. Use of the barrier method provides a new way to examine the risk-management strategies of the mine. The breakdowns in organisational safety management systems are explicitly identified. Full article

The young driver problem is typified by high crash rates early in licensure that decline with experience, but are higher initially and decline more slowly for the youngest novices. Despite considerable effort, only Graduated Driver Licensing System (GDLS) policies have been shown to improve novice young driver safety outcomes. Unfortunately, GDLS policies are mostly limited to countries with a relatively young licensure age. Meanwhile, it is not entirely clear how GDLS and other young driver transportation safety efforts, including driver training and testing, supervised practice and parental management of young drivers, can best be configured. Notably, professional training can foster improvements in vehicle management skills that are necessary, but do not assure safe driving behavior. Substantial recent research has focused on training methods to improve driving skills, but the safety benefits of driver training have not been established. While prolonged practice driving increases experience and provides supervisors with opportunities to prepare novices for independent driving, the transition to independent driving challenges novices to employ, on their own, poorly-mastered skills under unfamiliar and complex driving conditions. Licensing policies and parental management practices can limit the complexity of driving conditions while novices gain needed driving experience. Nevertheless, an emerging body of literature suggests that future advances in training and supervision of novice teenage drivers might best focus on the translation of learning to independent driving by fostering safe driving attitudes and norms, judgment, dedicated attention to driving tasks and self-control at the wheel. This study describes chores when farmers were either fatally or seriously injured and required emergency medical treatment as a result of overturns of tractors with or without rollover protective structures (ROPS). Data from the 2002 Kentucky Farm Tractor Overturn Survey were used for this study. The data were collected by a telephone survey of a population-based random sample of 6063 (7.98%) of Kentucky’s 76,017 farm operators as listed in the Kentucky Agricultural Statistics Service database. Of farm operators interviewed, 551 (9.1%) reported 603 overturns and 5512 (90.9%) reported no overturns in the history of their farm, covering a period from 1925 to February 2002. Only the latest overturn was considered to improve recall accuracy. In addition, since the 1925 to 1959 time period had only 49 (8.1%) of the overturns reported, (14 farmers did not provide the year of most recent overturn); only data from the 1960 to 2002 period (approximately 41 years) were used. After making these adjustments, incidents evaluated included 25 cases (one fatal and four serious nonfatal injuries) that involved ROPS-equipped tractor overturns and 88 cases (24 fatal and 64 serious nonfatal injuries) that involved non-ROPS tractor overturns. Chores at highest risk for tractor overturns were identified for which educational and ROPS retrofit interventions could be emphasized. The highest frequency of overturn-related fatalities and nonfatal injuries were associated with hay harvesting, rotary mowing, and on-farm travel chores. These three chores represented 68.2% of fatal events and 50.0% of permanent and 56.6% of temporary disability overturn incidents. Tragically, in countries such as India and China with emerging mechanization, a large majority of tractors are produced without ROPS that can be expected to result in the same overturn-related epidemic of deaths experienced in highly mechanized countries, despite evidence of the protection provided by ROPS. Currently, mainly aggregated statistics are used for bicycle crash risk calculations. Thus, the understanding of spatial patterns at local scale levels remains vague. Using an agent-based flow model and a bicycle crash database covering 10 continuous years of observation allows us to calculate and map the crash risk on various spatial scales for the city of Salzburg (Austria). In doing so, we directly account for the spatial heterogeneity of crash occurrences. Additionally, we provide a measure for the statistical robustness on the level of single reference units and consider modifiable areal unit problem (MAUP) effects in our analysis. This study is the first of its kind. The results facilitate a better understanding of spatial patterns of bicycle crash rates on the local scale. This is especially important for cities that strive to improve the safety situation for bicyclists in order to address prevailing safety concerns that keep people from using the bicycle as a utilitarian mode of (urban) transport. This Commentary addresses the ongoing disagreements between many safety advocates who endorse traditional models of prevention and those who oppose them, arguing that safety measures are offset by risk compensation (RCT). The debate is especially heated with respect to regulatory or legislative prevention measures. After explaining the rationale behind risk compensation (aka risk homeostasis theory) (RHT), I provide examples of RCT studies to explain why I believe they should be rejected. The main basis for my rebuttal, however, rests on data that show steady declines in unintentional injury mortality, which, according to RCT, should not have occurred. There are many other reasons for rejecting this theory, and it seems that the time has come for the debate to finally be concluded. Full article

