        <p>Religious thought and spirituality can be considered as a part of natural human capacities. There is an exponential rise in clinical research in the relationship between religion, spirituality and positive health outcomes. Most of these studies, however, have been primarily descriptive, rather than explanatory, focusing on identifying their underlying mechanisms. Almost no attempts have been made to find novel methods to mirror and monitor positive, and possibly negative, reactions related to the local and general effects of religion and spirituality in healthy subjects and patients. As this area of interest is rather new, we propose to put forward a new hypothesis that effects of religion and spirituality can be objectively studied by various exhaled biomarkers, some of which have already been developed and tested in health and disease. The lungs are particularly well suited for this purpose, as we have easy access to exhaled air and thereby a possibility to develop methods that measure compounds directly released from them. This work is the first step in the convergence of medical and theological research by linking various biomarkers and physiological measures with indicators of individual belief systems, religiosity and spirituality.</p>

      <title>1. Introduction</title>
      <p>Research within the field of religion and spirituality in health and disease is expanding rapidly. It has been shown that religiosity is an effective coping resource for people with health related problems, and it has been correlated with better health outcomes. Considering the consistency of the findings and the diversity of the methodological approaches used, it is unlikely that the salutary effects of religiosity and spirituality are solely attributable to chance or biased methods.</p>
      <p>However the processes by which religiosity and spirituality are utilized and their effects on various biochemical, inflammatory and immune parameters of both healthy individuals and patients have not been adequately explored. This may be the reason that not all of the evidence is conclusive. Thus, the efficacy of religious practice relative to health outcomes, with religiosity measured as religious attendance in the subset of 27 studies has been inconclusive at best [<xref ref-type="bibr" rid="B1-religions-03-00467">1</xref>,<xref ref-type="bibr" rid="B2-religions-03-00467">2</xref>]. Almost no attempts have been made to find novel methods to mirror and monitor positive, and possibly negative, reactions related to the local and general effects of religion and spirituality in healthy subjects and patients.</p>
      <p>As this area of interest is rather new, we propose to put forward a new hypothesis that the effects of religion and spirituality can be objectively studied by various exhaled biomarkers, some of which have already been developed and tested in healthy individuals and patients.     <sec sec-type="results">
      <title>2. Results and Discussion</title>
      <sec>
        <title>2.1. Rationale for New Category of Religious and Spiritual Biomarkers</title>
        <p>The current focus on the relationship of religion and spirituality with healing is a remarkable and unanticipated current developed within academic medicine [<xref ref-type="bibr" rid="B3-religions-03-00467">3</xref>].</p>
        <p>Most medical schools in the 1990s dropped religious and spiritual dimension from their vocabulary. This was partially due to an increasing emphasis on the scientific basis for medical practice, e.g. “evidence-based” medicine. This inattention inadvertently undermined the medical student's own spiritual growth [<xref ref-type="bibr" rid="B4-religions-03-00467">4</xref>], and it certainly inhibited the development of a clinical facility when confronting spiritual issues.</p>
        <p>However, medicine as a “sacred vocation” [<xref ref-type="bibr" rid="B5-religions-03-00467">5</xref>] is coming back. Religious or spiritual problems were recognized as a new diagnostic category (Code V62.89)<xref ref-type="fn" rid="fn001-religions-03-00467">1</xref>: the <italic>Diagnostic and Statistical Manual of Mental Disorders</italic> acknowledged that crises that challenged a person's spiritual composure or the emergence or maturation of one's spiritual identity could present times of psychic-spiritual and even physical distress [<xref ref-type="bibr" rid="B6-religions-03-00467">6</xref>]. The validity of this category has been further improved, so that more subtle distinctions can be made, differentiating mental disorders and religious or spiritual problems from "pure" religious problems [<xref ref-type="bibr" rid="B7-religions-03-00467">7</xref>].</p>
        <p>Approximately 70% of U.S. medical schools now include spiritual dimension (religion and spirituality) in their curricula [<xref ref-type="bibr" rid="B8-religions-03-00467">8</xref>] as a component in the Medical School Objectives Project for medical schools, this represents an approximately 33-fold rise over the last 15 years. The new curricular guidelines of the American Association of Medical Colleges [<xref ref-type="bibr" rid="B9-religions-03-00467">9</xref>] reads: “In all their interactions with patients, physicians must seek to understand the meaning of the patient's stories in the context of the patient's beliefs, family, and cultural values” [<xref ref-type="bibr" rid="B5-religions-03-00467">5</xref>].</p>
        <p>Patient centeredness is now considered as a central component of high-quality health care [<xref ref-type="bibr" rid="B10-religions-03-00467">10</xref>,<xref ref-type="bibr" rid="B11-religions-03-00467">11</xref>]. This is to help practitioners provide care that is concordant with the patient's values, needs and preferences, including their religiosity and spirituality. In fact, in a 2011 GALLUP Poll<xref ref-type="fn" rid="fn002-religions-03-00467">2</xref>, most of the US adults believed in God (no change since the 1940s), and 79% believed that faith can help people recover from disease.</p>
        <p>There is a considerable number of clinical trials, including children, adults and elderly patients, in which significant positive responses to various spiritual and religious means have been observed, and some mechanisms of their actions have been suggested. This is reflected by an exponential rise in research in the relationship between religion, spirituality and health outcomes [<xref ref-type="bibr" rid="B8-religions-03-00467">8</xref>,<xref ref-type="bibr" rid="B12-religions-03-00467">12</xref>] with a &gt;2,500-fold increase in the number of articles in the last 48 years (<xref ref-type="fig" rid="religions-03-00467-f001">Figure 1</xref>).</p>

        <p>Most of these studies, however, have been primarily descriptive, investigating associations between spirituality and health, rather than explanatory, focusing on identifying their underlying mechanisms [<xref ref-type="bibr" rid="B13-religions-03-00467">13</xref>]. Earlier and current research is also limited by the failure to use simple non-invasive biomarker measurements that have been recently developed [<xref ref-type="bibr" rid="B14-religions-03-00467">14</xref>,<xref ref-type="bibr" rid="B15-religions-03-00467">15</xref>,<xref ref-type="bibr" rid="B16-religions-03-00467">16</xref>] and validated [<xref ref-type="bibr" rid="B17-religions-03-00467">17</xref>,<xref ref-type="bibr" rid="B18-religions-03-00467">18</xref>,<xref ref-type="bibr" rid="B19-religions-03-00467">19</xref>].</p>
        <p>The research focus now is turning to examining not only whether religious factors are relevant to human health, but also how spirituality may functionally impact medical and psychological wellbeing [<xref ref-type="bibr" rid="B20-religions-03-00467">20</xref>] and illness. A combined approach, treating the human person as a single, united whole<xref ref-type="fn" rid="fn003-religions-03-00467">3</xref> and the sacred in medicine raise a new set of medical, theological and ethical questions, which are discussed in this article. The spiritual dimension may restore the historically close relationship between medicine and religion, the twin traditions of healing [<xref ref-type="bibr" rid="B21-religions-03-00467">21</xref>].</p>
        <p>This is of particular importance in an era of patient dissatisfaction with a technologically advanced, but increasingly impersonal form of medical care [<xref ref-type="bibr" rid="B22-religions-03-00467">22</xref>]. Research in religion and spirituality in health and disease will benefit from the existing arsenal of simple, non-invasive exhaled biomarker assessments. Creating personal bio-medical and spiritual patients profiles may offer personalization of medical care and significant benefits to the patient at little cost.</p>
      </sec>
      <sec>
        <title>2.2. Understanding Its Potential Health Effect, not Trivializing Religion</title>
        <p>There is a certain danger of religious activities being “prescribed” by either clerical staff or medical doctors, because of the growing number of research studies demonstrating a positive link between religion and health outcomes. We agree with the concern expressed by Sloan <italic>et al.</italic> [<xref ref-type="bibr" rid="B23-religions-03-00467">23</xref>], that “an instrumental approach” to religion, derived from the need to define variables for use in scientific studies, or to validate religion by its effects on health, should not be used to trivialize religion. Although it is agreeable that religion does not need science to justify its existence or appeal [<xref ref-type="bibr" rid="B23-religions-03-00467">23</xref>], it can be argued that medical science needs to be able to quantify its effect in health and disease.</p>
      </sec>
      <sec>
        <title>2.3. Religiosity <italic>vs.</italic> Spirituality</title>
        <p>Spirituality is often confused with religiosity, as the two constructs are closely related, but are not synonymous. Both of them deal with the divine and can involve practices aimed at increasing feelings of connection with a higher power [<xref ref-type="bibr" rid="B24-religions-03-00467">24</xref>]. There are important differences, however:</p>

            <p>a) Religiosity is seen as more of a public, formal, and socialized practice; spirituality is characterized as more private, naturally occurring [<xref ref-type="bibr" rid="B24-religions-03-00467">24</xref>]</p>