All-terrain vehicles (ATVs) have become increasing popular in many countries around the world, both for occupational use, as well as recreational use. With an increase in popularity, and the supply of heavier and more powerful machines on the market, major traumas and deaths from ATV use are growing concerns for public health and injury prevention professionals. This review of the literature on ATVs will focus on the mechanism and patterns of ATV-related injuries, the challenges of injury prevention, and the effects of legislation and regulations regarding ATV usage. The increasing burden of injuries and the substantial economic cost from ATV-related traumas and deaths calls for intensification of injury prevention efforts. Modification of risk factors, institution of regulations and legislation, and enforcement of those rules are important steps for prevention of ATV-related harm. Full article

Therefore, alternative bottom-up simulation modelling approaches are expected to complement traditional equation-based models. In this paper we present an agent-based approach to explore bicycle-bicycle accidents. Specifically, we hypothesise that (1) bicycle-bicycle accidents are based on the population of encounters between cyclists rather than on bicycle flows and (2) that encounters have a non-linear relationship with flows. Bicycle flows and encounters are simulated by means of an agent-based model that is implemented for the road network of the city of Salzburg. Simulation results are tested against a 10-year dataset of police records on bicycle-bicycle accidents. The results affirm both hypotheses: First, cyclist encounters exhibit a linear relationship to accidents and thus suggest being the true population of bicycle-bicycle accidents. Second, flows show a relationship in the form of a second-order polynomial function with encounters as well as accidents. Full article

Safety inspectorates have been established for all modes of transport in Norway. This paper explores whether establishing a safety inspectorate is related to safety performance. Long-term trends in safety in aviation and rail were compared before and after safety inspectorates were established for these modes of transport. In aviation, there have been no passenger fatalities after the safety inspectorate was established. The number of (non-fatal) accidents in scheduled and charter flights has been between zero and five per year, which is higher than predicted according to the long-term trend in the period before the safety inspectorate was created. In rail, both the number of accidents and the number of passenger fatalities have been lower after the creation of the safety inspectorate than predicted according to long-term trends in the before-period. The paper shows statistical relationships indicating that safety performance has improved, at least in rail transport. A causal interpretation of these relationships is not possible on the basis of the analyses in this paper. Establishing safety inspectorates may improve transport safety, but showing this rigorously is extremely difficult. Full article

Quads are the leading cause of fatal non-intentional injuries on Australian farms. Due to normal age-related physiological and cognitive changes, farmers 50-years of age and above are at increased risk when using quads. This study identifies a non-statistically significant increasing trend for fatal quad incidents involving this cohort in Australia. It is contended that these vehicles are not “fit-for-purpose” for many typical agricultural tasks more broadly and that the ageing process further exacerbates these risks. Encouraging and promoting the use of more “fit-for-purpose” vehicles in the agricultural sector should be the primary focus of intervention approaches. Supplementing this, other approaches that reduce risks, specifically relating to rollovers, crush/asphyxiation and head injuries must be enacted. Full article