          </list-item>
          <list-item>
            <p>b) Religiosity consists of specific, prescribed behaviors and practices sanctioned by an identifiable group of people [<xref ref-type="bibr" rid="B25-religions-03-00467">25</xref>]; spirituality may be unstructured, and independent from formal institutions [<xref ref-type="bibr" rid="B26-religions-03-00467">26</xref>]</p>
          </list-item>
        </list>
        <p>These differences between religiosity and spirituality may not be simply academic. Medical cancer, cardiac and HIV patients, when they were not provided with specific definitions, differentiated easily between the two constructs [<xref ref-type="bibr" rid="B27-religions-03-00467">27</xref>]. It is not surprising, perhaps, that “spiritual individuals” reported that their spirituality helped them to heal themselves, and self-identified religious patients reported that their healing was the direct result of divine intervention.</p>
        <p>Older adults who reported attending religious services more frequently had better functional 1<sup>st</sup> and 7<sup>th</sup>-month recovery following an acute myocardial infarction (AMI) than those who considered themselves more spiritual [<xref ref-type="bibr" rid="B28-religions-03-00467">28</xref>].</p>
        <p>Therefore, religiosity and spirituality, although often overlapping, should be investigated separately.</p>
        <p>Human beings’ relation to that which they regard as holy, absolute, spiritual, divine, or worthy of special reverence (definition of religion in Encyclopaedia Britannica)<xref ref-type="fn" rid="fn004-religions-03-00467">4</xref>, is both difficult to describe and to measure. Therefore, assessment of the potential effect of religiosity and spirituality on health by investigating the existing and discovering novel biomarkers, may not be simply academic, but can be used in the clinic in the future. In medicine, a biomarker can be considered as anything that may be used as an indicator of a particular physiological or psychological state of a human being. A number of biomarkers have been shown to be affected by psychological stress [<xref ref-type="bibr" rid="B29-religions-03-00467">29</xref>,<xref ref-type="bibr" rid="B30-religions-03-00467">30</xref>]. “Spiritual biomarker”, can be defined as any physiological and/or psychological biomarker that is capable to reflect the changing state of a human being involved in a certain religious and/or spiritual activity. Spiritual biomarkers can help characterize and quantify the biological and psychological impact of religion, religiosity and spirituality on health and disease. </p>
      </sec>
      <sec>
        <title>2.4. Healing <italic>vs.</italic> Cure</title>
        <p>There is a tendency to equate healing with cure. When an illness is incurable, there is a common misperception that healing is not possible either. Yet there is an opportunity for healing, even in the midst of an incurable illness. Healing can be manifested as inner peace; peace and reconciliation with God, self, and others; acceptance; letting go; and, finally, a peaceful death [<xref ref-type="bibr" rid="B31-religions-03-00467">31</xref>,<xref ref-type="bibr" rid="B32-religions-03-00467">32</xref>].</p>
      </sec>
      <sec>
        <title>2.5. Integrated Approach to a Patient as a Whole Person</title>
        <p>Western philosophy, beginning with Plato, describes the human being as consisting of two separate parts—the body and the mind [<xref ref-type="bibr" rid="B31-religions-03-00467">31</xref>]. This division was further widened as medicine became more scientific, and religion was seen as a barrier to progress. Eventually, the realm of the body was delegated to physicians, whereas the care of the soul remained with religious leaders [<xref ref-type="bibr" rid="B31-religions-03-00467">31</xref>].</p>
        <p>Early Christian sources assumed a similar dichotomy between body and soul, although St. Irenaeus wrote that “the union of these (soul, the spirit and the body) constitutes the perfect man” [<xref ref-type="bibr" rid="B33-religions-03-00467">33</xref>]. And for this cause Apostle Paul made it clear that the saved man is a complete man as well as a spiritual man; saying thus in the first Epistle to the Thessalonians, “And the very God of peace sanctify you wholly; and I pray God your whole spirit and soul and body be preserved blameless unto the coming of our Lord Jesus Christ” (1 Thes 5:23).</p>
        <p>Just spiritual or physical medical care alone is not enough. The Jewish tradition asserts that humans are integrated wholes, that body, mind, emotions and will all are connected and that these faculties all affect one another [<xref ref-type="bibr" rid="B31-religions-03-00467">31</xref>]. The traditional Jewish prayer for the sick asks God to grant “healing of body and healing of soul” because it recognizes that the two are intertwined. Similarly, Islam teaches that God created the whole person as one unit and in a perfect form, and that the healing process must deal with all aspects of the human being [<xref ref-type="bibr" rid="B31-religions-03-00467">31</xref>].</p>
        <p>In the Second Vatican Council, the first chapter of the Pastoral Constitution of the Church in the Modern World noted, “the human person, though made of body and soul is a unity” [<xref ref-type="bibr" rid="B34-religions-03-00467">34</xref>].</p>
        <p>Therefore, whether from a Christian, Jewish, or Muslim perspective, this view of human beings as a whole has immediate implications for how patients must be treated. Furthermore, this approach makes an integrated approach of creating an individual, but integrated breath profile of a patient as a whole subject, extremely attractive.</p>
      </sec>
      <sec>
        <title>2.6. General Effects of Religion and Spirituality</title>
        <p>Positive association of religion and spirituality with physical and mental health, as it is judged, for example, by the frequency of church (synagogue, or mosque) attendance [<xref ref-type="bibr" rid="B1-religions-03-00467">1</xref>,<xref ref-type="bibr" rid="B21-religions-03-00467">21</xref>], has been demonstrated in many studies, and has been known since the Biblical times<xref ref-type="fn" rid="fn005-religions-03-00467">5</xref>.</p>
        <p>Spiritual and religious practices appear to positively influence medical outcomes and coping [<xref ref-type="bibr" rid="B35-religions-03-00467">35</xref>,<xref ref-type="bibr" rid="B36-religions-03-00467">36</xref>]. A National Health Interview Survey of 21,204 adults has demonstrated better physical and mental health outcomes, as well as increased longevity in those who attended church weekly [<xref ref-type="bibr" rid="B37-religions-03-00467">37</xref>].</p>
        <p>Most of these studies, however, have used cross-sectional designs, and very few of them used any objective measurements of biological parameters. In fact, it has been suggested that frequent religious attendance is simply a proxy for functional ability, especially in older adults whose health may prevent them from regular religious attendance [<xref ref-type="bibr" rid="B21-religions-03-00467">21</xref>].</p>
      </sec>
      <sec>
        <title>2.7. Mortality</title>
        <p>Religious commitment may increase longevity, starting with clergy. Thus, 12 studies, published between 1959 and 2000, examined mortality among American and European clergy [<xref ref-type="bibr" rid="B38-religions-03-00467">38</xref>]. All but one of the reports found lower all-cause death rates for clergy compared to the people of similar age in the general population. Protestant ministers consistently were found to have more than a 25% mortality advantage in various studies. Catholic nuns had a mortality advantage of roughly 20%–25%, whereas the mortality advantage of Catholic priests was just over 10%.</p>
        <p>In a nationwide cohort of Americans, predominantly Christians, analyses demonstrated a lower risk of death independent of confounders among those reporting religious attendance at least weekly compared to never. The association did not differ by gender or ethnicity [<xref ref-type="bibr" rid="B39-religions-03-00467">39</xref>].</p>
        <p>Attendance at religious services (at least weekly compared to never [<xref ref-type="bibr" rid="B39-religions-03-00467">39</xref>]) and prayer were strongly connected to reduced mortality [<xref ref-type="bibr" rid="B40-religions-03-00467">40</xref>,<xref ref-type="bibr" rid="B41-religions-03-00467">41</xref>], and a lower risk of death in a nationwide cohort of 8,450 Americans<xref ref-type="fn" rid="fn006-religions-03-00467">6</xref>, predominantly Christians. People (n = 10,059) who attended religious services ≥40 (40.8%), or &lt;40 times (22.4%) were less likely to have elevated levels of inflammatory markers of cardiovascular disease than non-attendees (36.8%) [<xref ref-type="bibr" rid="B42-religions-03-00467">42</xref>].</p>
        <p>The risk of dying from arteriosclerotic heart disease for men who attended church at least once a week was only 60% of the risk of men who attended church infrequently [<xref ref-type="bibr" rid="B43-religions-03-00467">43</xref>]. The same study also found that death rates from pulmonary emphysema were more than twice as high and that liver cirrhosis was almost four times higher for white men who attended church infrequently compared with white men who attended church at least once a week [<xref ref-type="bibr" rid="B43-religions-03-00467">43</xref>].</p>
        <p>A 12-year follow up study in industrial 3,638 Jewish Israeli males demonstrated that religiosity had a protective effect on mortality in younger employees, but the reverse was true for older employees (total number) [<xref ref-type="bibr" rid="B44-religions-03-00467">44</xref>].</p>
        <p>The lack of objective and non-invasive measurements in this study, however, may be the reason that the mechanisms of these associations were not fully elucidated. Future research should examine methodological and substantive explanations for the discrepancies among these studies.</p>
      </sec>
      <sec>
        <title>2.8. Religiosity and Spirituality in Aging</title>
        <p>Older adults who reported attending religious services more frequently had better functional recovery following an acute myocardial infarction (AMI) [<xref ref-type="bibr" rid="B28-religions-03-00467">28</xref>] and a more active communal life. In fact, spiritual participation and community involvement were associated with volunteering among adults 60 years or older [<xref ref-type="bibr" rid="B45-religions-03-00467">45</xref>].</p>
        <p>Interestingly, rates of volunteering varied by race/ethnicity: Bivariate analyses showed that non-Hispanic white older adults were more likely to participate in formal volunteering activities, while their African American and Hispanic counterparts tended to participate in informal volunteering activities [<xref ref-type="bibr" rid="B45-religions-03-00467">45</xref>]. There is evidence that African Americans rated their private religiosity higher and attended religious services more frequently than Whites [<xref ref-type="bibr" rid="B46-religions-03-00467">46</xref>].</p>
        <p>Medically ill older adults, a population of high users of health care services, who attended church weekly or more often were significantly less likely to have been admitted to hospital in the year prior to the study, had fewer hospital admissions, and spent fewer days in the hospital than those attending less often [<xref ref-type="bibr" rid="B47-religions-03-00467">47</xref>]. Patients unaffiliated with a religious community, while not using more acute hospital services in the year before admission, had significantly longer index hospital stays than those affiliated. Unaffiliated patients spent an average of 25 days in hospital, compared with 11 days for affiliated patients; this association strengthened when physical health and other covariates were controlled.</p>
        <p>There is the need for objective empirical examinations of these issues in countries other than the United States [<xref ref-type="bibr" rid="B48-religions-03-00467">48</xref>]. Given the positive associations of mental health, spirituality and social engagement with volunteering among older adults, they can be also involved in helping their physicians to use simple exhaled biomarkers measurements (e.g., exhaled NO measurements by a portable analyzer NIOX MINO). The latter was successfully used in general practice in older patients with chronic lung diseases [<xref ref-type="bibr" rid="B49-religions-03-00467">49</xref>].</p>
      </sec>
      <sec>
        <title>2.9. Cardio-Vascular Diseases</title>
        <p>There is a link between religious commitment and blood pressure, in that more religiously committed patients had lower blood pressures [<xref ref-type="bibr" rid="B50-religions-03-00467">50</xref>]. In fact, blood pressure among the nuns living in a secluded order in six nunneries in Umbria, central Italy, remained remarkably stable during the 30 years’ observation period [<xref ref-type="bibr" rid="B51-religions-03-00467">51</xref>]. None showed a rise in diastolic blood pressure to above 90 mm Hg, in contrast to the laywomen from the same region and similar demographic details. There were also much lower fatal and non-fatal cardiovascular events in the nuns [<xref ref-type="bibr" rid="B51-religions-03-00467">51</xref>].</p>
        <p>Re-education in religious and spiritual matters that would involve motivational reorientation of both, public and medical professionals, might revolve around a message that ideal cardiovascular health is much more than crisis intervention and beneficial lifestyle.</p>
        <p>A holistic cardiac rehabilitation intervention focusing on several psychosocial and biological predictors of coronary heart disease including depression, low spirituality and others was highly effective in lowering levels of depression and perceived stress, increasing levels of life satisfaction and spirituality upon program completion [<xref ref-type="bibr" rid="B52-religions-03-00467">52</xref>]. Future studies utilizing exhaled biomarker profiling are necessary to determine whether holistic programs are more effective than traditional programs in the reduction of cardiac risk factors.</p>
        <p>It has been suggested that a salutary effect of religion on blood pressure can be explained by the combination of the following correlates of religion: the promotion of health-related behavior; hereditary predispositions in particular groups; the healthful psychosocial effects of religious practice; and, the beneficial psychodynamics of belief systems, religious rites, and faith [<xref ref-type="bibr" rid="B53-religions-03-00467">53</xref>].</p>
        <p>In addition, it has been found that the rates of hypertension-related morbidity and mortality were significantly lower in typically conservative religious groups than in compared populations [<xref ref-type="bibr" rid="B21-religions-03-00467">21</xref>]. Spiritual wellbeing may be cardio protective, as it has been significantly related to lower systolic and diastolic BP, hs-C-reactive protein, fasting glucose, and marginally lower triglycerides [<xref ref-type="bibr" rid="B54-religions-03-00467">54</xref>].</p>
        <p>Little research has focused on the relationship between religious attendance and inflammatory markers of cardiovascular risk. A national representative sample of adults aged ≥40 derived from the National Health and Nutrition Examination Survey III 1988–1994 (n = 10,059) has demonstrated that 40.8% of the population attended religious services 40 or more times in the previous year while 22.4% attended services less than 40 times and 36.8% attended no religious services at all [<xref ref-type="bibr" rid="B42-religions-03-00467">42</xref>]. Non-attendees of religious services were more likely than attendees to have elevated white blood cell counts, highly elevated C-reactive protein, and elevated fibrinogen.</p>
        <p>These findings suggest that people who have attended religious services in the previous year are less likely to have elevated levels of certain inflammatory markers, however, current smoking has significant shared variance with religious attendance.</p>
      </sec>
      <sec>
        <title>2.10. Diabetes</title>
        <p>Diabetes affects 2.8 million people in the UK and 26 million people in the U.S.<xref ref-type="fn" rid="fn007-religions-03-00467">7</xref> The majority of sufferers have type II of the condition. Spiritual care is an important component of providing culturally sensitive health care to African Americans [<xref ref-type="bibr" rid="B55-religions-03-00467">55</xref>] and Hispanic adults in the USA, 78% of whom believed they had diabetes because it was God's will [<xref ref-type="bibr" rid="B56-religions-03-00467">56</xref>], and other patients [<xref ref-type="bibr" rid="B57-religions-03-00467">57</xref>] with diabetes, facilitating their self-management.</p>
        <p>Mormons in Utah may have lower cardiac mortality than other people living in the same State and the US population. A population of 448 patients was surveyed (2004 to 2006) for the association of behavioral factors with coronary artery disease (CAD), with routine fasting (<italic>i.e.</italic>, abstinence from food and drink) as the primary variable [<xref ref-type="bibr" rid="B58-religions-03-00467">58</xref>].</p>
        <p>Routine periodic religious fasting in a representative group (<italic>n </italic>= 448) of Mormons of Utah has been shown to be associated with lower diabetes prevalence [<xref ref-type="bibr" rid="B58-religions-03-00467">58</xref>].</p>
        <p>Although diabetics may be exempted from Ramadan fasting, many patients still insist on this worship. In fact, there was no significant change from pre-Ramadan in fasting blood glucose, post-prandial blood glucose, and HbA1c variables in fasting diabetics at post-Ramadan and one-month post-Ramadan [<xref ref-type="bibr" rid="B59-religions-03-00467">59</xref>].</p>
        <p>There are several exhaled biomarkers that potentially may be used to monitor the effect of religiosity and spirituality on diabetes, including exhaled carbon monoxide [<xref ref-type="bibr" rid="B60-religions-03-00467">60</xref>], carbon dioxide [<xref ref-type="bibr" rid="B61-religions-03-00467">61</xref>] and hydrocarbons [<xref ref-type="bibr" rid="B62-religions-03-00467">62</xref>,<xref ref-type="bibr" rid="B63-religions-03-00467">63</xref>]. A novel non-invasive method has been developed analyzing human tear proteins enabled by an alkaline microfluidic homogeneous immunoassay [<xref ref-type="bibr" rid="B64-religions-03-00467">64</xref>]. Thus it may be possible to measure tear glucose levels multiple times per day to monitor blood glucose changes without the potential pain from repeated invasive blood drawing methods.</p>
      </sec>
      <sec>
        <title>2.11. Asthma</title>
        <sec>
          <title>2.11.1. Psychological and Socioeconomic Stress, Inflammatory Biomarkers</title>
          <p>Asthmatic airways constrict in response to emotion and stress, and asthma symptoms are less severe in patients with higher religiosity and spirituality. However, the underlying mechanisms, potential extra thoracic contributions, and associations with airway pathophysiology in emotion-induced or spiritual-relieved airway responses have not been elucidated. Psychosocial stress and socioeconomic status (SES) are known to influence the pathophysiology of asthma, including serum inflammatory markers and exaggerated allergen response. Lower SES and related higher levels of stress and threat perception were associated with higher chronic stress and perceived threat, heightened production of IL-5 and IL-13 and higher eosinophil counts in children with asthma [<xref ref-type="bibr" rid="B29-religions-03-00467">29</xref>]. It has been confirmed that chronic stress and threat perception represented statistically significant overlapping pathways between SES and immune processes in children with asthma. Associations among SES, psychological stress, and immune pathways suggest that the experience of stress, particularly among lower SES children, has implications for childhood asthma morbidity [<xref ref-type="bibr" rid="B29-religions-03-00467">29</xref>].</p>