Introduction: Since 2006, the number of seriously injured bicyclists in The Netherlands has increased significantly. This is also the case for sport cyclists. Over 80% of sport cyclists are male. We propose three factors that may contribute to involvement in sport cycling crashes among males: Bunch riding (cycling in a group), the inflow of sport cyclists with little experience and a competitive attitude. Methods: Early 2014, a questionnaire was sent to 2625 members of the Dutch Tour Cycling Union to obtain data on involvement in sport cycling crashes in the year 2013 and possible contributing factors (e.g., bunch riding, experience, competitiveness, distance travelled). Binary logistic regression analysis was applied to compare data from male respondents (N = 744). Contrast was made between those who reported involvement in a crash (N = 313) and those who did not (N = 431). Results: Male sport cyclists who are involved in bunch riding and those who are relatively inexperienced (less than three years compared to more than 10 years) have a higher crash involvement (Odds Ratio (OR) = 1.79; 95% Confidence Interval (CI) = 1.26 – 2.54) and (OR = 2.93; 95% CI = 1.42 – 6.06) regardless of age, annual distance travelled and competitive attitude. Annual distance travelled was not related to crash involvement over the year 2013, indicating that cyclists who travel a longer annual distance have a lower risk (persons involved in at least one crash per km). Conclusions: We recommend that the efficacy of bunch riding training interventions among males is evaluated, with the focus on promoting safety among inexperienced sport cyclists and bunch riding. Side-by-side (SxS) vehicles have become increasingly popular, but there are few reports on injury epidemiology. Newspaper reports of SxS and all-terrain vehicle (ATV) crashes were analyzed for nine U.S. states from 2009 to 2011, including comparisons between the two vehicle types. Seventy-nine SxS crashes involving 104 injured victims were identified; three-fourths were males. There was a relatively high percentage of injured passengers (37%), and a higher proportion of female victims were passengers as compared to males (p = 0.015). Children <16 years of age were 44% of those injured and had the highest proportion of both passenger and operator victims as compared to other age groups. Over half of the crashes occurred on roadways; nearly two-fifths occurred at night. As compared to adults, a lower percentage of crashes involving youth were at night (p = 0.0037) but the percentages on roadways were similar. Only one in five roadway SxS crashes involved a collision with a motorized vehicle. Rollovers were the most common mechanism (50%). Two-thirds of victims were ejected, and one-half were struck or pinned by the vehicle. Twenty-eight deaths (27%) were reported. Although most current SxSs have roll bars, lack of safety belt use has likely reduced their benefit. Children should be prohibited from operating SxSs. Full article

The use of dedicated components as a means of achieving desirable levels of fault tolerance in a system may result in high costs. A cost effective way of restoring failed functions is to use heterogeneous redundancies: components that, besides performing their primary intended design function, can also restore compatible functions of other components. In this paper, we apply a novel design methodology called D3H2 (aDaptive Dependable Design for systems with Homogeneous and Heterogeneous redundancies) to assist in the systematic identification of heterogeneous redundancies, the design of hardware/software architectures including fault detection and reconfiguration, and the systematic dependability and cost assessments of the system. D3H2 integrates parameter uncertainty and criticality analyses to model inexact failure data in dependability assessment. The application to a railway case study is presented with a focus on analysing different reconfiguration strategies as well as types and levels of redundancies. Full article