          <p>The link between the perceived psychological triggers and stress in patients' daily lives suggests a physiological basis for emotion-induced asthma, as it has been demonstrated by the increased respiratory resistance and exhaled NO during unpleasant films in asthma patients [<xref ref-type="bibr" rid="B65-religions-03-00467">65</xref>,<xref ref-type="bibr" rid="B66-religions-03-00467">66</xref>]. Exhaled NO (FeNO) was a significant mediator of the association of both negative affect and daily hassles with lung function changes, confirming the pathophysiological link between stress and airway inflammation in asthma [<xref ref-type="bibr" rid="B66-religions-03-00467">66</xref>]. Interestingly, this response was blocked by ipratropium bromide and was not substantially associated with asthma severity, airway inflammation, hyper-reactivity and reversibility.</p>
          <p>There are several novel biomarkers measured in exhaled breath condensate in asthma, including microsatellite alterations [<xref ref-type="bibr" rid="B67-religions-03-00467">67</xref>] suggestive of organ-specific asthma and atopy, exhaled IL-4 and pH [<xref ref-type="bibr" rid="B68-religions-03-00467">68</xref>].</p>
          <p>Furthermore, an inverse association of SES with changes in FeNO levels in response to the conflict task, meaning that as SES declined, greater increases in FeNO were observed [<xref ref-type="bibr" rid="B69-religions-03-00467">69</xref>]. This study suggests that lower SES children with asthma may be more vulnerable to heightened airway inflammation in response to stress.</p>
          <p>In asthma elevated rates of exhaled breath temperature changes and bronchial blood flow may be due to increased vascularity of the airway mucosa as a result of inflammation [<xref ref-type="bibr" rid="B70-religions-03-00467">70</xref>,<xref ref-type="bibr" rid="B71-religions-03-00467">71</xref>].</p>
          <p>Recent studies have demonstrated lower levels or reactivity of endogenous cortisol in asthma [<xref ref-type="bibr" rid="B72-religions-03-00467">72</xref>]. Indeed, elevated exhaled NO was further increased and low salivary cortisol levels became highly elevated in patients with asthma after the stress protocol [<xref ref-type="bibr" rid="B73-religions-03-00467">73</xref>]. Acute psychosocial stress alone increases airway inflammatory markers in asthma and this increase is attenuated by stronger stress-related activity of the hypothalamic-pituitary-adrenal axis in asthma [<xref ref-type="bibr" rid="B73-religions-03-00467">73</xref>].</p>
        </sec>
        <sec>
          <title>2.11.2. Effect of Worship and Lifestyle</title>
          <p>It has been shown that engagement in Pentecostal worship is associated with reductions in circulatory salivary cortisol and enhancements in α-amylase activity, reflecting the expected reduction of stress after a day of worship [<xref ref-type="bibr" rid="B74-religions-03-00467">74</xref>]. Religious coping with stress has been shown to be closely related to problem-focused coping [<xref ref-type="bibr" rid="B75-religions-03-00467">75</xref>].</p>
          <p>The anthroposophic<xref ref-type="fn" rid="fn008-religions-03-00467">8</xref> family lifestyle reduces the level of stress and allergic diseases in infants [<xref ref-type="bibr" rid="B76-religions-03-00467">76</xref>]. Salivary samples of 330 infants and their parents demonstrated that low salivary cortisol levels in infants from anthroposophic families <italic>vs.</italic> families with more conventional lifestyles, may be related to an environment with a lower degree of exposure to stress, which could influence the development of allergic diseases [<xref ref-type="bibr" rid="B76-religions-03-00467">76</xref>].</p>
          <p>Although the positive role of religion and spirituality on psychosocial stress and clinical asthma outcomes has been demonstrated, controlled, laboratory paradigms that test associations between religion and spirituality and markers of airway inflammation in asthma are lacking. We speculate that investigating the role of religious coping in asthma may require more complex approaches, including exhaled NO and salivary cortisol measurements.</p>
        </sec>
      </sec>
      <sec>
        <title>2.12. Smoking and Chronic Obstructive Pulmonary Disease (COPD)</title>
        <p>There are important issues, especially with patients diagnosed with life-limiting illnesses (e.g., COPD). Despite the fact that end-of-life topics are important to patients, most clinicians focus on life-preserving treatments and avoid conversations about end-of-life care. A recent cross-sectional study of COPD outpatients (<italic>n</italic> = 376) who completed the Quality of Communication (QOC) questionnaire (outcome measure) demonstrated that all end-of-life communication topics were under-addressed by clinicians, regardless of their training [<xref ref-type="bibr" rid="B77-religions-03-00467">77</xref>].</p>
        <p>Religious commitment prevents health problems even among people who engage in religiously disapproved behaviors such as smoking. Thus “religious smokers” who attended church at least once a week were four times less likely to have an abnormal diastolic pressure than “less religious smokers” who attended church infrequently [<xref ref-type="bibr" rid="B78-religions-03-00467">78</xref>]. This difference was seven times more evident between “religious” <italic>vs.</italic> “non-religious” smokers.</p>
        <p>A recent study has shown that one session of water pipe smoking (WPS) causes acute biologic potentially harmful changes that might result in marked health problems. These included - increased blood pressure, heart rate, exhaled carbon monoxide (CO) and decreased FEV1, exhaled Feno and 8-isoprostane levels in exhaled breath condensate (EBC) and eosinophils in peripheral blood [<xref ref-type="bibr" rid="B79-religions-03-00467">79</xref>]. Potentially, impulse oscillometry can be used to differentiate patients with asthma from those with COPD [<xref ref-type="bibr" rid="B80-religions-03-00467">80</xref>].</p>
        <p>An acute [<xref ref-type="bibr" rid="B81-religions-03-00467">81</xref>] and chronic [<xref ref-type="bibr" rid="B82-religions-03-00467">82</xref>] effect of smoking on exhaled NO, exhaled CO [<xref ref-type="bibr" rid="B83-religions-03-00467">83</xref>] and exhaled 8-isoprostanes [<xref ref-type="bibr" rid="B84-religions-03-00467">84</xref>] have been described. We suggest that monitoring these exhaled biomarkers (NO, CO, 8-isoprostane) might be useful to assess the effect of religiosity in smokers with cardio-vascular problems.</p>
      </sec>
      <sec>
        <title>2.13. Depression and Stress</title>
        <p>Spiritual and/or religious individuals may experience a protective effect against the neuroendocrine consequences of stress. Young adults with higher composite religiosity/spirituality scores, religiosity, levels of forgiveness and frequency of prayer showed lower cortisol responses to stress [<xref ref-type="bibr" rid="B85-religions-03-00467">85</xref>]. Religiosity may have a protective effect on the physiological effects of stress among women with fibromyalgia [<xref ref-type="bibr" rid="B86-religions-03-00467">86</xref>].</p>
        <p>Hyperactivity of the hypothalamic-pituitary-adrenal (HPA)-axis is a common finding in major depressive disorder. Chemokines, in addition to their traditional inflammatory functions, also have neuromodulatory effects, e.g., they are expressed in dopaminergic neurons and affect dopamine turnover [<xref ref-type="bibr" rid="B87-religions-03-00467">87</xref>,<xref ref-type="bibr" rid="B88-religions-03-00467">88</xref>]. Chemokines are also involved in CNS development and in the maintenance of normal brain homeostasis [<xref ref-type="bibr" rid="B89-religions-03-00467">89</xref>].</p>
        <p>A few recent studies have shown increased levels of certain chemokines in the blood of depressed patients [<xref ref-type="bibr" rid="B88-religions-03-00467">88</xref>,<xref ref-type="bibr" rid="B90-religions-03-00467">90</xref>].</p>
        <sec>
          <title>2.13.1. Chemokines, Cytokines, Neuropeptide Hormones, Matrix Metalloproteinases</title>
          <p>Recently, 20 cerebrospinal fluid (CSF) biomarkers were measured in 124 drug-free suicide attempters: kynurenic acid, orexin, homovanillic acid (HVA), 5-hydroxyindoleacetic acid (5-HIAA), 3-methoxy-4-hydroxyphenylglycol, chemokines, matrix metalloproteases and cytokines [<xref ref-type="bibr" rid="B88-religions-03-00467">88</xref>]. Consequently, principal component analysis identified four factors explaining 52.4% of the total variance: factors 1 and 2 were characterized by high loadings of chemokines and cytokines, respectively, and were both associated with severe depressive symptoms; factor 2 was also associated with a high suicidal intent; factor 4 was characterized by strong loadings of the monoamine metabolites 5-HIAA and HVA, as well as orexin and interleukin-6. High scores on this factor were found in patients who performed a violent suicide attempt and in patients who subsequently completed suicide [<xref ref-type="bibr" rid="B88-religions-03-00467">88</xref>].</p>
          <p>Concentration of eotaxin in serum was almost 10 times higher in patients with major depressive disorder (MDD) compared to controls [<xref ref-type="bibr" rid="B90-religions-03-00467">90</xref>]. Depressive symptoms were highly associated with high loadings of eotaxins [<xref ref-type="bibr" rid="B88-religions-03-00467">88</xref>], other study also reported low serum levels of the chemokines MCP-1 and MIP-1b in patients with MDD [<xref ref-type="bibr" rid="B91-religions-03-00467">91</xref>].</p>
          <p>Recent technological advances in exhaled breath biomarkers measurements offer an opportunity to further elucidate the complex chemokine/cytokine network in MDD and their potential “spiritual modulation”.</p>
          <p>We speculate that measuring several inflammatory mediators simultaneously [<xref ref-type="bibr" rid="B17-religions-03-00467">17</xref>], such as eotaxins [<xref ref-type="bibr" rid="B92-religions-03-00467">92</xref>], cytokines and chemokines [<xref ref-type="bibr" rid="B93-religions-03-00467">93</xref>,<xref ref-type="bibr" rid="B94-religions-03-00467">94</xref>,<xref ref-type="bibr" rid="B95-religions-03-00467">95</xref>] levels in exhaled breath condensate may be used to monitor the modulating effect of religiosity and spirituality in patients with depression.</p>
          <p>Significant elevations of multiple pro-inflammatory and anti-inflammatory cytokines in patients with depression support a generalized chronic inflammatory state in MDD [<xref ref-type="bibr" rid="B88-religions-03-00467">88</xref>]. Therefore, and implicate additional cytokines and chemokines previously linked to cardiovascular disease.</p>