Researchers have a long history in the conduct of evaluations of road safety countermeasures. However, despite the strengths of some evaluative road safety evaluations that align with previous and current thinking on program evaluation, few published road safety evaluations have followed standard conceptualization and methodology outlined in numerous program evaluation textbooks, journal articles and Web-based handbooks. However, conceptual and methodological challenges inherent in many evaluations of road safety countermeasures can affect causal attribution. Valid determination of causal attribution is enhanced by use of relevant theory or hypotheses on the putative mechanisms or pathways of change and by the use of a process evaluation to assess the actual implementation process. This article provides a detailed description of the constructs of causal chain, program logic models and process evaluation. This article provides an example of how these standard methods of theory-driven evaluation can improve the interpretation of outcomes and enhance causal attribution of a road safety countermeasure. Background and Aims: Considerable variation has been documented with fleet safety interventions’ abilities to create lasting behavioural change, and research has neglected to consider employees’ perceptions regarding the effectiveness of fleet interventions. This is a critical oversight as employees’ beliefs and acceptance levels (as well as the perceived organisational commitment to safety) can ultimately influence levels of effectiveness, and this study aimed to examine such perceptions in Australian fleet settings. Method: 679 employees sourced from four Australian organisations completed a safety climate questionnaire as well as provided perspectives about the effectiveness of 35 different safety initiatives. Results: Countermeasures that were perceived as most effective were a mix of human and engineering-based approaches: (a) purchasing safer vehicles; (b) investigating serious vehicle incidents; and (c) practical driver skills training. In contrast, least effective countermeasures were considered to be: (a) signing a promise card; (b) advertising a company’s phone number on the back of cars for complaints and compliments; and (c) communicating cost benefits of road safety to employees. No significant differences in employee perceptions were identified based on age, gender, employees’ self-reported crash involvement or employees’ self-reported traffic infringement history. Perceptions of safety climate were identified to be “moderate” but were not linked to self-reported crash or traffic infringement history. However, higher levels of safety climate were positively correlated with perceived effectiveness of some interventions. Conclusion: Taken together, employees believed occupational road safety risks could best be managed by the employer by implementing a combination of engineering and human resource initiatives to enhance road safety. This paper will further outline the key findings in regards to practice as well as provide direction for future research. Full article

This study identified the areas of poor specificity in national injury hospitalization data and the areas of improvement and deterioration in specificity over time. A descriptive analysis of 10 years of national hospital discharge data for Australia from July 2002–June 2012 was performed. Proportions and percentage change of defined/undefined codes over time was examined. At the intent block level, accidents and assault were the most poorly defined with over 11% undefined in each block. The mechanism blocks for accidents showed a significant deterioration in specificity over time with up to 20% more undefined codes in some mechanisms. Place and activity were poorly defined at the broad block level (43% and 72% undefined respectively). Private hospitals and hospitals in very remote locations recorded the highest proportion of undefined codes. Those aged over 60 years and females had the higher proportion of undefined code usage. This study has identified significant, and worsening, deficiencies in the specificity of coded injury data in several areas. Focal attention is needed to improve the quality of injury data, especially on those identified in this study, to provide the evidence base needed to address the significant burden of injury in the Australian community. An epidemic exists related to all-terrain vehicle (ATV) fatal and nonfatal injuries in the United States and in Australia as well as in other countries. More than 60% of these injuries are associated with ATV overturns. While behavior-related methods have failed to abate this epidemic, engineered interventions have been lacking. However, one technology, the Quadbar™ (QB), shows promise as a crush prevention device for reducing overturn-related injuries and their severity. The question addressed in this study is “What is the potential public health and economic benefit of the QB when used on ATVs?” At 40% effectiveness, our model estimated that 5082 injuries related to ATV overturns would be prevented per 100,000 ATVs equipped with QBs. The overall societal economic benefit was estimated at US$3,943 per ATV that greatly exceeds the QB purchase price of US$478, which represents an 8:1 return on investment. Full article