          <p>While the etiology of cytokine elevations in MDD remains unknown and significantly more work is needed, the growing understanding of cytokines in individuals with MDD suggests the intriguing possibility that early intervention, perhaps with anti-inflammatory agents and/or antioxidants, might reduce or prevent downstream medical consequences of MDD.</p>
          <p>Corticotrophin-releasing hormone (CRH) in CSF, urinary and salivary cortisol were monitored in the suicide attempters for approximately 12 years after the index admission [<xref ref-type="bibr" rid="B96-religions-03-00467">96</xref>]. Low cortisol levels were associated with severe psychiatric symptoms, and lower evening salivary cortisol was found in suicide attempters compared to controls. Among women, repeated suicide attempts were associated with low morning and lunch salivary and urinary cortisol and CRH levels.</p>
          <p>Therefore, easily detectable (salivary or exhaled breath condensate) hypocortisolism may serve not only as a predictor of a suicidal attempt, but also of the potential positive role of religion in these patients.</p>
          <p>Religious commitment has been associated with a decreased prevalence of depression [<xref ref-type="bibr" rid="B97-religions-03-00467">97</xref>,<xref ref-type="bibr" rid="B98-religions-03-00467">98</xref>]. Higher levels of religiosity may protect against one of the most severe outcomes of depression: suicide [<xref ref-type="bibr" rid="B99-religions-03-00467">99</xref>].</p>
          <p>The reported results suggest that specific combinations of CSF and, potentially exhaled, biomarkers may discriminate between types of suicidal behavior and indicate an increased risk for future suicide and the role of religiosity and spirituality as a preventing means.</p>
        </sec>
      </sec>
      <sec>
        <title>2.14. Lifestyle and Nutrition</title>
        <p>Mormons in Utah may have lower cardiac mortality than other people in the same State and the US population. A population of 448 patients was surveyed (2004 to 2006) for the association of behavioral factors with coronary artery disease (CAD), with routine fasting (<italic>i.e.</italic>, abstinence from food and drink) as the primary variable [<xref ref-type="bibr" rid="B58-religions-03-00467">58</xref>]. Routine periodic fasting was associated with lower risk of CAD (64% <italic>vs.</italic> 76%), and this remained after adjustment for traditional risk factors. Fasting was also associated with lower diabetes prevalence. Therefore, not only proscription of tobacco, but also routine periodic fasting was associated with a lower risk of CAD.</p>
        <p>Future research should include an examination of the links between religious involvement and other important lifestyle factors that may influence maternal and child health. Thus, pregnant and postpartum women frequently (more than once a week) and regularly (once a week) attending church services had 80% and 60%, respectively, lower odds of drinking alcohol compared to women who attended less than once a week [<xref ref-type="bibr" rid="B100-religions-03-00467">100</xref>]. Similar patterns surfaced with regard to smoking tobacco, with the odds of smoking being roughly 85% lower among frequent attenders, and nearly 65% lower among regular attenders.</p>
        <p>Various health effects of religious activities of different religions and religious groups have been described. Thus, fasting during the holy month, Ramadan, is a religious obligation for all adult healthy Muslims. During this month, Muslims are allowed to eat and drink between sunset and dawn. However, they are not allowed to eat and drink after dawn. Indeed, beneficial effects of fasting during Ramadan on neutrophil phagocytic function biomarkers have been included [<xref ref-type="bibr" rid="B101-religions-03-00467">101</xref>].</p>
        <p>There is only one published paper investigating a link between religious style of life and exhaled breath analysis using thermal desorption gas chromatography mass spectrometry (TD-GC-MS) [<xref ref-type="bibr" rid="B102-religions-03-00467">102</xref>]. Male monks fasting for 63 hours<xref ref-type="fn" rid="fn009-religions-03-00467">9</xref> were examined as a group that simulates the medical status of victims entrapped under the ruins of collapsed buildings after an earthquake to be found by their exhaled breath. Religious fast resulted in a 30-fold increase of acetone concentration (5.8 ppm) which was similar to the reported 15-fold increase after 24-hour fasting [<xref ref-type="bibr" rid="B103-religions-03-00467">103</xref>], and to the levels in diabetes (1.7–3.7 ppm) [<xref ref-type="bibr" rid="B62-religions-03-00467">62</xref>]. A significant number of hydrocarbons has also been identified, suggesting high levels of oxidative stress in fasting monks.</p>
      </sec>
      <sec>
        <title>2.15. Complementary and Alternative Medicine (CAM) Phenomenon</title>
        <p>The search for the spiritual a dimension in their medical care may be behind the recent and strong trend towards the use of complementary and alternative medicine (CAM)<xref ref-type="fn" rid="fn010-religions-03-00467">10</xref> in conjunction with conventional medicine: According to the 2002 National Health Interview Survey based on 31,044 interviews [<xref ref-type="bibr" rid="B104-religions-03-00467">104</xref>], 54.9% individuals seek the help of CAM. It has been reported that some forms of CAM were used by 36% of U.S. adults aged 18 years, or by 62% when prayer specifically for health reasons was included in the definition of CAM [<xref ref-type="bibr" rid="B104-religions-03-00467">104</xref>].</p>
        <p>The prevalence of CAM use for treatment remains high (2007 U.S. National Health Interview Survey, n = 20,710) among adults (86.9%), regardless of their functional status, although mind-body therapies (MBT) (39.3% <italic>versus</italic> 17.4%, p &lt; 0.001) were prevalent in patients with functional limitations. Moreover, the belief in miracles is found to be common in the U.S., and it is an important determinant of how decisions are made for those with advanced illness [<xref ref-type="bibr" rid="B104-religions-03-00467">104</xref>].</p>
        <p>A recent cross-sectional survey of 328 patients attending an urban general practice in the Republic of Ireland demonstrated that 27% of the patients had visited a CAM practitioner within the past 12 months [<xref ref-type="bibr" rid="B105-religions-03-00467">105</xref>]. A significant positive association was found between CAM use and female gender, middle-aged, private health insurance and full time employment, and the most common reason for use was “to treat an illness for which conventional medicine was already sought” (42%). Interestingly, a high rate of non-disclosure to GPs was found (41% patients) and personal recommendation was the most important source of information (53.2%).</p>
        <p>One of the reasons for patients turning to CAM treatment is the eroding the state of the art of medicine when medical scientists set themselves apart from the doctor-patient relationship in order to obtain a knowledge that is stripped of personal elements [<xref ref-type="bibr" rid="B106-religions-03-00467">106</xref>].</p>
        <p>Religious and spirituality research offers promise in breaking down the dichotomy between the art of medicine and science by directing scientific investigations to techniques of ameliorating illness, thus enhancing the art of medicine and patient care [<xref ref-type="bibr" rid="B106-religions-03-00467">106</xref>].</p>
        <p>The prevalence of CAM use for treatment, including combination of CAM and prayer for health, is high. Public health strategies are needed to promote increased screening for spirituality use in CAM by health care providers, promote increased use of objective non-invasive means, <italic>i.e.</italic>, exhaled biomarkers to monitor therapeutic effects of CAM which will promote health care interactions and facilitate communication about CAM safety and efficacy [<xref ref-type="bibr" rid="B107-religions-03-00467">107</xref>].</p>
        <p>This also indicates that the current popularity of an alternative healthcare system and the extent to which modern patients have turned to “spirituality in treatment” are approached with the hope that they would help treat and prevent disease and enhance quality of life.</p>
        <p>Many important questions remain to be answered in pursuit of the goal of individualized medicine <italic>via</italic> therapeutically-oriented research on the religion/spirituality effect. Constructing individual and objective patient profiles and their changes in the course of treatment by using non-invasive exhaled biomarkers might be of great value for physicians necessarily relying on individualized judgments about how to relate to particular patients. It is reasonable to suppose that the interaction between clinicians and patients is more or less likely to promote optimal therapeutic outcomes; and these interactions can be evaluated by exhaled biomarkers.</p>
      </sec>
      <sec>
        <title>2.16. Religious, Ethnic and Behavioral Differences in Health</title>
        <p>The role of religion in the patterning of health inequities and how this is related to ethnicity and socioeconomic status has been studied in 14,924 Christians, 4,337 Muslims, 656 Sikhs, 1,197 Hindus and 2,682 people reporting not to identify with any religion with different ethnic backgrounds, who were interviewed as part of the Health Survey for England in either 1999 or 2004, adjusted for age, gender and socioeconomic status and periodicity [<xref ref-type="bibr" rid="B108-religions-03-00467">108</xref>]. Multivariate analyses using nationally representative data were performed on self-assessed fair or poor health, longstanding limiting illness, diagnosed diabetes, diagnosed hypertension, waist–hip ratio, body mass index, current tobacco use and participation in no regular physical activity.</p>
        <p>As expected, the odds ratios for general health, hypertension, diabetes, waist-hip ratio, tobacco use and physical activity stressed the importance of ethnicity in the patterning of health inequalities. Unexpectedly, however, there was strong evidence of an important, independent role for religion, with risks for the different health indicators varying between people with the same ethnic, but different religious, identifications [<xref ref-type="bibr" rid="B108-religions-03-00467">108</xref>].</p>
        <p>Non-Christians were more likely to report activity-limiting illnesses than Christians, regardless of ethnicity. More importantly, the risks for the different health outcomes often varied between groups with the same ethnic, but different religious, associations. The ethnic/religious patterning of health also varied considerably by gender, supporting the need to explore the health risks of men and women separately [<xref ref-type="bibr" rid="B108-religions-03-00467">108</xref>].</p>
        <p>Several non-invasive exhaled biomarkers were successfully used for recognition of ethnic differences in both adults and children. Differences in FeNO and forced expiratory lung volumes between South Asian and white children were found to be evident and existed from a very young age [<xref ref-type="bibr" rid="B109-religions-03-00467">109</xref>], also reference ranges for FeNO in healthy Japanese adults were similar to those of Caucasians [<xref ref-type="bibr" rid="B110-religions-03-00467">110</xref>]. Immigrant children of Turkish and Moroccan origin showed a higher prevalence of allergic sensitization and FeNO compared to Dutch children [<xref ref-type="bibr" rid="B111-religions-03-00467">111</xref>].</p>
        <p>These findings show important differences in the health of people of different religions, suggesting that our understanding of health inequalities can be further enhanced by relevant non-invasive biomarkers that measure the impact of this underlying heterogeneity.</p>
        <p>The effect of closely intertwined social and religious ideologies on health risk behaviors in sexual minority youths in the United States has been recently reported by Hatzenbuehler <italic>et al.</italic> [<xref ref-type="bibr" rid="B112-religions-03-00467">112</xref>]. The central finding of this study is that the religious climate surrounding lesbian, gay, and bisexual (LGB) youths serves as a determinant of their health risk behaviors. Although a similar influence was found among heterosexual youths, it was more strongly associated with the health behaviors of LGB youths [<xref ref-type="bibr" rid="B112-religions-03-00467">112</xref>].</p>
        <p>Contrasting results have been recently reported of a protective effect of religion on health in heterosexual youths and its lack among LGB youths [<xref ref-type="bibr" rid="B113-religions-03-00467">113</xref>], <italic>vs.</italic> the data that showed that LGB youths living in counties with more supportive religious climates exhibited fewer health risk behaviors, indicating that religion can be protective for LGB youths [<xref ref-type="bibr" rid="B112-religions-03-00467">112</xref>]. An objective measure of the religious climate assessed at the ecological level in the latter study, <italic>vs.</italic> an individual-level measurements of the environment (e.g., perceived discrimination) used in the latter study, provided the methodological limitations of existing studies on environmental risk factors and health risk behaviors among LGB individuals, which have relied on individual-level measurements of the environment (e.g., perceived discrimination).</p>
        <p>Therefore, further research is needed, exploring novel objective biomarkers that are not confounded with health status that, potentially, may lead to biased estimates of the association between religion and health of members of socially disadvantaged groups.</p>
      </sec>
      <sec>
        <title>2.17. The Efficacy of Prayer<xref ref-type="fn" rid="fn011-religions-03-00467">11</xref></title>
        <p>C.S. Lewis wrote:</p>

        <p>[“Medicine, as all true doctors admit, is not an exact science. You need not, unless you choose, believe in a causal connection between the prayers and the recovery. For prayer is request. The essence of request is that it may or may not be granted”]</p>
        <attrib>[<xref ref-type="bibr" rid="B114-religions-03-00467">114</xref>]</attrib>
		</disp-quote>
        <p>Although the majority of studies reviewed rely on correlational data, there is nevertheless considerable evidence that prayer, religiosity, and spirituality can be associated with health benefits. This is the case despite the fact that the correlations between prayer and other religious behaviors are rarely reported or controlled statistically (or experimentally) to understand the unique contribution of prayer to health [<xref ref-type="bibr" rid="B2-religions-03-00467">2</xref>].</p>
        <p>The empirical results from recent randomized controlled studies on remote, intercessory prayer remain mixed. </p>
        <p>Several studies have, however, appeared in prestigious medical journals, and it is believed by many researchers, including apparent skeptics, that it makes sense to study intercessory prayer (IP) as if it were just another experimental drug treatment [<xref ref-type="bibr" rid="B115-religions-03-00467">115</xref>]. Among the many recent attempts to demonstrate the medical benefits of religious activity, the methodologically strongest seem to be studies of the effects of distant IP. There are several studies ranging from highly critical [<xref ref-type="bibr" rid="B116-religions-03-00467">116</xref>,<xref ref-type="bibr" rid="B117-religions-03-00467">117</xref>], to moderately positive [<xref ref-type="bibr" rid="B118-religions-03-00467">118</xref>,<xref ref-type="bibr" rid="B119-religions-03-00467">119</xref>,<xref ref-type="bibr" rid="B120-religions-03-00467">120</xref>] and highly positive [<xref ref-type="bibr" rid="B121-religions-03-00467">121</xref>,<xref ref-type="bibr" rid="B122-religions-03-00467">122</xref>].</p>
        <p>Generally, claims of benefits of IP are not well supported by well-controlled clinical trials. It is still unclear whether prayer itself or knowledge that IP is being provided may influence outcome [<xref ref-type="bibr" rid="B123-religions-03-00467">123</xref>]. Periodic reassessment of epistemological assumptions on which the majority of research domains are based is important, because they influence our interpretation of experimental outcomes, especially in the studies of IP.</p>
        <p>Some of the early IP studies claimed that the “randomized double-blind design” was used, when one hospitalized group of patients received IP by participating Christians praying outside the hospital, and the control group did not [<xref ref-type="bibr" rid="B124-religions-03-00467">124</xref>]. Indeed, it was observed that the IP group of cardiac patients had a significantly lower severity score compared with the control patients who required ventilator assistance, antibiotics, and diuretics more frequently [<xref ref-type="bibr" rid="B124-religions-03-00467">124</xref>].</p>
        <p>Intercessory prayer did not significantly decrease the odds of cardiac patients experiencing a bad or intermediate outcome, nor did it affect their mortality rate [<xref ref-type="bibr" rid="B125-religions-03-00467">125</xref>]. This, however, may be due to the inclusion of an assumption of poor outcome for those not accounted for in the final analyses. Although based on heterogeneous data, another Cochrane review (10 studies, 7,646 patients) reported no statistically significant effect of intercessory prayer for re-admission to a Coronary Care Unit [<xref ref-type="bibr" rid="B126-religions-03-00467">126</xref>].</p>
        <p>A more recent study (Study on the Therapeutic Effects of intercessory Prayer, STEP) was conducted at six US hospitals to evaluate whether receiving IP or being certain of receiving IP was associated with uncomplicated recovery after coronary artery bypass graft (CABG) surgery [<xref ref-type="bibr" rid="B123-religions-03-00467">123</xref>]. It was concluded that IP itself had no effect on complication-free recovery from CABG, but certainty of receiving intercessory prayer was associated with a higher incidence of complications.</p>
        <p>The paper by Schwartz and Dossey [<xref ref-type="bibr" rid="B127-religions-03-00467">127</xref>] argues that the dose-dependent design characteristic of drug trials, and the adopted use in the STEP and other studies concerning blindness and randomization, is not the optimal model for intention-healing research and must be reappraised. New lines of research linking belief and intention with novel informative biomarkers may be important for an accurate and meaningful study outcome. Major problems with the data on IP are the lack of objective outcome variables, raising problems of multiple comparisons. Apart from the need of more rigorous experimental, longitudinal protocols and comparisons of prayer with known treatments, future studies need to be based on investigations with proper measures of control variables.</p>
        <p>Integration of religious or spiritual dimensions of an individual's life into therapeutic activities raises important ethical considerations. Prayer may be considered as either alternative or as an adjunct to conventional medical therapy. Most of the physicians and clerics will strongly discourage the former and the majority of patients will desire the adjunct model. In the future, a helpful principle may be to inform but not to recommend [<xref ref-type="bibr" rid="B5-religions-03-00467">5</xref>].</p>
        <p>Before the gap between prayerful and medical practice can be bridged, research into the efficacy of prayer and religiosity might need to be reconstructed, considering humans as integrated, multifactorial beings living within a complex milieu [<xref ref-type="bibr" rid="B2-religions-03-00467">2</xref>]. Then, prayer, religiosity, and spirituality might be assessed and understood better, and the influence of spirituality and prayer within humans' daily lives might be more accepted.</p>
        <p>The use of prayer as a therapeutic resource and the means of its assessment in published peer reviewed journals are the focus of this review. No attempt will be made to advocate prayer as a therapeutic supplement to any therapy, or to validate prayer purely scientifically. It is agreeable, however, that the self-reported frequency of prayer may be superficially related to the personal and underlying meaning of religion [<xref ref-type="bibr" rid="B23-religions-03-00467">23</xref>].</p>