A total of 10,561 all-terrain vehicle (ATV) related deaths have been documented for the years 1985 through 2009 in the United States, most of which were associated with overturns of the machine. The current analysis addresses the question, “How effective is the QuadbarTM (QB) as a crush prevention device (CPD) in preventing ATV overturn-related injuries?” A CPD is designed as a guard against crushing injuries to the ATV rider in the event of an overturn. The analysis used a prevention effectiveness model to address this question. Based on this analysis, the CPD and more specifically the QB were found to potentially prevent serious injuries and death to ATV riders that result from overturns. Systematic real-life studies are needed to evaluate the prevention potential of CPDs that are in use to guide the implementation of policies to better protect the public from these injuries. Traditional all-terrain vehicles (ATVs) are designed for single riders. Although carrying passengers is a known risk factor for injury, how passengers contribute to ATV crashes remains poorly understood. To address this question, we performed a retrospective chart review of ATV crash victims at a U.S. trauma center (2002–2013). Of 537 cases, 20% were passengers or drivers with passengers. The odds of backward rollovers, falls/ejections, crashes on sloped terrain, and collisions with motorized vehicles were all significantly greater when passengers were present. In contrast, the odds of self-ejection or falls/ejections over the handlebars were significantly lower than falls/ejections to the side or rear, in crashes with multiple riders. Among all ejections, self-ejections had the lowest head and highest extremity injury scores and being ejected over the handlebars or to the rear resulted in worse head injury scores than being ejected to the side. In summary, our study found that passengers increased the odds of specific crash and injury mechanisms and that head and extremity injury severity varied by ejection type. Safety interventions including seat design changes that prevent carrying passengers, and a strict, well-enforced no-rider rule are needed to effectively prevent passenger–related deaths and injuries. Left-turn crashes occur frequently at signalized intersections and often lead to severe injuries. This problem can be addressed through the implementation of protected left-turn signals. This gives left-turning vehicles the right to enter the intersection free from conflict with opposing drivers and pedestrians. The present study analyses the effect of this measure on crash occurrence. The study included 103 signalized intersections with left-turn signals in highways in Flanders, Belgium, of which 33 received only changes in the signal control and 70 also received additional changes. The effect on traffic safety is analyzed through an Empirical Bayesian before-and-after study on crashes, in which general trend effects and regression-to-the-mean are controlled. On the 33 intersections that received only changes in the signal control, the number of injury crashes decreased significantly (−46%, 95% CI (−36%; −55%)) during the after-period. This was mainly attributable to a decrease in left-turn crashes: −60%, 95% CI (−39%; −74%). The number of rear-end injury crashes did not change significantly after the implementation of a protected left-turn signal. A larger effect was identified for more severe crashes (involving serious injuries and fatalities) as compared with crashes resulting in lighter injuries: −66%, 95% CI (43%; −80%). Furthermore, the effect of left-turn phasing on the number of injured car occupants, cyclists, moped riders and motor cyclists was examined, and favorable effects were found for each of these groups. Use of all-terrain vehicles (ATVs) has become standard practice on the modern ranch. The unique operating conditions present on a ranch, subject the occupational ATV user to hazards requiring awareness and specialized training. The purpose of this study was to apply social marketing methods to address a specific environmental health and safety issue present in the agricultural industry. A series of four ATV tip sheets were created in topic areas specific to the challenges that ATV operators encounter on a ranch. In order to evaluate the intended audiences’ perception of the tip sheets, a questionnaire was administered to all agriculture operators and producers throughout McCone County, Montana, USA. Questionnaire responses indicated that the tip sheets contained quality information and were relevant to the occupational hazards present when using ATVs for agricultural purposes. Future work should focus on the dissemination of this information and continued emphasis on industry specific training for ATV operators. Full article

Whole body vibration (WBV) exposure is recognised as a risk factor to the high prevalence of spinal musculoskeletal disorders (MSDs) experienced by farmers. The purpose of this study was to identify self-reported predictors that could be used to develop statistical models for WBV exposure (expressed as A8rms and VDV) in farmers operating agricultural quad bikes. Data were collected in the field from 130 farmers. Linear mixed effects modeling was used to determine the models of best fit. The prediction model for A8rms exposure (explaining 57% of the variance) included farmer age, estimated quad bike driving hours on day of testing and the type of quad bike rear suspension (rigid-axle rear suspension with two shock absorbers). The best model for VDV exposure (explaining 33% of the variance) included farmer age, estimated quad bike driving hours on day of testing and the type of quad bike rear suspension (rigid-axle rear suspension with two shock absorbers). The objective of this study was to analyze workers’ compensation injury and fatality data associated with the occupational use of all-terrain vehicles (ATVs) in Montana. Data were provided by the Montana Department of Labor and Industry Workers’ Compensation Injury and Occupational Disease Database. Claims were identified based on a search of injury codes related to vehicular claims and then narrowed by a keyword search for events related to ATVs. Two hundred and fifteen ATV-related claims were identified between 2007 and 2012. The majority of claimants were identified as male (85%), with 23% of total claims between the ages 20–29 at the time of injury. The agriculture industry accounted for 59% of all claims. The results of this investigation indicated that the cost of occupational ATV injuries and deaths during the study period totaled nearly $2,600,000. The agriculture industry is disproportionally represented in ATV workers’ compensation claims in Montana. Characterizing and understanding the risk factors associated with occupational-related ATV injuries is critical for developing strategies and programs aimed at injury prevention. Evaluating the gaps in data acquisition and reporting could aid in ensuring comprehensive and complete future investigations of ATV incidents. Full article