        <p>This work may also represent an important step in the convergence of multiple realms of research by linking various biomarkers and physiological measures with indicators of individual belief systems.</p>
        <sec>
          <title>2.17.1. Prayer and Various Clinical Conditions</title>
        <sec>
          <title>2.17.1.1. Stress</title>
          <p>Engagement in church worship is associated with reduction in stress. Salivary cortisol, as a biomarker of stress and sympathetic nervous system activity, was measured in a group (<italic>n </italic>= 52) of Apostolic Pentecostals in New York's mid-Hudson Valley [<xref ref-type="bibr" rid="B74-religions-03-00467">74</xref>]. Saliva samples were collected at four predetermined times on consecutive Sundays and Mondays to establish diurnal profiles and compare days of worship and non-worship.</p>
          <p>A significant decrease in cortisol and an increase in α-amylase on a non-worship day compared with a service day were demonstrated [<xref ref-type="bibr" rid="B74-religions-03-00467">74</xref>]. This suggests that engaging in religious worship activities would reduce baseline stress levels after a day of worship among Pentecostals, and may be monitored by simple non-invasive salivary biomarkers. Greater composite religiosity/spirituality, religiosity, frequency of prayer and attendance at services of young adults were associated with lower blood pressure in males, elevated blood pressure in females, and lower cortisol responses to stress [<xref ref-type="bibr" rid="B85-religions-03-00467">85</xref>].</p>
        </sec>
        <sec>
          <title>2.17.1.2. Cancer</title>
          <p>Spiritual healing practices such as faith healing and prayer have been found to be some of the most common strategies used by patients with various types of cancer [<xref ref-type="bibr" rid="B24-religions-03-00467">24</xref>,<xref ref-type="bibr" rid="B128-religions-03-00467">128</xref>].</p>
          <p>The recent advances in the psycho-oncological and psycho-neuroimmunological investigations of cancer patients has allowed the rediscovery of the importance of spiritual faith in influencing the clinical course of neoplastic disease, not only in terms of supportive care but also as a potential prognostic variable [<xref ref-type="bibr" rid="B129-religions-03-00467">129</xref>]. The tumor response rate and three-year survival achieved in a group of 50 metastatic non-small cell lung cancer (NSCLC) patients with a high degree of faith was significantly higher than in the other group of patients [<xref ref-type="bibr" rid="B129-religions-03-00467">129</xref>]. Moreover, the mean post-chemotherapeutic lymphocyte number was significantly higher in the patients with evident spiritual faith than in the other patients. This study also suggests that the effect of faith may be assessed by various lymphocyte-mediated biomarkers.</p>
          <p>There are several non-invasive biomarkers measurable in exhaled breath condensate, such as ferritin and superoxide dismutase [<xref ref-type="bibr" rid="B130-religions-03-00467">130</xref>], LTB-4 and IL-8 [<xref ref-type="bibr" rid="B131-religions-03-00467">131</xref>], microsatellite alterations (MAs) at the long arm of chromosome 19 [<xref ref-type="bibr" rid="B132-religions-03-00467">132</xref>], cyclooxygenase (COX-2) and survivin [<xref ref-type="bibr" rid="B133-religions-03-00467">133</xref>], IL-2, TNF-alpha and leptin [<xref ref-type="bibr" rid="B134-religions-03-00467">134</xref>], which, if deemed feasible, have clinical implications in the monitoring of lung cancer and as an religious outcome predictor.</p>
        </sec>
        <sec>
          <title>2.17.1.3. Arthritis</title>
          <p>Patients with long-standing moderately severe rheumatoid arthritis (RA) were shown to derive significant short- and long-term physical benefits (pain and functional impairment, physical signs, erythrocyte sedimentation rate and C-reactive protein) from in-person intercessory prayer ministry [<xref ref-type="bibr" rid="B22-religions-03-00467">22</xref>]. The specific therapeutic agent of prayer could not be proven definitively, because the intervention was multimodal, combining the putative factor of prayer ministry with group education and counseling. Increased hydrogen peroxide in RA [<xref ref-type="bibr" rid="B135-religions-03-00467">135</xref>] may be used to monitor the effect of intercessory prayer in the treatment of these patients.</p>
        </sec>
        <sec>
          <title>2.17.1.4. Open-heart Surgery</title>
          <p>The major finding of this paper of 295 middle age patients undergoing open-heart surgery was unexpected. Of the four major types, only the use of “petitionary” prayer was related to better postoperative well-being, mediated through optimism, whereas “conversational” prayer was associated with poor outcomes, mediated through acute stress [<xref ref-type="bibr" rid="B136-religions-03-00467">136</xref>].</p>
          <p>The results of another trial in IP were less optimistic: IP itself had no effect on complication-free recovery from coronary artery bypass graft (CABG), but certainty of receiving intercessory prayer was associated with a higher incidence of complications [<xref ref-type="bibr" rid="B123-religions-03-00467">123</xref>].</p>
          <p>Lower pre-operative exhaled NO and its reduced response to nitroglycerin accompanied pulmonary vascular dysfunction in patients after open-heart surgery [<xref ref-type="bibr" rid="B137-religions-03-00467">137</xref>]. The possibility of using exhaled nitric oxide dynamics to investigate the effect of prayer in cardiac patients with pulmonary endothelial dysfunction merits further studies.</p>
        </sec>
        <sec>
          <title>2.17.1.5. Alzheimer’s Disease</title>
          <p>Spiritual practices are meaningful activities to connect with patients’ personal faith, important memories, and present support. Prayer was the most important spiritual practice used for coping by patients with Alzheimer’s disease (AD) [<xref ref-type="bibr" rid="B138-religions-03-00467">138</xref>]. Prayer provided a personal connection with God, produced beneficial effects of emotional healing, reassurance, and hope, and was important to persons with Alzheimer’s disease because it provides a sense of control over the progressive cognitive impairment [<xref ref-type="bibr" rid="B139-religions-03-00467">139</xref>].</p>
          <p>Another important spiritual practice used for coping was engagement in church activities, which provided vital connections with other people and possibly a sense of belonging and identity for the individual with AD. Since AD interferes with verbal communication and comprehension of conversations, individuals with AD often become socially isolated. In addition, losing one’s memory also means losing one’s self-identity. Being a member of a faith community is an identity; therefore, it may provide a sense of belonging and offer opportunities to break the social isolation.</p>
          <p>Interestingly, the two participants in this study who spoke of feeling isolated also no longer attended the churches that they had attended for many years. Notably over half of the study participants continued to serve in church roles fulfilling their need to be useful and productive [<xref ref-type="bibr" rid="B138-religions-03-00467">138</xref>], differing from Snyder’s participants [<xref ref-type="bibr" rid="B140-religions-03-00467">140</xref>] who reported decreased attendance.</p>
        </sec>
        </sec>
      </sec>
      <sec>
        <title>2.18. Belief and Expectation: Their Role in Health and Disease, Placebo Effect</title>
        <p>An objective identification of a potential religious placebo effect and its extent are extremely important. It is likely that religious/spiritual activities may provide a greater placebo effect than conventional medicine, but at present there is little empirical supporting evidence. There are certain similarities between the placebo and religion/spirituality effects in health and disease. These include the ritual of treatment (<italic>vs. the ritual of a religious service, regular treatment </italic>vs. <italic>regular spiritual practice</italic>), the clinician-patient (<italic>clergy-patient</italic>) relationship, and the hope for relief that requires either a self-generated cognitive/emotional response (<italic>vs.</italic><italic>intercessory prayer</italic>) or the intervention of others (<italic>physicians </italic>vs<italic>. clericals</italic>). Although there is no "real" drug involved in both (placebo and religion/spirituality), the actual positive consequences of their “administration” may well be physiological, emotional, behavioral, and/or cognitive, and real.</p>
        <p>Both of them need the emotional trigger of hope for relief in order to activate internal healing mechanisms; and both of them require the intervention of others, rather than self-generated cognitive/emotional responses, additionally, the spiritual effect can be generated independently of other people. Part of why the phenomenon of the spiritual effect on disease and illness has been relatively neglected within biomedicine is that the latter focuses on a biological conception of disease that is treated by technological interventions (drugs, medical procedures, implanted devices, and surgery). The doctor-patient relationship, as a means of treatment, has also been largely ignored by scientific medicine [<xref ref-type="bibr" rid="B141-religions-03-00467">141</xref>].</p>
        <p>We suggest that a new way of thinking and considering the therapeutic equality of words and rituals and real drugs may have profound implications both for clinical trials and for medical practice. Indeed, recent research has revealed that words and ritual-induced biochemical and cellular changes in a patient's brain are very similar to those induced by drugs [<xref ref-type="bibr" rid="B142-religions-03-00467">142</xref>].</p>
        <p>The “spiritual placebo effects” are not chemically generated and are due only to the subject's optimistic belief and expectation that the spiritual interactions will produce positive consequences. It has been shown that the expectation of reward (in this case, the expectation of clinical benefit) [<xref ref-type="bibr" rid="B143-religions-03-00467">143</xref>] influences the clinical benefit associated in both <italic>verum</italic> and placebo groups [<xref ref-type="bibr" rid="B144-religions-03-00467">144</xref>].</p>
        <p>The major mechanism of the placebo effect is the patient’s belief/expectation that this medication or procedure will help them with their disease [<xref ref-type="bibr" rid="B145-religions-03-00467">145</xref>,<xref ref-type="bibr" rid="B146-religions-03-00467">146</xref>]. </p>
        <p>The placebo effect also depends on the informational context. Lower expectations, when patients are told that they may get either a drug or a masked placebo [<xref ref-type="bibr" rid="B147-religions-03-00467">147</xref>], will reduce its effect. In contrast, higher expectations, when placebos are presented deceptively as a known beneficial [<xref ref-type="bibr" rid="B148-religions-03-00467">148</xref>,<xref ref-type="bibr" rid="B149-religions-03-00467">149</xref>,<xref ref-type="bibr" rid="B150-religions-03-00467">150</xref>] or more expensive [<xref ref-type="bibr" rid="B151-religions-03-00467">151</xref>] treatment, increase its effect.</p>
        <p>Spiritual experiences and practices involve a variety of physiological mechanisms that are comparable if not identical to those engaged in placebo responses. It has been suggested that meaningfulness seems to be both a hallmark of spirituality and placebo reactions [<xref ref-type="bibr" rid="B152-religions-03-00467">152</xref>], suggesting that spirituality, under certain conditions, may be a predictor of placebo response and effect. Therefore, it may be regarded as an overarching psychological concept that is important in engaging and facilitating psychophysiological mechanisms that are involved in health-related effects of spirituality. </p>
        <p>Development of novel objective techniques of religiosity and spirituality assessment, is not merely of theoretical interest, but may also be clinically relevant. Caution needs to be exercised before applying these techniques in patients, for both secular and religious orientation, as for some patients, focusing upon their spirituality may become an additional source of distress and anxiety.</p>
      </sec>
    </sec>
    <sec sec-type="conclusions">
      <title>3. Conclusions</title>
      <p>Research increasingly demonstrates that religiosity and spirituality are associated with measurable and clinically significant impacts on people’s health. The majority of end-of-life communication between the physicians and patients, especially with those diagnosed with life-limiting illnesses, are under-addressed by clinicians [<xref ref-type="bibr" rid="B77-religions-03-00467">77</xref>].</p>
      <p>There are well more than 100 instruments in the published literature to study religion and spirituality [<xref ref-type="bibr" rid="B153-religions-03-00467">153</xref>,<xref ref-type="bibr" rid="B154-religions-03-00467">154</xref>]. A total of 564 research studies were identified in nursing research journals for the period 1995–1999, of which 12% included at least one measure of spirituality or religiousness [<xref ref-type="bibr" rid="B154-religions-03-00467">154</xref>]. The majority of these studies (although only 19.3%) were qualitative and contained a measure of religion or spirituality, compared to 9.9% quantitative studies.</p>
      <p>There are several major and common limitations of these developed tests:</p>
      <list list-type="bullet">
        <list-item>
          <p>Subjective, with motivated response tendencies and use of various analogue response Likert-type scales [<xref ref-type="bibr" rid="B155-religions-03-00467">155</xref>], affected by demographic variables [<xref ref-type="bibr" rid="B155-religions-03-00467">155</xref>].</p>
        </list-item>
        <list-item>
          <p>Difficult and lengthy assessment with questionnaires ranging from 12 to 98 questions [<xref ref-type="bibr" rid="B156-religions-03-00467">156</xref>,<xref ref-type="bibr" rid="B157-religions-03-00467">157</xref>], which could be problematic in initial contacts with acutely ill patients [<xref ref-type="bibr" rid="B158-religions-03-00467">158</xref>]. Direct questions on matters of spirituality or religion may be viewed as intrusive or totally inappropriate [<xref ref-type="bibr" rid="B159-religions-03-00467">159</xref>].</p>
        </list-item>
        <list-item>
          <p>Influenced by culture, ethnic and other factors. Thus, the psychometric properties of the STS [<xref ref-type="bibr" rid="B160-religions-03-00467">160</xref>], SELF [<xref ref-type="bibr" rid="B161-religions-03-00467">161</xref>], SOI [<xref ref-type="bibr" rid="B157-religions-03-00467">157</xref>] tests have been shown to be reliable with American, but not with the samples outside of American culture (in Indian students and teachers, for example, due to a mixture of Hindu, Christian, and Muslim participants).</p>
        </list-item>
        <list-item>
          <p>Lack of standardization, reliability and validity studies, including cross-cultural and language validation.</p>
        </list-item>
      </list>
      <p>However, there is little convergence of tests used across studies and the absence of any objective non-invasive biomarkers assessments.</p>
      <p>This is the case despite the fact that most patients desire to be offered basic spiritual care by their clinicians.</p>
      <p>The vast majority of the studies investigating the connection between religious involvement and good health are correlational, use inadequate measures of religious commitment (e.g., religious denomination or other single-item measures), and seldom assess the intensity of religious commitment [<xref ref-type="bibr" rid="B4-religions-03-00467">4</xref>]. Nevertheless, a beneficial, or salutary, effect for religious or spiritual involvement on physical and mental health status is clearly shown. Professional medical and clerical associations and educational institutions are beginning to provide clinicians information on how to incorporate spirituality into practice [<xref ref-type="bibr" rid="B162-religions-03-00467">162</xref>]. Deistic, agnostic and atheistic people may have various and quite different lifestyles and moralities compared to their Christian counterparts.</p>
      <p>In fact, it has been shown that a Christian and an atheist would react differently when presented with a concept like God, and this will be paralleled with brain activation [<xref ref-type="bibr" rid="B163-religions-03-00467">163</xref>]. The results indicate that the anxiolytic power of religion, as reflected by the reduction of the error-related negativity (ERN), a neural signal biomarker derived from anterior cingulate cortex, is clearly seen in believers. </p>
      <p>In contrast, priming non-believers with religious concepts causes an increase in ERN amplitude [<xref ref-type="bibr" rid="B163-religions-03-00467">163</xref>]. This would seem to suggest, from an atheistic point of view, as it was expressed by Stenger in his book “God the failed hypothesis” [<xref ref-type="bibr" rid="B164-religions-03-00467">164</xref>], a power of the mind that goes beyond known physical capabilities. It is agreeable that this would also emphasize the need for a series of quantitative, repeatable (under similar conditions and by different and independent investigators), performing the study without any prejudgment of how the results should come out [<xref ref-type="bibr" rid="B164-religions-03-00467">164</xref>].</p>
      <p>To conclude, we identified several religious and spiritual factors and mechanisms, associated with different human conditions and symptoms in health and disease. This review is the first attempt to identify several potential “religious and spiritual” non-invasive biomarkers in different patients and healthy subjects. </p>
      <p>Considering that the multidimensional character of most concepts and phenomena in spiritual and religious matters are complex, one-dimensional instruments/approaches may not be sufficient for most research purposes.</p>
      <p>Research into spiritual and religious profiles by using multiple exhaled biomarkers is at its early stage. Although there is a clear increase of the number of publications on religion-mortality, new research should include the analysis of more dimensions of religious involvement, including religious life histories; different race, ethnic and socioeconomic groups; correlation with a wider set of social, psychological, and behavioral mechanisms by which religion and spirituality may be related to health and disease. We also suggest that such an approach may lead to more individualized treatment options in the future.</p>
      <p>Therefore, the following may be recommended:</p>
      <list list-type="bullet">
        <list-item>
          <p>Within a single study, use multiple biomarkers to build up an individual “spiritual profile” of a patient.</p>
        </list-item>
        <list-item>
          <p>Considering that the multidimensional character of most concepts and phenomena in spiritual and religious matters are complex, one-dimensional instruments/approaches that assess these constructs as a global entity may not be sufficient for most research purposes.</p>
        </list-item>
        <list-item>
          <p>Investigating spiritual and religious profiles by using multiple exhaled biomarkers and integrative approaches, such as metabolomics for the systematic study of the unique chemical fingerprints found in specific cellular processes, should be considered.</p>
        </list-item>
        <list-item>
          <p>Considering the need to study fast changes in spiritual and religious profiles of patients receiving treatments, a single marker, for example exhaled nitric oxide, can be used and measured on-line with an individual portable instrument, for example with NIOX MINO.</p>
        </list-item>
        <list-item>
          <p>If an investigator is going to use tests in a study, then consideration should be given to using multidimensional measurements. Ideally, profiles of exhaled biomarkers should be combined with psychometric tests and with other operational measures (e.g., behavioral observation, structured or semi-structured interviews) so that studies utilize a multidimensional approach to gather data for any given construct.</p>