On the basis of the analyses of past cases, we demonstrate how cognitive biases are ubiquitous in the process of incidents, crashes, collisions or disasters, as well as how they distort decision making and lead to undesirable outcomes. Five case studies were considered: a fire outbreak during cooking using an induction heating (IH) cooker, the KLM Flight 4805 crash, the Challenger space shuttle disaster, the collision between the Japanese Aegis-equipped destroyer “Atago” and a fishing boat and the Three Mile Island nuclear power plant meltdown. We demonstrate that heuristic-based biases, such as confirmation bias, groupthink and social loafing, overconfidence-based biases, such as the illusion of plan and control, and optimistic bias; framing biases majorly contributed to distorted decision making and eventually became the main cause of the incident, crash, collision or disaster. Therefore, we concluded that, in addition to human factors or ergonomics approaches, recognition and elimination of cognitive biases is indispensable for preventing incidents, crashes, collisions or disasters from occurring. Skitching is the act of hitching a ride on a vehicle while riding/using a non-motorized wheeled device (e.g., skateboard or bicycle). To date there has been little discussion of skitching beyond media reports on the serious and often fatal ramification of this activity. To rectify this and improve our understanding of skitching including: who participates; circumstances and motivation; and possible injury prevention strategies, informed by the Haddon’s Matrix, an integrative review was undertaken. To gain a comprehensive overview, the review encapsulated information from a variety of sources including peer reviewed literature, grey and popular internet sources including news and social media. There was an absence of literature from which strong conclusions could be made; however, some preliminary insights were obtained. A key participant group is young males, likely a function of their use of non-motorized wheeled devices, adolescent risk taking and the influence of peers, such that the behavior amongst this group is largely thought to be opportunistic. A number of prevention strategies are proposed including targeting young males and young drivers, provision of/retrofitting skate parks, educating young drivers and improving helmet use. There is also a need to incorporate coding into injury data collections to capture skitching. Full article

Background: Younger and older pedestrians are both overrepresented in train-pedestrian injury and fatality collision databases. However, scant research has attempted to determine the factors that influence level crossing behaviours for these high risk groups. Method: Five focus groups were undertaken with a total of 27 younger and 17 older pedestrian level crossing users (N = 44). Due to the lack of research in the area, a focus group methodology was implemented to gain a deeper exploratory understanding into the sample’s decision making processes through a pilot study. The three main areas of enquiry were identifying the: (a) primary reasons for unsafe behaviour; (b) factors that deter this behaviour and (c) proposed interventions to improve pedestrian safety at level crossings in the future. Results: Common themes to emerge from both groups regarding the origins of unsafe behaviours were: running late and a fatalistic perspective that some accidents are inevitable. However, younger pedestrians were more likely to report motivators to be: (a) non-perception of danger; (b) impulsive risk taking; and (c) inattention. In contrast, older pedestrians reported their decisions to cross are influenced by mobility issues and sensory salience. Conclusion: The findings indicate that a range of factors influence pedestrian crossing behaviours. This paper will further outline the major findings of the research in regards to intervention development and future research direction. Full article
